That wasn't the worst possible ending. Put the virus in the water and then get lyons to activate and you kill basically everyone in the wasteland except the fascists and yourself. Now that's pretty bad.

You can't even attack the brotherhood of steel or side with the enclave, so your choice to suddenly betray them feels cheap. This isn't the worst ending. The worst ending is if you blow up megaton and annihilate rivet city(and in my opinion making lyon do it to show how much you care just about yourself). And if you live, you can blow up the citadel turning the brotherhood against you. Yeah we're evil!

It was in the Brotherhood of Steel DLC. The DLC unlocks the option for Fawkes to activate the purifier and you can go do the BoS quests.
Peacemaker You couldn't even blow fawkes if you really wanted to, supermutants don't have genitalia.
You need the Broken Steel DLC.

Yes, but people exposed to even the smallest bit of radiation was considered a mutant and would be killed.
Actually as you weren't born in a vault the water harms you too.
If you done the survival book quest in Megaton to the max (take severe radiation sickness) you mutate from the Bramin milk.
That's the best ending in my book.
