      Dr. Scott Mosser, Transgender Top Surgeon I had the surgery here in Cincinnati! My surgeon was great just failed to mention this process! I did a bunch of research and found that a lot of people said that they panicked too until it was explained that that is normal! I did document my experience on my channel! I too wanted to help guys dealing with gynecomastia understand the process!

    I just realized this video addresses a tranagender population. I still want to thank you, I learned more here than with ny surgeon.
    If I may, I had a breast reduction (G to C cup) and two months after surgery and one nipple is still onberted while the other one is barely out :( I feel I've lost my femininity 😢 plus the pigmentation and puffiness is horrible.
    Thanks for your video, patience is all I have and hope for the best.

    Thank you so much, this is extremely helpful!! I am 2 weeks post-op and my nipples look so crusty and miscolored, I'm glad to know that's what it should look like and it will flake off on its own.

    Thank you so much! I’ve been insanely anxious about the occasional yellow stuff on my gauze pads when I change them. I am relieved but still anxious, mostly because I think I’m going to screw this up.

    Hello! Thank you so much for this video!
    This isn't something you expect when you get top surgery and my surgeon didn't go into detail with how it would look like during the first month, so naturally I panicked when my nipples started to look like dry bacon! I remember sending pictures to my surgeon and I was very surprised -yet relieved- when he told me they were doing amazingly! I also found your video while anxiously waiting for his answer and it helped me quite a bit with the paranoia.
    I'm now 2 months post op and my nipples are way past the stressful scabbing process and they look amazing! I'll send your video to every single person who's about to get top surgery so they don't get too anxious about their grafts.

      How's the pigmentation? I'm at two months and one nipple is still inverted 😢, the other I can barely see it. Some parts of the areola are puffy.

      What do you mean by "inverted"? I don't think nipples should be inverted or invisible. Could you call your surgeon to be sure? It's hard to tell without seeing pictures.

      One nipple is not out, at all. It is flat with the areola.

      Oh, I completely get that! English isn't my birth language either.
      We're your nipples flat before the surgery! I know that some people have "inverted" nipples naturally. Where are your areolas puffy exactly? Around the incisions or at random spots? I had some puffiness at a random spot and it was nothing. I think I'm starting to understand your situation. My nipples were also completely flat for the first few months because they were somewhat lifeless. As time went on they started to react to the weather again.
      Could you get into more detail about what it looks like? Can't you see them at all? I still suggest contacting your surgeon if you have concerns.

Tomorrow I am 4 weeks post operation but I don’t know “how to determine if my body has accepted my nipples etc” apologies I don’t really have any support from my surgeon etc. thanks again for this kindly please keep going and making more videos you are saving lives and shaping mind sets (around pre or post surgery concerns etc) you are truly amazing

    I feel a bit less concerned about them now, I had surgery 10 days ago & what you described is what they look like, they are not swollen but have pink around the nipple is that also a part of the healing process?

      It's always best to check with your surgeon, but it can take up to a year or more until the final healing of your nipples is complete and the redness around the areolas has disappeared

    How often should one wash the nipple area/change the padding around them? Does too much cleaning damage the grafts? Or is it just best to leave the area?

      We recommend that our patients do not shower until after their first post-op visit. At this time, the bolsters will be removed and folks are free to shower and let soap & water run over the nipple grafts. However, folks should still not scrub the area. In the time between surgery & the post-op visit, patients do not have to do anything to the area. After the post-op, folks will be given instruction on how to care for the area.     Could you possibly explain in another video the possible complications of post-top surgery? Because I am 2 weeks post-op and I have a very small part of my scar that's a little green, my nipples have turned black, I had an abscess appear one side of my chest and now both sides are very hard under the skin, all of this already treated and explained to me by my doctor now. However, my surgeon hadn't explained these possibles problems that can occur after surgery, so I think it would be helpful for others who want to do top surgery to know what to expect and not to panic like I did xD thanks again!

    I'm so glad I came across this video, I have been anxiously looking this stuff up. Question, is it ok if there is a smell? Not like an infection, it smells medical. I have read that a lot of people can smell it too, I'm just worried about what it is. Is it possible I'm smelling the scab and the fluids?

    I see so many videos with info on regular DI but hardly anything on keyhole recovery. Can you explain the healing process for that considering it’s a different process with how the nipples are preserved? For example, I am currently 2 weeks post op, scar tape is still on and in fact. I was able to shower and remove everything after 48 hours and never had any nipple bolsters for obvious reasons. Had my 1 week post op last Tuesday and was told everything is fine and healing well just needed to wear the vest 24/7 for the next for weeks. But now my nipples have a thick black crust ( I’m assuming it’s scabbing ) around the perimeter where the incisions were made. My doctor used glue instead of stitches so that also makes a difference. I’ve tried reaching out to my doctor and her team for more info and help but they do not respond at all as she told me during my app that her nurses rarely ever relay messages to her so she hardly is able to keep in touch with her patients due to this.

    I'm two weeks post op, I have Vaseline on my nips and when changing the dressing the scabs already fell off but now my left nipple (who was an inverted nip before) is looking good except the tip part that was inverted before
    It's looking a bit yucky with yellowish oozing and a scab/brown part still in the middle
    It's very distressing to look at

    I have a lot of bruising on my left side. I’m 13 days post op and my left nipple started to ooze blood (I saw the yellow fluid first, then noticed my shirt was bloody later on). Should I contact my surgeon?

      Yes, anytime you are experiencing changes with your body it is best to let your surgeon & PCP know (if seeing one regularly). It is likely that you are experiencing normal bruising and fluids but is best to check in.

    I’m 10 days post op, and I had taken the bandage off my nipple grafts. The nipple was kind of stuck to the gauze and pulled off some dry blood, which made a small spot bleed fair amount. (It stopped after applying a bit of pressure and a new bandage) I’m....grossed out and scared since tomorrow I can officially begin applying bacitracin to them.

      That sounds like a very normal tear in skin after removing a bandage. Check in with your surgeon as soon as you can if you continue to experience bleeding and mostly hope our video here was helpful in assuring you that your chest is recovering.

      I am 11 days post op and this keeps happening to me. I take off the nonstick bandage and scabby pieces of the top layer get pulled off. It bleeds a little and I can see the pink layer underneath. My nurse told me it was normal, but I’m still terrified because it’s been doing this since I was allowed to change the dressings 5 days post op. It makes me scared to change the bandages.

    Normally how long does it take for coloring to start showing I have a little on 1 and none on the other and almost a month after surgery

      Snook Coleman this is a great question! Uneven nipple coloring (or in your case differing nipple color) is called "dyspigmentation" which can occur any time
      pigmented tissues are operated/grafted. Most patients slowly regain pigment
      uniformity a year or more after surgery. Some patients never fully regain the
      normal pigment. If pigment has not fully returned after 1 year, patients can get
      a tattoo to that area to restore pigment, but this is not very commonly needed

    I'm worried because I took a long shower and once I started patting it dry one started to crumble like a scab so I started picking some of the one were sort of hanging off I thought I would take the whole thing off but I noticed so pink skin and some light blood me thinking the scab wasn't ready to completely ready to scab off so last night I decided to see if it was ready by trying to pluck it all off but then blood squirted a little then it stared to bleed out it stopped bleeding but now I'm worried the pieces I did pluck off were actually part of my nipple. It's sort of kinda like a half opened jar to a lid right now. The office is closed and idk what to do.

    Is it normal for the sutures to have a little gap in between them? Like looks there is a pink gap in between regular skin and the skin graft one week after when the initial dressing is removed?

      When the surgeon removes the outer bolster dressing about 1 week after surgery, the grafts are a little bit delicate, but already have a new blood supply. During days 7-21 after surgery, the graft gains a lot of strength, and is quite resilient after 21 days. By 42 days (6 weeks) after surgery, the grafts should be fully healed and usually have the strength of non-grafted tissue.

    Must not have done this on black guys. Pink is not something pleasant to look at on my skin and one of them stayed that way, but blotchy with pink. Got my top surgery in Nov. 2021.

    Im ten days post op and the original nipple graft looks like it just wiped off with the bandaid on one side and it’s just a whole lot of pink rawness is that good or bad ?

    What if I'm 4 weeks post op, and they both look completely perfect except one of them in the middle has a little bit of a sandy color of something that looks like it could be something crusty that'll fall off? My friend thinks it's infected but I didn't think about it till he mentioned it

    I have been trying to reach my surgeons office but no one is answering. I have gotten the dressings changed today (10 days post op) and 2 nurses said 2 different things. One thinks my nipples are dead and the other said it will be fine because it bled a bit meaning its alive.. Is it normal its black?

I could still not contact my surgeon dirrectly as i didnt have his personal number and his office still dont answer. As for the nipples they are now completely fine. Im close to 2 months post op and still have not gotten a post op appointment lol. Thank u so much for taking the time to answer i really appreciate it.     If the nipples are flat right when the bolsters are removed, will they perk up at all during recovery? I am worried that too much of my nipples were lost.
