Can the signature be electronic but just my name in italics. This makes the signature different from my go to signature design. Will this be an issue? Thanks
@@meesumrajani423 I think the date will be the date of submission. The date when you are actually submitting the application, not any dates prior to that.
One have one question, on my passport it’s just my given name and there is no surname, so in my consent form what i will enter in my first and last name

the stream next to your file number and your invited stream has to be same for obvious reasons. And that goes in consent form as I have explained.
guys make sure it is correctly dated as this form is taken really serious.
