Surprise I don't know, but I see this look regularly when I have to wake up patients after they had their first sleepless night and 2 hours of sleep in the early morning in hospital, just to measure and document their vital signs. They often wake up with 75% of their brain cells still asleep, and have exactly this look, huge, confused eyes, mouth open, no tension around the mouth 😧.

His lips are so FABULOUSLY FULL without the fillers why the heck did he even consider it
Because people who don't like their body always focus on the part they see as flawed.
When people look at themselves in the mirror everyone focus on part they like or don't like and rarely see the body as a whole.
The best way to counter that is to ask someone (with good esthetic sense) to take pretty pictures of you, in various angles. Picture lie less than a mirror, mostly because you always look at yourself with the same "bird's eye view". Meaning the camera (here your eyes) is above the subject (your body). It's a very special perspective that can often distort the proportions.
While when you look at other people you see a variety of different angles, if you are sitting, standing, smaller or taller than the person, keep in mind that they will never be able to see themselves from your perspective without someone taking a picture or video.
I've been there too i have naturally full lips but i wasnt loving myself and i was insecure about my look so i was thinking about having lip injections but i didnt done it. After few years i realized my lips are already full.

They're not conservative. This is just being responsible. Cosmetic surgery is fine when you have a little of it done. But such drastic changes or a large number of changes is a problem and can absolutely be detrimental to a person's health.

If I am just being accurate, I can say that his lips are not that natural as it should be before the filler. But it still looks natural, but you can see a little bit of aura of the filler, you can really see that it is 'plumper'? I don't know if that's the right word haha.
