This is a cardinal sin. You do NOT go out, at least, as the third batter and swing at the 1st pitch after 2 consecutive outs with 2 batters. The 3rd batter must work the count to try and give his pitcher as much rest in between innings. You see some of the smartest baseball plays in the major leagues and also some of the dumbest.

On the other end of the spectrum, I have a memory of an interesting four pitch sequence that began a game between the Angels and the Rangers in 1982. As I recall, on the first pitch, the batter hit a single off of the Angels' Ken Forsch. On the second pitch, the batter lay down a bunt and Forsch threw it wildly past the first baseman. With runners now on second and third, Forsch hit the batter with his third pitch. Now, with the bases loaded, Forsch threw his fourth pitch and the batter deposited it in the seats for a grand slam. A certain kind of perfection! Of course, this was so long ago that I might be doing some rewriting but I'm largely convinced it (or something quite close to it) happened.

Brian Bohanon of the NY Mets threw a 3-pitch inning in the 3rd against San Fransisco, whose Rod Beck pitched a 3-pitch inning in the 9th. There is one other example I can find - - so as far as I can tell, it's NEVER happened in the same inning, but it's happened twice in the same game.
And there is also a third case where the same pitcher threw a 3-pitch inning in two different innings in the same game.

But it IS strange. There have been - - in the history of recorded baseball - - 197 3-pitch innings (or half innings). This is extremely rare. Three pitches to three batters recording three outs - has happened 197 times on the record. Remember - this isn't a game-by-game stat; it's an inning-by-inning stat. Records are incomplete when you go back past about 1920, but even if this were the case since....say..... 2000. 162 games played by 30 teams. Divide by 2 since there are two teams per game. That's 2,430 games. Times 9 innings per game (this doesn't even take into account extra inning games obviously) - and you get 21,870 innings of baseball played. 197 three pitch innings divided by 21,870 innings is less than 1%. And this only goes back to 2000. Imagine if this included the entire history of the league. Going all the way back to the late 1800s. In only 197 innings was there an inning with 3 outs recorded with 3 pitches. That 1% would drop to virtually 0. "It's not all that strange" is an absurd thing to say.
