Yes, there is a lot omitted from the book. Very unusual for Silver Reed, their books are normally pretty comprehensive. It was this lack that prodded me into making these videos.
It sounds most likely that you are using yarn that is too thick, or the stitch dial setting is too low. Or, maybe you’re trying with something like cotton which has no stretch. I recommend starting with a DK weight of yarn, dial set to 4. Or worsted weight, dial set to 7. If it’s in balls or skeins then you need to wind into cakes
It depends on the method! The Russell levers control the needles in hold. If they are at I then any needle at D position will not knit. The side levers control whether needles will knit or slip.

The point of the ravel cord here is to hold the open loops of yarn in place on the needles, allowing the needles to move forward, and the next stitch to knit through, without the initial row becoming unravelled.
