1. Scenario:
A carpenter was working on an incomplete scaffold approximately 3 meters high, which was missing a mid-rail. While working, his tool fell to the edge of the scaffold. As he reached out to retrieve it, he lost his balance and fell. Fortunately, he was wearing a full-body harness with a double lanyard, which was anchored to the top rail ledger, preventing a fatal accident.
This scenario highlights the critical importance of ensuring that all scaffold components are properly installed and inspected before use to prevent falls. It also underscores the necessity of using appropriate personal protective equipment (PPE) as a secondary safety measure when hazards cannot be eliminated, substituted, or controlled through engineering or administrative measures..
2. Objectives of the Drill:
• Reinforce the importance of eliminating work-at-height hazards by ensuring a safe working platform.
• Emphasize the need for proper use of fall protection equipment and PPE.
• Evaluate the emergency response efficiency of the workforce.
• Assess communication effectiveness during an emergency
3. Observations During the Drill
a. Workforce Response to the Emergency
• Team members acted promptly to rescue the suspended worker.
• HSE professionals conducted an immediate assessment to confirm that no injuries were sustained.
• All site personnel gathered for a post-drill discussion to review lessons learned.
b. Awareness of Emergency Contact Numbers

• The emergency was effectively communicated to the HSE team and site management.

4. Recommendations & Corrective Actions:
• Scaffolding Compliance: Ensure all scaffolds are fully equipped with guardrails, mid-rails, and toe boards before use.
• Proper Anchorage: Reinforce training on using designated anchor points instead of top rails for harness attachment.
• Rescue Preparedness: Conduct periodic rescue drills to improve response time and efficiency.
• PPE Enforcement: Regularly inspect and enforce the use of proper PPE for all work-at-height activities.
• Communication Enhancement: Display emergency contact details at prominent locations for easy accessibility.
5. Conclusion:
The drill successfully demonstrated the importance of pre-use scaffolding inspections, appropriate PPE usage, and efficient emergency response. While the worker was saved by the full-body harness, preventive measures should be prioritized to eliminate such risks altogether. Further training and awareness programs will be implemented to enhance site safety and compliance.
