  • The aging process of the face is influenced by four key anatomical and physiological changes:
    1. Skin Elasticity and Texture: One of the most noticeable changes in facial aging is the decline in skin elasticity and texture. This is primarily due to the decreased production of collagen and elastin, two essential proteins that provide structural support and elasticity to the skin. Additionally, the skin's ability to retain moisture diminishes as levels of hyaluronic acid decrease, leading to a dry, thin, and crepey appearance.
    2. Volume Loss: Facial ageing is marked by a reduction in subcutaneous fat, particularly in the midface region, such as the cheeks and temples. This loss of volume can result in a hollowed appearance and contribute to the sagging of overlying skin. Additionally, the resorption of bone, particularly in the maxilla and mandible, further contributes to a reduction in facial volume, altering the overall contour and projection of the face.
    3. Muscle Changes: Over time, the muscles of facial expression can undergo hypertrophy and increased resting tone, particularly in areas prone to repetitive movements, such as the forehead, glabella, and around the eyes (orbicularis oculi). This contributes to the formation of dynamic wrinkles, such as crow’s feet and frown lines. Conversely, other facial muscles may weaken, leading to ptosis of the lower face and neck.
    4. Soft Tissue Sagging and Redistribution: As facial ligaments that anchor the skin and soft tissues lose their integrity, combined with gravitational forces, there is a downward migration of facial fat pads and other soft tissues. This leads to sagging in areas like the jowls, nasolabial folds, and marionette lines, creating a tired or aged appearance.
Understanding these four key changes-loss of skin elasticity, volume loss, muscle changes, and soft tissue sagging-provides a comprehensive overview of the facial ageing process and aids in the development of targeted rejuvenation strategies.
