Okay, to whoever is curious on how you can have Farid and Harper alive, I believe you just need to complete the game and just replay the mission (not rewind the story). May only work on PC since I just replay the mission some time after completing the campaign/story and not rewinding the story, having all characters alive in the mission (Karma, Farid, Defalco, Harper).
Kravchenko, Hudson, DeFalco, and Farid die regardless of the decisions the player makes. On the other hand, the deaths of Menéndez and Mason are canonical. Woods is canonically alive On the other hand, it is not known after: Briggs, Karma, Harper and Salazar, etc.
karma is dead melendez is canonical dead too that means that Cordis die took place while it's karma if she's alive that stops it harper is alive normally because menendez died in the real story Farid tried to kill melendez but he couldn't and that saved harper instead Salazar is dead if we also follow my story because Harper is going to kill him
Without mods or hacks you can not save both on the same save at this point so likely a hack same way you could get tali on Jack's mission for example in me2

You can actully save karma and harper in one ending. just fail to secure karma so she will lives but not recapturd and do not kill harper. by killing menendez you can have woods and mason and karma and harper all of them alive.

You have to complete a strikeforce mission to save karma without killing dofalco the mission is called second chance you should see if after the mission karma
I always kill Harper so Farid survives, also DeFalco is dead and Harper is alive. I prefer the true ending if you spare Menedez because it avoids blacks ops three given killing menedez is canon. Plus he freaks out after realizing all is lost with his plan. It’s like closing a story plot points all at once.
In the canon route Defalco dies during the Karma mission and Farid Kills Harper(also Mason can be shot in the leg instead of the head if people didn’t know)
BO2 is so frustrating, due to the multiple outcomes. It took so much work to get the good ending, after I realized certain choices were important and more so than I ever realized until I finished the campaign initially. I always play campaigns on Normal whenever possible, so I can enjoy the story before replaying on Veteran. I couldn't find a way to get the higher difficulty achievements without completely replaying the campaign and making sure I made the right choices was really not an appealing prospect. So BO2 is one of the only 360 CoD from CoD4 to Ghosts that I didn't officially get the Veteran achievement for.
I don't remember anymore how you get this scene I remember at in my PS3 days I had this scene but replaying it recently on PC not is there something specific I have to do?
In the mission “Achilles Veil” the one where you had to either “kill” Harper or Menendez, if you kill Harper, Farid will be alive until the next mission where he dies either by DeFalco, or by Salazar. Hope this info helped you remember ;)

Chloe can't survive if Harper is alive. That means that Fed is not alive and there is no one to take the bullet, or shoot Defalco.
