Why I Left Medicine... Forever

  • So I've been agonising about this for a few months now, and I think I'm at a point where I can comfortably say that I'm leaving medicine for good. In this video we're going to have a bit of a chat about the thought process behind this decision, and I'll share my thoughts around the main fears that I had after investing 8 years of time and money into it. Maybe I'll change my mind and come back one day...who knows?!

      I would read that so fast! I've watched you for a couple years now, and you have been integral in my career as a fitness coach, and now as a software engineer.
      You and I are similar in the sense that we love to teach and help others live more fulfilling lives. Now I'm pursuing that freedom so I can do that too! Hopefully I'll be up there with you someday, helping thousands and millions of people make the most of the opportunities they have in front of them. I would consider myself lucky.

    I'm only 30 & I've switched careers 3 times, but I've never been happier. Life is too short to not do what you love, or to find your purpose. Props to you Ali for standing up for what you want & going for it.

    I'm a teacher at a public school and I'm just disrespected every single day. While I do still love teaching, it's just hard to go through these mental breakdowns. I've been thinking of teaching privately and wanting to really invest time on blogging which I started in the past but stopped for X reasons. Watching your videos are really making me consider it once again. I bought the book you recommended SHOW YOUR WORK and I can't wait to read it!

    During my med school applications I always come back to this video to make sure that medicine is what I want to do and even now I still come back to watch the video, there are many valuable information and lessons just in these 40 minutes.☘

    Thank you so much for your vulnerability in discussing this. I recently left my profession as a nurse practitioner to pursue growing my own TH-cam channel doing patient education on a scale so much larger than I could seeing patients and I’ve felt ALL of these things. It’s so validating to hear someone I respect a lot battling through all of the same things. Excited to see you figure out what’s next! 🍀

      After 20 years of physician job in USA, I took early retirement at age 50 when my investment return was about 5 times of my job income. It was not easy decision to leave a job I love. But a life of self-reliance is more rewarding experience. All physical illness and mental stress related issue vanished after couple of years of retirement. Meanwhile investment wealth keep growing with snowball effect. Life is good.

    I have just made the decision to quite school to pursue a business venture and it was one of the scariest thing i've did. i am so grateful for this video and for how open you are about this career change. personnally, i was terrified of what others would think of me. I've always be far more concerned than i should about others' opinion and social status but seeing you be brave enough to follow your own heart gave me the courage to do it too. So thank you for all you are doing. You might not have saved my life per say but you definitely saved my hopes and dreams. you've changed my vision of life and the future for the better.     That moment when he said, "If you're a medical student and you want to unsubscribe, that's totally cool," I felt so touched. When I was trying to learn how to manage the stress of medical school, I saw one of your videos and that really provided useful tips. Since then, I've turned a big fan, and I don't really care if you're a doctor or a TH-camr or you are just chilling in Maldives all day making videos about it.

    I love the part at the end where you weren't afraid to own up to your decision and addressed the person who said the only reason they were watching you was because of you being a doctor/TH-camr. It just shows how powerful we can be if we stop caring about pleasing other people or living for them. Love your authenticity!

    Thank you a ton for this video. I'm a 3rd yr medical student currently at a similar crossroads to you. Every point you brought up in this video has crossed my mind in the past 6months. I genuinely believe teaching is a more noble, impactful and rewarding career than Medicine. The community I live in is especially in need of good teachers. I haven't quite worked up the courage to leave Medicine and take that leap. I respect how your views on practising Medicine have changed over time and hope to have my own story to tell one day :)

      I’m sorry but I’ll have to respectfully disagree, I get it you guys hangout in the same circles and there’s a general sense of grandiose attached to the work you do - but plis! You’re not saving any lives. You’re not somebody revolutionary - there are thousands teaching the same thing, preaching the same thing. Yes this makes more economical sense for you - congrats. But don’t lie to yourself to belief that your work has more impact than actual scientists, doctors and engineers.
      You guys are ENTERTAINERS - who try to be informative - while making tonnes of money. THATS IT. Accept it for what it is

      @@saswatsatapathy658 they don't claim to be better than being doctors though. If you watch Ali's interview with Lewis, you might have a better perspective on this situation, and why Lewis says that perhaps for Ali it's better to follow his calling to a different path in life :)

    A thing I wonder about for many of your videos, especially this one, is do you use these frameworks to make an important decision or do you just use your gut to make the decision and then justify it using these frameworks?
    Over the years I've collected a lot of frameworks for decision making but in the end I tend to go towards my gut when it really matters. I'm considering changing that habit.

      I don't know about him, but in my experience every important decision I make, so not something which is part of my daily routine or I do subconsciously, starts from a gut feeling, which is my subconscious telling me things aren't right. Then I dig into that feeling and rationalise what's going on and make changes accordingly. Sometimes the choice you would make based on your gut feeling is rationally the best option, others it's not. And if you wonder how to properly rationalise, my suggestion is to look deeply inward and try to come up with something which works best for you. No matter how exhaustive and efficient some framework might be, you need to have something matching the way you and only you can know for sure.

      Normally gut for small decisions, but for big ones like this I actually use the framework :)

      interesting question. I think the idea of having a framework is that at some level it helps your gut to make the decision.

    Im a doctor currently and having this exact conversation with myself. Thanks for making this video man. And honestly once a doctor always a doctor. You did the work to get that title. You may not be able to practice but you earned that title.

    I feel like during quarantine, people started to make decisions based on what they truly want. Like this is the best time for us to realize more of ourselves.

    I am 1 month away from becoming a certified doctor and I have decided to quit medecine after I graduate , at least for now . It's just not for me and I realised it a few years ago but I kept pushing in the hopes of me changing my mind but also because of how scared I was to walk away from somthing I dedicated 7 years of my youth to , watching this video was kinda healing because it felt good to hear the thoughts and feelings I couldn't explain myself being explained in a very thoughtful way , thank you so much .

    I am an artificial intelligence engineer (bachelor + masters = 7 years ) & now studying veterinary medicine (5.5 years)! For me, I will be happier helping animals than sitting in a cubicle programming for the rest of my life...I will also be using my AI skills in starting a business combining medicine and AI in many ways....I also have started my own youtube channel as well!..Life is crazy and I am determined to find my life's purpose! let's go ...

      woah. yep i get you. cs here to with ai ms. but i'm thinking of leaving. the biggest way you can help animals is by leaving them off your plate and out of your clothing. let's see if you problem solving brain and human in you can figure this out.

    I’m at a breaking point right now. Everything is so overwhelming. Being a doctor was never for me but because of income problems I had to choose med school, I’m in my third year and I can’t stop thinking that I’m spending my youth for a job that I don’t want. I wanted to be a scientist. I was in love with the idea of learning about the universe. But what I’m doing right now is “memorizing drug names”. I don’t know yet but I’m considering going for a neuroscience phd in a better country after graduation. Actually if I had a choice I would leave everything behind and travel. The world offers too much to only stick around the same area. Wish it was not this hard…

      I totally know what you mean because I'm going through something pretty similar. I had to go for med school because I had no more options and I basically was chasing a lifestyle and not a passion since I knew I could be able to make enough money to live pretty well in the future and tweak some things. But the truth is that there's so much out there I wanna do and medicine is soo freaking time consuming and overwhelming it makes me feel like maybe I'm not doing things the right way and I should risk it all and stop trying so hard to be successful on people's eyes and live up to their expectations and society's, and instead just follow my dreams and figure things out throughout the journey

      Same problem :( i am third grade at med school. In our country doctors dont earn salary that we can call "enough for a doctor". For example an ophtalmologist says that he earns only 800$ :(( And there is violence against to doctors. There is 36 hour watch :( because of these watches doctor Rümeysa Şen has died in a traffic accident that she couldn sleep after night watch. Because you have to continue to work after watches. It makes me depressed about my future. I want to get speciality in UK bc at least doctors works in humanitarian conditions. And gets their salary well. But i am not good at english well. I have to get 7 points in ielts. I am sad so much :((

    I actually started loving this channel more because of this video because you are being authentically yourself. You don’t have to be a doctor to continue doing what you actually love and what makes you happy. This video as a pre med student is actually inspirational because so many people put themselves all into being a doctor and don’t even know who they are if they aren’t the person pursuing a medical profession or a future doctor or a current doctor. I think it’s admirable that you were a doctor and realized you didn’t have to stay in a profession that didn’t satisfy you.

    Ali, this video has warmed my heart so much. The amount of care, effort, & time you have put into this decision and then sharing this with others, is truly special. I can imagine how difficult this decision must have been.

    I left teaching after 26 years. I wanted to leave for years and finally could financially but I waited too long. It was way too much work for the pay. I lost too many weekends with my family planning lessons and grading papers. I hope you inspire many young people to think about how they want to spend their time and to not let fear drive their decisions. 💛

      @Tanya I am slowly getting into tutoring. There are companies that run coordinate online tutoring and local tutoring that I have used for my kids. Their calculus/statistics tutor is in San Diego and we live north of Los Angeles. If you teach math or science you can make over $100 an hour. Of course where you live matters but if it's online you can reach families on the coasts(in the US) and they will pay for good tutors. My other suggestion is corporate training or going to work for educational book publishers doing presentations and teacher training. Good luck!

    As a practicing physician myself, your thoughts resonate a lot with me as I've have mixed feelings about the modern practice of medicine and the impact we actually have as individual doctors. While I don't know if I'd leave clinical medicine if given the opportunity, I applaud you for making the brave step of forging your own path in life rather than cling to and live for the expectations of others. I appreciate your honesty and agree that you're going to be a doctor, your heart should be in the right place and you recognized it early and quit now rather than slowly hating it out of quiet desperation. I've been reading a lot of Seth Godin recently and I think most people are just too afraid to take risks and feel jealous or threatened when others take the chances that they secretly wish they could take. I wish you well and look forward to seeing where your journey takes you.

    Thank you for your vulnerability and honesty. I’m a nurse balancing a lot of other things and see me leaving bedside soon to pursue other goals. People have a hard time knowing that we leave something they’re emotionally attached to in the world that shows empathy…not realizing that changing and growing to enjoy life and therefore have more impact in more lives other than bedside may be just what we’re actually called to. It’s a journey and we’re allowed to make curves in the trails. Thank you 🙏🏾 and I absolutely love your videos and will continue to watch! How can I take your course! Lol 🍀

    I graduated med school in 2018 and just was so burnt out I haven’t done a single thing for the past three years. I’ve become mentally, physically, and emotionally broken during this time. But I would never take it back. At the end of this dark tunnel of depression has come the realization that I actually do love medicine. I went to med school because it’s what my parents wanted for me all my life. These three years have shown me that I actually do want to be a doctor. Not for my parents. But for me. So I’m thankful for this 3 year break. It means that for the next 35-36 years I will be doing something that I actually want to do. I appreciate that you took the time to make this video Ali! It shows that people can always change whatever they’re doing in life. And it’s ok to change your path when you want/need to.

      You sound so much like me! I also graduated med school in 2018, extremely burnt out. My mental health was the worst it's ever been, especially after I failed a board exam. I wasn't sure I could keep going and at one point had decided to give up. But after a break, I realized how much I loved and missed medicine and was so sad to think I'd never be able to practice in a clinical setting and decided to try again and I'll hopefully be starting residency next July.
      I wonder if Ali will feel the same at some point. I get why he doesn't feel like he needs it anymore though, doing something that causes thousands of people to message you about you how much you helped them sounds so much better than working as a doctor.

      Aww! I can totally relate to your experiences as I am in the same position myself and I really miss practicing medicine after a 3-year Break. Rn I decided to apply for residency next year since this year's applications already ended. Thank you so much for your comments, it made me feel less alone.

      I felt every word you said ! I've been in the same position for two years now , failed the residency entry test ( and 4000 other people , 3rd world country , residency spots are very limited) been through a depression and finally picking myself up , will retake the exam in two months .     🍀 Thank you for sharing! I honestly found this enlightening and really enjoyed listening to your experiences and thought processes. I'm at that stage in my own life where I'm really anxious about my future so this helped a little bit to process my own thoughts. I did start listening to you partly because of your medical background. It felt relatable. I worked as a RN in the hospital for 6 years and got so burnt out to the point where I was physically and mentally ill. However, your content sort of made me realize that there's more to life than just being a nurse. I struggled with the thought of being replaceable even though I was doing my best and I try to radicalize this fact.     I graduate med school and I am a midwife but while I was in med school I was volunteering and working with youth organizations. At some point the volunteering became activism and I realized that I am interested in more social aspects and human rights rather than medicine. And I was enjoying the things that I am doing for free besides medical school more rather than working in a hospital. So, now I am pursuing a degree in Political Science and I am quite happy with my decision. :) 🍀

      @@mikafizz1022 I felt the need to know more about governments, governance, political regimes, party systems, political movements and so on. That's why enrolled political science degree. But the field it's quite broad, so you can pick up stuff from a broad spectrum. I find that absolutely amazing. There people interested in more theoretical aspects, philosophy, social justice, comparative politics or purely methodology and research based stuff. So, a political science degree gives you so many opportunities in that sense.

      @TheEditor107 I think she meant that she is a registered midwife, Bachelor of Science in Midwifery is probably her undergraduate program. And she went to medical school and graduated. Idk how it works in your country, but in some countries, midwifery is a 4-year undergraduate program and can also be a pre-med.

Also a lot of these charities are paying workers huge amounts of money so we know the money is not going to good places. Literal former head of states work for charities getting paid 10s of thousands just for a speech for a so called charity.

    Hello. I am a doctor who left the clinical practice a few years into it, just like you did, and this resonates with me. I agree that most people do not get the "why" behind it all. I now work as a healthcare data specialist and a part-time pregnancy coach and it brings me more joy and fulfillment than the clinical service ever did. There are things only some are good at, and once you have found yours, why wait around? Wishing you the best!

      ​@@gwendolyneayiera5046 Healthcare data is a new and upcoming area that deals with a bunch of things including managing electronic medical records, medical terminology, etc... You can look into Clinical Informatics and Ontology to get an idea about this field. There are many parallel fields to medicine, but most of them do require clinical expertise, i.e a few years of clinical practice following graduation. Hope this helps!

    🍀thank you so much for sharing this Ali. You'll never understand the impact that this specific video has had on my life. I was in a similar situation a few months ago, balancing my mind between staying in a job for the money, status and societal approval, or quitting what I've been doing for 12 years+ (which I used to call my "life goal") to start a business. And this mental chaos really brought me to a terrible place mentally.
    Fast forward 8 months after making that jump and quitting; although it still fills my thoughts sometimes of what "could have been", I think I'm much happier now. The pursuit of something I actually enjoy and look forward to every day gets me out of bed excited about life. 🙏🏾

    Yes you saved me, because now I can quit too, quit being something I don't like, quit doing it just because I have spent 8 years of my life to build this career, when I'm neither passionate nor making close to what I could potentially make with my set of skills. I loved it when you said you don't regret studying for it because you enjoyed the journey, and its the journey that matters than the destination itself, I can truly resonate with that. I can finally quit the decision I made as a kid, and finally move ahead with my life with what I truly want.
    You might not realize it, if this video hadn't came, I don't know what I would have done with my life, probably suicide.
    And remembered that, you might have lost a few audience due to this decision, I assure you, just because of this decision you got even more committed audience like me.
    Thank you Ali, you got my support until the end of time! Do what matters to you, so that we could be confident in doing things that matters to us! I'm eagerly waiting for your next video to come out.

      8 yrs. may seem real long when one is young, & it is as a proportion of life lived, but as one ages & gains greater perspective (longer time line), one realizes 8 yrs. really isn't that long! Remember average life expectancy is 80 something years & nowadays, folks are averaging 12 different jobs.

    Just when I thought the video was going to be boring enough to cause me to leave, you really had a way to reel me back in; thanks to your genuineness and insight!☘️ I'm at an awkward age of 22 years old and still trying to "figure out" what I want to do with my life. You helped me realize that change and uncertainty is okay. You inspired me because I want to be a doctor too, but I am not sure if it is the right thing for me; and that's okay. I am inspired because if I do become a doctor, I know that I do not have to stay a doctor if the time comes for me to leave. I thank you for your bravery and your honesty. l thank you for your perspective because by doing so, you are helping other uncertain 20 year olds develop a clearer sense of the world we live in.     Speaking as a medical student myself, I still find the business aspect of things fascinating! For me the switch just shows that even if I go through uni and fall out of love with medicine, I'm not restricted to the one career path. All the best on the transition mate!

    I just wanted to express how grateful i am for this video. It showed up on my feed at a time i really needed it ( either a sign from the universe or Google just collecting and analysing my data really well). For context, I've taken up arts because I've been passionate about all things creative, but, a part of me which always did enjoy science and the whole process of learning associated with it became increasingly vocal and made me question a choice i had made with as much reasoning and research i could. Through you outlining not only the aspects of your life and medicine that made you quit medicine , but also the whole breakdown on what we look for in work helped me realise exactly what i am looking for long term.     Once a fan, always a fan. Personally, the prefix doctor never meant anything special for me. You're a good person, pleasant and interesting to watch.     This might actually be my favorite video of yours! There's so much shame surrounding doctors who decide to leave medicine. I honestly think pursuing something else that's better for you should be normalized. If people in other careers are encouraged to do that, then why can't we? Life's too short to force yourself to do something for the wrong reasons.

      April, I agree! there is a burden of responsibility with medicine that can be heavy to carry. We have other aspects of ourselves outside of medicine, for example, I'm really creative, I love art, business, finance. I can choose to pursue other interests

      True! It took about 4 years for me to feel almost comfortable and not ashamed about my decision to stop practicing medicine

    I’ve been at similar crossroads. Even family deter you from what you really want to do, because they’re scared you’ll fail. However, live your truth. I believe we know what we should do deep down and it’s distressing to deny it. You’ll be fine, more than fine. You’re why I watch your videos. You are unique, in a very good way :)

    I've always loved how genuine you are. You've always been an inspiration amongst a lot of other TH-camrs who have changed my life and I fully agree with your thought process and can see where you're coming from.     🍀 Your thought process is actually incredibly mature and well-planned. I don’t even watch you to see what you’re doing (medicine, productivity, etc), because that’s kinda superficial. I like watching because of the value you end up bringing to us no matter the area you’re acting in. I feel like that’s true success, when people don’t follow you for what you do, but for who you are.

    I just reapplied to med school for the 3rd time and finally realized maybe it's not for me. Always scary to walk away from something I dreamt of doing all my life (I'm 26) but I know I can find more fulfillment and make a bigger impact being a PA and continuing my business as a permanent makeup artist. I resonated with a lot of your thought process in this video and wanted to thank you. It takes maturity and courage to walk away! Proud of you and rooting for you!!! 😊

      I'm a recently graduated MD and I've failed to match into residency last year because I failed one of my board exams the first time I took it. I never failed anything else and have since passed but residencies are so competitive that programs don't even bother to interview me with their thousands of applicants. I often wish I did PA instead. I think PA is such a great job, most of the positives about being a doctor without most of the negatives.

      @@thelifecoachmedicalmodel6648 100% agree, so many thing need to change about medical school and residency training. We need to be treated like human beings, not robots that you can just program to work non-stop without needing rest or making any mistakes and who are super happy to do it all. And that's barely even the tip of the iceberg, so many things need to improve. I'm hopeful but it will take a very long time
