When you work a lot on a particular paper, you actually deserve the corresponding author along with a second author. If the idea belongs to you, you can claim the 1st author or the patent rights if that is a model. But an authorship always matters besides its chronological order, but claiming your order is your right. Any particular journal has a committee (usually the ethical committee) to always take care of or address such issues/confusions, if someone wants to logically claim. I hope this answers your question.

While some of the selected states in the USA (slowly, many states are planning to do the same in the near future) accept foreign residencies to practice as a clinician directly (but first, the particular candidate has to work with a university hospital which helps them acquire the state license to practice), all states compulsorily require the candidate to finish the USMLE steps to practice.
Yes, it does. And it depends on your goals. If your goals are to follow the best treatment protocols, then follow clinical. If your goals are to discover some ground breaking discovery, then follow and work with basic science. If your goals are to simply acquire residency or competitive residency/fellowships training, then simply follow the researchers who follow either of these.

No one cares, you are an international graduate period. Think about the options if you are sure about UMSLE then go abroad
