      after 15 yrs of chronic cough..I cough no more...50 mg Tramadol once in the morning...I would sometimes have a shocking feeling in the back of my throat and cough for 10 - 50 minutes...coughing up lots of phlegm..no more yeah!

      I am not cured and occasionally I still have a long coughing spasm where I cough up clear phlegm but the majority of my life I do not cough

    Exactly my symptoms. I wish my doctor would look at this. So embarrassing when you have to talk then sudden cough and no treatment works. 😪

      Hope you feel better. I have been coughing too for the past 3 days...it's exhausting.
      Try Vicks vaporub just under your nose and steam. It gives some relief.

      One is try clearing your throat from time to time another thing is drink water and the last one is exercise, I went to the pool swam and ran around and I’m now coughing a lot less often

    When I have a tickley persistent cough. I chew on a piece of ginger. In a couple of minutes, it’s gone away.

    I am just sobbing, just sobbing! I have been coughing like this for over 4 decades. It has impacted my life in so many ways. I have lost relationships, I can't see my friends very often all from this crazy thing. If it happens in front of children, their mothers worry that I shouldn't be around them. It's horrible. Nobody has been able to help me in ALL that time. Because nobody has understood it. I am going to send these two videos to my doctor. Let's hope she understands and helps me. Thank you, thank you from the bottom of my heart. Finally, there is hope.

      Try smooth dairy or dark chocolates, York peppermint-patties and also Gabapentin & lysine-foods & honey & starches. They all help thicken mucuos, which helps prevent "tickle". Air-conditioning and forced-air heat & acidic/spicy/rough/chalky foods make it worse.

    This is the closest description of exactly what I’m going through. My cough goes and goes suddenly for over 11 years now, I lose sleep, it causes chest pain, abdominal pain, urination, vomiting, headaches, watery eyes, leaking nose, it’s so annoying. People always assume I’m sick and offering my water

      My mom did for 55 years we both had pneumonia it started after that. Cellular structure gets damaged possibly from pneumonia or avirus around the larynx

      We both have the same symptom. Because of this cough. I cannot be facing anyone but stay in the house. I am so afraid of going out or going to work. My friend and families stay away from me. I told them that I am fine but no reason why this cough is keep going on and on. I have tried many ways but never had luck.

      Slilppery Elm is helping me to expel the toxins in my lungs from the chemtrails. I'm going on 8 weeks of hacking. I sound like a smoker. I understand that the chemtrails include nano-size particles of fiberglass wrapped in aluminum that you can plainly see at 400x. I just ordered a microscope to check it out myself. I coughed into a paper cup, let the flem dry out, then looked at it with a magnifying glass and I saw a ton of tiny little things sparkling when I held it up to the sunlight. I can't wait to get my microscope. Then, I can monitor how much is there and try different things to remove it and watch and see if the amount goes down. If it's on my lungs, it's all over, including my brain and arteries. If it builds up over time in my arteries or brain, I could have a stroke. So, hope this helps anyone else out there who has this horrible cough that seems to never go away. There are things you can do to expel the toxins from the chem trails. I learned today the benefits of using activated charcoal powder to eliminate toxins in your body. Gonna start that tonight.

    (Part 2 of 2) BUT, with care, good oversight, and starting at very low doses, it's surprising how many can in fact take a neuralgia medication quite successfully. Some drug interactions are only "yellow," not "red" alerts, and we often try a "yellow alert" medication in low doses.       I had this last year I ran to the hospital they said it was stress. Then this year came back and I noticed the samething when I get exited and anxious I get a bubble of air coming up and cough. So annoying :/ it scares me.

    When I get this cough I start immediately blowing my nose even though I don't feel like I need to. What I get is small amounts of watery drainage and after a couple minutes of continual blowing of my nose the cough stops as suddenly as it began.

      I have struggled with chronic cough for many years. The cause appears to be "multi-factoral." I can say that one of the triggers is anxiety. Just hearing the voice of my boss would bring about a coughing fit. If I am experiencing negative emotions, it can get quite severe. I am guessing this is caused by stomach acid churning upwards during anxiety, splashing on my poor larynx, triggering the cough.

    Tickle happens alot. And coughing all the time. It feels I might throw up at the end. Also feels like very heavy mucus is in my throat all of a sudden. Specially when eating.

      So just an update. I started to take famotidine for heartburn and I cant remember the last time I coughed..
      I think the last episode I had was after choking on water going to my air pipes. For the rest of that day my throat had heavy mucus and I had to cough all day. And once when I drank something cold.. thats it. Such a relief.

      @@aminahshentour8002 I too believe that this can easily be solved by an antacid( even baking soda). When I take it I am relieved from larynx spasms, but never noted if the coughing diminished. Will try this tonight.
      Question Is mucus in the esophagus just an excess of stomach acid ( simply put)?

    Hi Dr. Bastian: My cough began June of 98. I have had it ever since. I have so many of the symptoms you described. This could be me. I have been treating with a Neurologist for about 3yrs, with Baclafin. It helped at first, but not now. I am affected by cold (air conditioning), strong smells, talking, stress. I get pain in my chest. Once the spasms begin it sets off a chain like reaction, and just gets worse, as does the pain. Please tell me what you recommend I do

    I traveled from Arizona to Illinois to see him. Amazing man, I cried when he understood what I had been going through. He said that many patients have reacted that way. 24/7 coughing is no picnic..

      please chew and swallow fresh garlic clove and wash down with water everyday. start 3 times a day. 24/7 cough well try anything. i have 14 year cough fits daily and garlic is only relief except opiates which is no good. wish you well Jan

    I encourage all to listen to your body and stop and think what you are breathing what you are drinking and eating. Look at the ingredients it could be damage by the synthetic chemicals. These chemicals cause nerve damage and causes you to have longer episodes of sickness than you normally have. If you learn how the lungs work to clear pathogens and environmental effects on their function it would be clear. I have eliminated milk and cream for a while and this often works. There are synthetic perfumes for example in cleaning products that accumulate and you can see in the bottle that it is harmful to nature. If it is toxic for fetuses children and fish it also irritates you and weakens you.

    (Part 1 of 2) At our practice we do Botox injections into the larynx in approximately 20 patients each week, almost always for a condition called spasmodic dysphonia (SD). It is a great treatment for that disorder. By contrast, neuralgia medications seem to be the mainstay for SNC. Of the many hundreds we’ve treated for SNC, a significant percentage take other medications that seem to contraindicate these medications. If that is your circumstance, certainly no problem with a trial of Botox...

    Twenty five years with this cough. I been wrongfully diagnosed with acid reflux. Allergy diagnosis was wrong also. Mainly at night. Going from an upright position to lying down will trigger it also. Also, if I’m too hot that triggers a coughing spell too.

    I developed this pattern about seven years ago after a severe cold all other symptoms disappeared except for the cough! It became almost a daily thing….My coughing occurs the moment me head hits the pillow
    Or when I eat cookies or chips
    Or when I sing
    Or when I get nervous or angry, the worst part is I started wearing pads then later depends because I cough do hard it caused incontinence😢
    Or when I drink juice
    It is a hard violent cough which hurts my chest like tiny needles or leaves me teary eyed it starts with a scratchy spot in the throat, there thin mucous but it seldom passes out the mouth
    This sudden cough last from 2 to 4 minutes then just as sudden it’s gone y😮
    He might be on to something🤔

    My doctor just diagnosed this after 2 years of coughing. I also have 1 paralyzed vocal chord. Last night for the first time I took 10 mg. of Amitriptyline and I didn’t cough at all! It was a miracle!

      I also have this cough for too long when I started playing with herbs iam much better .I can cough after sometime. I take ginger,cloves, thyme ,bayleaves, cinnamon,tummeric,cayyyene pepper and honey ,i drink this tea morning and night its really helping

    I am so glad that after decades of living with a chronic cough, I finally found this video. I asked my GP for one of the prescriptions mentioned in part 2, and my coughing was lessened by about 80-85% - to the point where it was much easier to deal with, and my quality of life improved dramatically. Thank you so much, Dr. Bastian!

Gabapentin has similar properties as "flu-friendly" high-lysine such as eggs/dairy. ...Here's how: Gabapentin & lysine-foods & even honey/starches, thicken (thus tame) mucuos. In my case, the Gabapentin-thickened mucuous, upon being swallowed post-nasally, acts like waxy-balm. Note my brother and I both have SNC from having lived in forced-air heat houses & too much raw-onion foods & acids in childhood. But mine is worse from ongoing interaction with a difficult person. (She elicits shouting from everyone till their throats are raw.)

      @@Laryngopedia Hi, thanks for your video. I have just come across it tonight, and I think this is what I have. I had a big episode of it whilst at uni in 2012 amd I put it down to anxiety. I ended up with a hiatus hernia because of the wretching. It has started up again in the last couple of weeks 😞.
      Does it make sense that it is worse when I am on my own as this seems to be the case

      50 mg of Tramadol in the morning only ... and 16 years of coughing after I had pneumonia ..no one talks about phlegm...I can cough up 1/2 to 1 cup at a session

    Ive been coughing for 15 yrs now and it all started caused of my horrible allergies,My fam hates having me around due to this chronic coughing but they don't get it! I feel an itching and mucus in my fucking throat and i cough because its the only thing that can relieve the annoying Ness of it all.I HATE THIS! Even at department stores my family finds me on the isle because they follow my cough.🗣plus i pop everyones ear drums and it happens around 50+ times a day!!😠

      mine I think started when I was workin at a restaurant.. 15 yrs ago. used to clean the vents with eazy off.. I think that's what messed up my throat. Sometimes it goes away. but always comes back. its so annoyin.

      I have the same symptoms: itching and mucus aghh and I cough cuz don't like that mucus feeling. But I also have bad allergies. What hellos me is grind ginger and mix it with hunny and store it in your fridge and have one tea spoon every morning and night before bed time. Good luck. 😃

      YariGlows215 same as with , sometimes spasms and can't breath for a while, if near smoke, perfume. Often at a meeting, movie theatre anywhere there is an audience,ha limits ya life a bit. Always take water bottle, drink through straw, purse your lips keep tongue on roof of mouth, I use this for water.hope this helps. Real curse for us

    My mom is coughing like that... She said there is something in her throat that she thought it's a kind of phlegm, itchy that it triggered her to cough so hard made her teary eyes and runny nose. The cough is so dry and she said when spit out looks bubbly saliva. It is also disturbing because coughing would occur when it's about bedtime at night and at dawn. My mom is 76 years old

      I am also in same situation.
      My doctor have advised some medicines. It is working but only till i take medicines.
      1. Gargling with warm black tea boiled water twice daily. (no salt added)
      2. Telekast F (an anti-allergy medicine) - suspecting some kind of allergy is triggering it.
      3. PAN 40 (gastric tablet to be taken early morning empty stomach) - suspecting acid reflux.
      I really don't know which of the three is working for me but i am relived for now but it gets same when i discontinue the medicines.

      I heard that high vitamin B1 (tiamine) 500mg 3-4 times a day cures it as it is a nervous predisposition.

    Dr. Sir... I experience those symptoms, but I always am coughing up phlegm. So - is my the "purposeful" type?

      I cough up phlegm for 16 yrs. In my investigation I learned that the cellular structure which regulates the phlegm is damaged from a virus or pneumonia which I had 16 yrs ago. There is no solution but there are way to calm the cough. I take Tramadol but the regular prescrip is amitriptylene. I've had every test spent thousands over the yrs. Ricola Honey Lemon Echinecea or Dual Action is helpful in calming the cough. Also, Fisherman's Friend, Original Extra Strong, Menthol Cough Suppressant.

    Not individual advice, but orienting information: SNC seems to be a neurological disorder, so treatment is via neuralgia medication. GERD-related inflammation may lower the threshold for sensory disturbance activation; so, treating GERD may help. Still, if the cough is neurogenic, it’s important not to focus on GERD, but on the neurogenic disorder GERD is simply “kicking up.”

    I found something helpful (at least to me). Placing a hot water bottle/hot pack compress at the back of my neck completely suppressed the ‘tickle’ and therefore cough immediately. Working longer term little by little too. Please make sure that you do not have it too hot obviously….

    I just want to thank you for providing this information. This has been going on for me for just over two years and I thought I was losing my mind. Unfortunately, sudden, sharp coughing also triggers migraines for me, which I also have chronically. It's not a good combination.

    I have had this exact type cough since receiving the second dose of Pfizer Covid vaccine two months ago. I have Charcot-Marie Tooth syndrome (neurological). Constant cough day and night. I feel a spasm in my upper chest and have that dry, violent cough (totally non-Productive). Do I see a pulmonologist or neurologist? Or Dr Bastian?

    Well, this fits perfectly the cough I've been battling since I was a kid. It wasn't much of a problem in southern California, but when I moved to Portland Oregon, after a bad cold, it got worse and didn't let up until months later. How do climate and regional variations of air quality affect SNC? I'd like to spend a couple months or so in California, or Boulder, or other places I've lived before, to see if the cough goes away or at least returns to "normal" levels. But this is not practical :(

    I would call the ENT dept of a nearby univ or medical center. Ask the triage nurse if he or she has heard of SNC. If so, ask for a physician interested in SNC. My guess is, for now, there are only two or three physicians truly interested and experienced with SNC in any metro area. Try to find one; else you risk rounds of tests and treatments for “usual suspect” diagnoses-to no avail.

    Ur very accurate.... I dread that tickle in d side of my throat.... I don't get it often but I dread it as iv asthma.... I was told not to use benylin as it trigger s asthma... It works on tickless cough s... I only takes a sip.... My throat is wrecked I'm only after covid n iv a cold again since yesterday no end to it tanks for d advice 🇮🇪

    I have had exactly the symptoms described since I was seven years old. I am now 42. I remember waking up one day with a cough, although I didn’t feel unwell, and it never went away. It worsens when I’m agitated, when I change position, and sometimes even when I’m amused. It annoys those around me, and my attempt to suppress it for their benefit (and I suppose my own) has led to the emergence of various ticks. I had measles and scarlet fever when young and wonder if it might be related to these in origin. I must admit, though, that I have often thought it might be pure neurosis. It is nice to see that at least one doctor has given this some attention.

      Try smooth dairy or dark chocolates, York peppermint-patties and also Gabapentin & lysine-foods & honey & starches. They all help thicken mucuos, which helps prevent "tickle". Air-conditioning and forced-air heat & acidic/spicy/rough/chalky foods make it worse.

      I'm 58. I've had a persistent throat clearing since I've been in my late 20's. I've always had sinus issues , post nasal drip or so I think. I've been to ents over and over they put the camera down and say looks normal. Migraines you name it. I've always said it feels like I'm drowning, my throat feels like it feels with fluid and I have to clear my throat often hundreds of times a day. Very annoying to me and those around me. Nobody believes me I've had people come up.to me In stores saying oh have you checked for asthma? I'm not short of breath, and clearing my throat so much makes me have a very sore throat. So frustrating. If this is the answer I will be grateful if somebody listens

    Over a year of constant violent coughing, 6 specialist, multitude of scans scopes and X-rays and in 4 days your gabapentin dose recipe stopped the cough completely!       this is one answer as well as other medications I chose 50 mg of Tramadol...stopped coughing after 15 yrs of coughing up phlegm and no moree shocking feeling in the back of my throat!!..I have my life back

    I cough like twice every 30 minutes. Just a random light cough. Probably all the smoking I do

      Wow you’re blessed. I cough 2-4 times every 20 seconds and it is a very hard cough that makes me get tears in my eyes and i get gagging reflex and have to vomit few times a day. I wish doctors would be more qualified as no one knows what I have. I feel like we’re living in 16th century.

      Lost Cause i dont know what that is, im from germany. I have been to lung specialist, ENT specialist and have been treated with medicaments for asthma,allergy etc nothing worked. And yeah its bad and gets worse with time I feel really exhausted. I’ll try to tell my doctor about this video and nerve ending medicaments but i doubt he’ll listen as he always changes topic when i talk about my coughing

    Thank you so much for this information. I have been suffering from this for at least 14 years. Mine are generally caused by saliva/beverages going down the wrong way. I had a terrifying attack this evening, which brought me to this video. I have figured out that keeping calm is important, but did not have a name for it and didn’t understand the mechanics. I generally take long, deep, and very constricted breaths in, just to get any air into my lungs. They seem to be getting more severe and with greater frequency. I will try the straw method for my attack (hopefully never). You mentioned that nerve damage may be a cause. I’m wondering what kind of nerve damage...

      Same here. I was looking into it because I have elderly relative who coughs everyday throughout the day so I was curious and it seems that this explains what she has. She’s been doing it for 2 years now and she’s 70.

    I suggest that there could be compromised vagus nerve involvement.
    I've had a meningioma that compressed my right vagus, including the laryngeal branch. For a couple of years I had paralysis of the right side of the larynx (with voice loss), and coughing as described in the video (coughing is ongoing). I've had a couple of laryngospasms - very frightening.

      A virus or pneumonia can cause damage to the cellular nerve structure around the larynx...after 15 yrs of coughing after having pneumonia 5-6 times a day with lots of phlegm...I take 50 mg of Tramadol only in the morning and my coughing has STOPPED...I feel like I have my life back!!

    Oh God. I've had the worst memories with this problem. I've had teachers kick me out of examination hall because i was coughing continuesly and the more I cough the more anxiety I get and the I cough even more. And the worst part was my parents wouldn't believe me cause they never saw me in that situation...:(

    20+ years of coughing. No doctors have been able to help. Weather triggers it, esp high-energy storms. Eating. Laughing. Talking. Sometimes dryness, often with too much phlegm.

    I never heard of this and I’ve had it for couple years. Just last night I woke around 10 coughing so bad that my veins squeezed and was in serious pain and tired.
    I get this from cold air usually and no cough medicine except the pearl small round pill helps me.

      I had some horrible flu or something about 12 years ago, was freaking me out, I ended up having to stop by the local
      hospital's ER which I was trying to avoid, they checked me out, MD just gave me "Tessalon Perles" which is that basic
      strong cough medicine they dole out there, and that seemed to knock out the cough part of it, which was a horrible
      wracking cough that was causing my sides and ribs pain. I was sore for like a week after this illness, which was otherwise
      not serious. Over the past 13 years I've also had a cough from acid reflux, which of course, can come and go which is
      very annoying. Taking Prilosec or Xantac will generally knock that out. I never smoked, but I was around second hand
      smoke for years ages ago, alas.

    So sorry I can’t consult in this format. So do not take this as medical advice. Just “information” that might orient your thinking. First, your story is not unique (cough following spine or thyroid surgery). Second, the treatment is to use one of a list of neuralgia medications. I believe you will find more information in the part 2 video on this subject. Also look at the written information on www.laryngopedia.com under “sensory neuropathic cough.” And finally, be sure to put the word “mucus” into the search window. The article that comes up “When Mucus Seems to be....” may save you tremendous time “chasing mucus around.” So sorry you are struggling with what sounds like it may be SNC, and all the best in your search. Most can find tremendous (if often incomplete) relief from this problem by use of a neuralgia medication or capsaicin spray (also described on www.laryngopedia.com). RWB

    Curious he mentions not much research or scientific papers have been written about this condition. Why is there not more information about this for physicians and patients?

      An excellent question. For background: The diagnosis of SNC is based in match to a syndrome. I have been saying for years that it is a primary, prima fascie diagnosis. Unfortunately, there is no xray, blood test, or other "measurement" that validates the diagnosis. Many doctors dislike/dismiss this "observational" approach to diagnosis. They therefore don't know about SNC, or they dismiss it, or insist that it can only be considered as a diagnosis of exclusion. Doctors tend to order and re-order tests for the big 3: allergy, acid reflux, and asthma. Patients become exhausted, cynical, and jaded... Do keep in mind, however, that there is a growing literature on SNC, including my article on amitriptyline and an open source article that can be easily found by typing "PeerJ Bastian" into your browser. Laryngopedia also has a wealth of additional information on the trail to discovery, medical jadedness, capsaicin, etc. Just type "cough" into the search window of Laryngopedia.

      @@Laryngopedia thank you for the reply. My understanding of Elavil used to be that of depression medication but now have found uses for neuralgia.
      I have also heard from Allergist and ENT physicians that this kind of cough is usually labeled idiopathic in nature or as mentioned as an exclusionary Dx when standard 3 tests return negative results. Could there perhaps be a Vargas nerve connection to this kind of cough? Seems the cranial X is rarely mentioned or studied. Thank you again. I will research you suggested journals.
