Triage X: basically this is Black Lagoon, but focused much more on the s'xual part of the criminal underworld however, it's about "medical assassins" seeking to "cleanse" the world of the "disease" of evil people, by using the ultimate "elixir" that cures everything: death/violence/killing but, there's still more story/plot too, around one of the characters and the main male character, that is actually quite good and even emotional too, despite the heaviness of the s'xual ("fanservice") content, this is actually quite a good anime with good story, plot, and character development, that will really surprise you

I haven't seen anyone mention 86, or Mobile Suit Gundam: iron-blooded orphans in the comments yet, so I'm here to suggest those two anime for adults. There's dozens more I've seen, but those two always come to mind as they exceeded my expectations and aren't talked about nearly enough.
For me #1 is Butareba It starts as a stupid and pervert, but also cute isekai about a man turning into a pig and meeting a cute slave girl. This facade goes on for 4 episodes and 95% of the audience quit this anime at this point already. But after episode 4 it turns REALLY dark and by episode 8-12 the story is darker than a fricking black hole. I saw this almost 2 years ago but still randomly pops in my mind to haunt me. It's not not (that) graphical or a horror, it's about the brutality of humans and what they are capable of...
