Jill is a Police Officer with 20 years’ experience on the force. As an 18-year-old, she did a ride along and was hooked. She prefers being a front-line officer rather than sitting behind a desk. Even though she had 4 different motor vehicle accidents while at work as a police office, had neck fusion to repair herniated disc, Jill loves what she does as a Police Officer keeping her community safe.
Jill suffered with jaw pain, headaches, facial pain, neck and shoulder tightness and fatigue for over 2 years. When she was outside as part of her job, especially during cold weather, all her symptoms were aggravated. She did not connect all of these symptoms since many of them could be related to her injuries. Her jaw problems were worsening and even locking up at times.
Jill had tried everything she knew of for her TMJ, jaw problems. Her dentist made a night guard. Jill tried chiropractic and Botox injections for her TMJ. None of that relieved her pain. Her dentist then referred her to an oral surgeon. Jill did not want surgery. Google search led to her discovering Dr Raman’s expertise in non-surgical, physiologic treatment approach and his decades of experience.
His clinical examination confirmed that she had jaw alignment “TMJ” problems and that is the root cause of her many symptoms even beyond the jaw.
Jill went through detailed diagnostic tests including our unique "Raman CCMD Protocol" of Physiologic Neuro Muscular Dental (PNMD) procedure to determine the optimal jaw and neck alignment. Balance tests revealed that her postural compensations were primarily due to the jaw alignment. So that linked her jaw problem to whole body symptoms such as fatigue.
All findings and treatment options were reviewed during the consultation prior to starting phase 1 treatment. She was then fitted with a physiologic neuromuscular fixed orthotic for 90 days as a reversible phase 1 treatment. This is both a therapeutic and diagnostic step. Even though phase 1 fixed orthotic is only on the lower teeth, like teeth extensions to make up for the jaw alignment discrepancy, there needs to enough of an improvement of symptoms to give the patient confidence to go forward with treatment or to give up and reverse course.
Jill started noticing improvements gradually. This is one of the important reasons that we use a PNM Fixed orthotic. It corrects jaw neck alignment truly 24/7, every second of the day. The key is how accurately the jaw / neck alignment was diagnosed first. That is the alignment that the fixed orthotic maintains.
Within 6 weeks of Phase 1 PNM Fixed orthotic, all of her symptoms were improving significantly. Jill reported 20% overall improvement of symptoms. Muscle palpations showed 17/44 painful points with pain score of 55/132 vs 34/44 and 91/132 pre op. Cranio-cervical range of motion 90 degrees to the right and 70 degrees to the left vs 2 degrees right and left pre op.
By the end of Phase 1 Fixed orthotic therapy, Jill reported 55% overall improvement of symptoms. Muscle palpations recorded 2/44 painful points with pain score of 25/132 vs 34/44 and 91/132 pre op. Even with neck fusion surgery history, Cranio-cervical range of motion 90 degrees to the right and 90 degrees to the left vs 2 degrees to the right and left pre op Reviewed top symptoms and level of improvement 1. Headaches - 55-60%. 2. Jaw pain - 60%. 3. Shoulder pain - 50%. Fatigue - 50%. Energy - 55%.
Jill was sure that she was on the right track to improve. Phase 1 proved that PNMD treatment was actually resolving her neck, jaw, and other symptoms, even though the upper arch was unchanged and narrow. They made the decision to move forward with Neuromuscular Functional Orthopedics / Orthodontics (NFOO)as phase 2 option. By correcting the upper arch, there will be more room created for the lower jaw and tongue which affects airway. That in turn affects quality of sleep at night and neck posture during the day. Jill was informed that correcting the upper jaw form may result in gaps. She was OK with that if that is what it took to improve her debilitating symptoms.
Once the upper arch was corrected, the lower teeth were brought up along with their bone and gum support, in sequence, to meet the upper teeth to maintain the optimal jaw and neck alignment. This takes about 3- 4 years to complete that treatment.
Jill is at 18 months of Neuromuscular Functional Orthodontics / Orthopedics treatment with braces. Reviewed top symptoms and level of improvement 1. Headaches - 70%. 2. Jaw pain - 60%. 3. Shoulder pain - 60%. 4. Fatigue - 71%. Upper Cervical Rotation was maintained at 90 degrees to the right and left.
The role of NFOO is to stabilize the optimal alignment of the jaw and neck by moving the teeth to that position. Jill will complete her braces NFOO treatment soon. She would have achieved optimal function and pain resolution at the same time for long term.
