    I have never seen a channel this good at explaining what exactly happened on an aircraft accident. Take a look at Smithsonian, they charge you to watch an hour long episode that doesn’t even explain it throughly. Meanwhile this channel makes everything for free and explains it correctly. Gosh, I wonder how long these take. Good job and don’t give up!

      I immensely dislike any documentary that hires actors. An actor that remains silent and only recreates the actions taken for a visual demonstration is fine, but when they try to be emotional... that just seems extremely insensitive and for profit.
      Don’t put on a show, you have no idea what happened in there so don’t dramatize it for your own profit. Play any released audio from the cockpit, but don’t hire someone to pretend to be real people facing a life or death situation. It’s especially appalling when it was made with family and friends still alive to watch it.
      Wait 50 years for it to be a *little* tasteful, or at least have someone there to guide you throughout the production process. Have an air traffic control personnel that was there shape the depiction, they could tell you how a pilot was feeling and how they came across. Have people who knew the pilot help with their portrayal, so you don’t have first time actors giving exaggerated performances. It’s so distasteful how some companies can handle these events so poorly.

      Frontier has a great safety record though. I'm terrified of flying but chose to fly Frontier several times a few years ago. I'd take a breakfast voucher for the $25 seat I had lol.

      Not all that great....it is the crews fault this happened. The pilots walk around checklist includes checking the door latches. The pilots failed at their job.

      Pilots are humans, they arent perfect. They make mistakes. Its why planes have so much built in redundancy and ways to recover from human error. However, when the incident happened they reacted appropriately. Thats what matters. Not expecting perfection.

    The pilot was phenomenal! He listened to the flight attendants and reacted immediately in the proper way. Thank God for him and all in the cockpit. No injuries and ALL SURVIVED!!!!
    But, they all survived no thanks to the sloppy work of the maintenance crew. 38 issues like this!!! Why did Frontier not address this after ONE incident??? Luckily, no one was harmed. Totally avoidable!!

      @@Dumpy332, I'm a retired airline mechanic and I agree it may be a design issue, but it still goes back on the mechanic who last closed the cowling. If the mechanic is not in jail he's at least broke from all the fines from the F.A.A, because after a mechanic closes an engine cowling, he signs his name and FAA license number stating that he closed the cowling in accordance with the aircraft maintenance manual.

      I wanted to highlight that this may be Frontier's first issue. The number 38 is the number of issues occurring on A320 family aircrafts across the world.

      @@Dumpy332 The engineer is trained to work on the Airbus and its CFM engines and signs a document to confirm the work they did, including locking the cowling latches. The point is, the engineer is trained, knows where the latches are and knows they must be latched. So why do they sometimes not latch them? perhaps you can shed some light on this.
      All 38 incident investigations found that the cowlings had not been properly latched.
      These cowling issues are not confined to Airbus and have occurred on Boeing aircraft too.
      I think it is more likely a training or procedural issue because there must be a reason why the engineer would claim they have completed their work without actually completing the job by latching the cowling shut.

    And yet after all these incidents, there is still no requirement for an aural or visual warning in the cockpit about an ajar or open cowl, but on cars we have both for ages.

      It doesn't give me the chills anymore but it used to, so I understand. It's an efficient way of reporting. I suppose they could ask for bodies or people on board. The point is to report one number that includes revenue and non-revenue passengers, crew, adults, infants, everyone all in a single number.

      @@mbrock1741 Strange reasoning. I believe that your soul can exist connected with, or outside of your body. Wikipedia says: “The soul acts as a link between the material body and the spiritual self, and therefore shares some characteristics of both. The soul can be attracted either towards the spiritual or towards the material realm.” But this is more of a semantic and philosophical discussion. I've read somewhere that they ask about the number of souls, because planes sometimes carry a coffin(s) with a deceased person. So they only want to know the number of living persons on board they have to rescue in case of a crash, or to count as victims of the crash.

    I am releasing NTSB's report for them..."We have found that, due to a malfunction of someone's hand, the cowl latches were not secured properly. That lazy S.O.B has since been replaced with someone with a fully functional hand."

    Airbus have designed the cowl latches so that they are easy to detect if left unsecured. The pilot doing the walk around preflight inspection must be willing to (and is obligated to) get down and have a look underneath the cowls. I do this on each walk around as it is the pilots responsibility to conduct this check before each take-off.

      What kind of maintenance worker would work on an engine and not ensure the cowling is latched? Sure happened a lot. Engines are very sophisticated; how do they complete the work on an engine with precise specifications and forget something that seems pretty easy to do; latched the cowling.

    Worked Avionics for a number of years - over time you will see a lot of "lack of attention to detail." Some of these episodes could have resulted in catastrophes - like: Tape left over Static ports of the pitot-static system, or tape left over static ports of EPR systems (engine pressure ratio) Sometimes, a "second set of eyeballs" miss the problems, because they do NOT LOOK at the maintenance work prior to signing the work off. Should go to jail if lives are lost.

I can't imagine that anyone who has received military training or similar, (like civil aviation maintenance and flight crew?), was not told, ad nauseam, that "sloppiness kills". If a person has not positively verified that an item is correctly positioned their only conclusion must be, MUST be, that the item is NOT correctly positioned. "Safe" is not the default value.
      Two great positives: the professionalism of the flight crew and the ATC personnel, and, oh yes, the sheer brilliance of The Flight Channel.

    I've flown on Frontier once in my life. Coincidentally, the destination was Vegas. (BDL to DEN to LAS, return was LAS to DFW to BDL) My experience with them was excellent, the pilots actually greeted the passengers one by one as they boarded (this was post-9/11), the attendants were friendly, the planes were clean, I honestly could not find anything to complain about.
    I was really surprised, because until then, I had only flown Eastern and Delta/ASA, and as a result had low expectations. I was really happy to see a "budget" airline doing a better job than the more prestigious airlines!

    On wing mounted bypass engines it is very hard to see the latches without almost getting on your hands and knees. No excuse though as the A&P doing the nightly service check was responsible to assure they were latched.

      That's what the walk around preflight inspection is for...people aren't perfect, human factors will occur from time to time. 38 is a fraction of the 10's of thousands of flights that departed without incident. But I get what you're saying!

      I agree. A simple microswitch to detect if the latch is locked would do the trick. For all latches on the plane, it would not add that much to the cost of the plan. I'm guessing, but significantly less than the cost of a new cowling.

      @@berniesteadman7642 Ideas such as this all sound good in theory but are unrealistic in practicality and implementation. You can't *safety proof* the entire plane, weight becomes an issue then. This is why checklist and inspections exist. The problem isn't the latch, the problem was human error! My personal solution to this, which I have suggested for years, is to only be able to actuate the latch if some sort of 'key' or pin is inserted with a long red flag/streamer attached like with pitot tubes. You have to insert this in order to open the latch, and it cant be removed until the latch is properly closed. Some suggest relocating the latch, but that can still be overlooked almost as easily as it is in it's current location at the very bottom of the cowling!

      @@tima.478 When relying on a human/visual checks a certain failure rate for detection has to be assumed by engineers. Even if it visually obvious, the assumed failure rate is still significant. I.e. it has to be assumed the issue will be missed a significant amount of the time.

    I flew on this same Frontier flight with a picture of a polar bear on the tail exactly 2 years earlier. That was a close call. I liked flying Frontier. I guess the ticket prices were low because they weren’t sure how much of their planes would make it to their destinations.
    The crew on this flight were outstanding. So was the passenger who got a great picture of the damaged engine.

      Just a few years ago, there was a similar situation on a Southwest flight, except the engine debris punctured a window and led to a decompression that sucked a woman halfway out the window and killed her. So, yeah, it can be terrifying.

      It has been addressed. Maintenance has to sign the logbook that the cowls are latched every time they remove them. Also, this is not a big emergency. Your car would still run with the hood off.

      I also don't understand that, in all these instances it ended fine in the end but next time the parts gets sucked into the engine or damage the wing or damage/open the fuselage and it can get really serious. I assume they can easily install some kind of sensor or mechanism to solve the problem.

    Why would FC offer 26L or 26R to the pilot , AFTER he’s already told by another pilot that there is debris on 26R? That seems dangerous.

      just giving the pilot options but likely it was off to the side of the runway if the other plane had no issues taking off.

      26L is longer than 26R. He wanted the left for the longer rollout since he was heavy and down to one TR (thrust reverser).

    Kudos to the developers of Flight Simulator. Such an epic hard task. So impressive. I cannot even imagine the work it took to get every detail into the game. From one Software Engineer to another, I salute you! Be super super proud of this amazing accomplishment!
