The problem with Piano especially is that most of the times you run into weird phasing issues when you just sum them to mono. I haven't found a good solution for that. Sending only the right channel is a possibility, but the player has to know it, otherwise he could be tempted to readd the lowend that he misses, snd than everything is out of balance.
Would this also apply to main out? For a mono sound system? Run 1 cable out of the board to the system instead of 2.
Width will affect the Main, Yes. but I strongly suggest you to run Stereo Mains as much as possible

You could use the Width and send a blend the signal, if your not Full mono, phasing may happen but shouldn’t be drastic enough to cause problems
