- Due to supply constraints, some diabetes patients are not being able to resource their correct Ozempic dose pen device of 0.25mg or 0.5mg. So this video explains how to administer lower dose of Ozempic (0.25mg or 0.5mg) from a 1mg Ozempic pen device.

What happens though when you have 1mg leftover? They are good for 56 days (8 weeks), so if someone starts with 0.25 for 4 weeks then moves to 0.5 for 4 weeks thats 8 weeks and only 3mg used leave 1mg to be wasted?

The pen resets after each injection to 0. So you only need to do each time the same number of clicks, as mentioned in the video eg 18 clicks for 0,25.

I'm used to a 1 mg pen but all I have is 0.5 pen and I took one dose of 0.5 and one dose of 0.25 did I do that right?

Ozempic pens are like 1970's technology compared to Trulicity and Mounjaro. It's a shame they don't update the pen design.
Sadly Mounjaro pen is also going to be downgraded to same technology of Kwikpen globally. They already launched this in UK. I have already made a video on that and am going to release one on demo soon

I have the one pen of 1 mg I kept turning nonstop then I just injected myself. So hopefully next week I’ll just click all the way to the end for my next 1 mg. 🤦🏾♀️

I believe you may have taken 0.5mg by taking 36 clicks. This is the calculation
18 clicks = 0.25mg
36 clicks = 0.5mg
72 clicks = 1mg
You may bring your pen to clinic and I could check your technique directly to advise

Please always use a new needle as best practice since the old needle can be contaminated.
You can easily attached 4mm size BD insulin pen needles or Novofine insulin pen needles 4mm

The Ozempic pen you are using should be disposed of (thrown away) after 56 days, even if it still has Ozempic left in it.

Hello, thank you for your video! I am a bit confused with the clicks and the dashes, so why are we counting clicks and not dashes? Because 18 clicks is 15 dashes as I counted on my pen and this is really stressful if we are taking the right dose 😢
Yes the two clicks for priming the test shot are not coming as dashes so ignore them.
You get 0.25mg dose of semaglutide which is initiating dose.
There is a supply problem for the 0.5 mg pens and I already missed my initial dose which was supposed to be last Friday. The pharmacists told me that some people are buying in bulks and hoarding it for weight loss purposes and leaving us Type 2 diabetics struggling to get the supplies for our own medication. There are a few 0.25 mg pens being sold in some phramacies and I wonder if I can use 2 shots of it to compensate for a single shot of 0.5 mg without causing overdosage? Thanks.
I'm sorry to hear about your difficulties. Yes two doses of 0.25mg will equate to 0.5mg. Unfortunately you will need two pricks for injection

My random blood sugar tests in the last month yielded a range between 120-150 mg/dl and hopefully the increase in the dosage will yield much lower readings this coming weeks and good results for my next scheduled blood chem in December. Currently, the only pens available in stock are the semaglutide 5mg/3ml (1.66mg ml) Prefilled pens. Could you please possibly confirm how many clicks I would need for a weekly starter dose of 0.25mg.

If a break longer than 8 weeks happened from high dose then you may trial lower dose because it might be a good chance to get same response with lower dose. This also avoids unnecessary side effects with sudden starting off high dose

You have done the dose correctly... since the volume is very less so its not noticeable... wait for next three doses and it will become noticeable

If the dial didn't go to zero, it could be a faulty pen or you didn't click the button properly

Yes that's correct. Please ensure the storage is correct and the fluid doesn't get cloudy after 6 weeks. Discard use of the colour of fluid changes or your storage is not correct

@@hptjustme if you are first time starter then please start with low dose of 0.25mg. A drug should never be used as a scary supplement. Ensure you also speak to your endocrinologist. Never start on high dose. Go slow, go safe in stepwise way

You will start from zero every time you inject.
Using same needle is significant risk of cross contamination of pen device, injection site reaction, possible wrong dosing with needle getting blocked

This has been the off licence method of prescription in times of shortage and COVID and it worked well for the patients ensuring consistency of supply.
Usually when patients up titrate the dose after a month, the 1mg pen doesn't last that long
Please ensure pen is always in refrigerator if using this way and you always check the fluid in the pen is clear and colorless
Always seek your endocrinologist supervision as this is a drug and not a supplement

The 1mg pen is designed to give 4 doses of 1mg if you do the full dial (72 clicks)
After 4 doses of 1mg the 1mg pen finishes which means it will last you 1 month to take 4 doses from 1 pen of Ozempic 1mg... each weekly dose will be 1mg of you do the full dial which is 72 clicks
When you take 18 clicks each week to take 0.25mg dose then you are taking only 1 fourth of your normally weekly 1mg dose to be taken from this pen...
Forget the maths and remember that Ozempic 1mg pen gives four doses of 1mg for each week if you do full dial each time without any click counts... this will last you 1 month

There's nothing wrong with it but 4 pricks in one go are a bit too much I'd suggest.
You have to change injection site and use new needle every time. It will also be quite costly to do this way.

It's best practice to consume the pen within 6 weeks because the efficacy of molecule becomes questionable after 6 weeks.
But in real world experience during COVID pandemic and shortages, the pen has been used longer than 6 weeks under close endocrinologist supervision without any problems

why do Ozempic people seem to lose weight but not so much on Trulicity? thought they was the same. they not?
They are different medicines from the same class. Ozempic is Semaglutide which has better data on weight loss while trulicity is dulaglutide (a bigger molecule of GLP-1) with dosages adjusted mainly for diabetes control ....

As long as the fluid is colorless and not frozen and you are finding the efficacy there and you have not broken the cold chain... meaning kept it in for of your fridge and its not expired

Who would anyone want rapid up titration? I'd strongly discourage such practice. These are medications and not to be considered a magic wand or supplement.
Please try to use the minimal effective dose for maximum benefit.
Everyone plateaus at some point and if you rapidly escalate to high dose then you have no room for titration when reach a plateau.
Please ensure you are being supervised by your endocrinologist

It's best to restart with 0.25mg as you have been off for 3 months.

Ensure you eat small portion of food, eat slowly, hydrate yourself.
Take ondansetron preemptively to avoid vomiting.
Ondansetron 4mg to 8mg three times daily for today and tomorrow

It's the wrong approach and you develop tolerance to high dose this way.
Get the maximum benefit with minimum dose. 0.25mg to continue.
Ensure you have supervision of your endocrinologist to guide you.

@fatmehahmed9546 I personally discourage two injections same day as patients need to spare the sites and use alternate sites each week. Sometimes, I recommend taking 0.25 mg every 3 days to get steady state of 0.5mg per week. But that's only in exceptional cases. Why should one do it when taking 0.5mg once weekly would be a cheaper option if that's the recommended dose.
The only reason I could think would be tolerance issues with full dose of 0.5mg in one go and then I split the dose. But that's of licence advice for strict supervision under an endocrinologist. You can't take youtube as your endocrine advice as there's no responsibility in such conversations

18 clicks from 1mg pen mean that 0.25mg once weekly dose. The pen device can last 16 weeks.

Why do you want to use 0.5 mg twice? Are you trying to get 1mg dose.
If yes then you will be injecting it twice and should use new needle for new injection prick

You need new needle with each injection. 4 will only be enough if you are using 1mg pen to take 1mg dose

@@Sriwijaya888 you cannot buy two needles... it comes as a box of insulin needles 4mm size. Usually boxes can have as many as 25 to 100 needles... but buy the smallest number as you need only 4 more needles if using the 1mg pen to take 0.5mg dose since you will get total of 8 doses

18 clicks is 0.25mg dose. We normally advise this dose for whole month.

Yes I am on prescription for this but I have a hard time counting clicks. What I meant to ask is, if I am on 0.25mg dose per week for a month, and I purchased a 1mg pen, I can use it 4 times right? So if I use for the first injection, I will count 18 clicks I understand. But how about for the second use? Will I still count 18 clicks for the second 0.25mg or do I need to click 36? Then 54 for the 3rd time and 72 clicks for the 4th? Or just 18 clicks everytime? Thank you so much for replying
My doctor said I can use 1mg pen multiple times but did not give instructions on how to use by counting clicks
@myradejarlo448 18 clicks will give you 0.25mg dose. You can stay on this dose as long as good clinical response and have clinical assessment every 4 weeks

It will have 16 doses... it's preferable to maintain cold chain and ensure your drug isn't discoloured. 6 weeks is official usage but people have used longer without any issues. Please consultant your endocrinologist as this is not medical advice

Always start with low dose of 0.25mg... this is best practice and it's ideal to maintain on the lowest effective dose

I'm afraid to do 1 mg. Isn't this going to make the vial go empty with 72 clicks?
72 click is 1mg which is one full dial on the 1mg pen... that pen contains 4 doses of 1mg ... so 1 full dial will not finish the whole pen
