- At WPS, we want to help our customers have the best possible experience with our health care plans. That's why we have a Case Management Program for our members. Every Case Manager is a Registered Nurse who is there to help you with your health care questions and concerns. We believe you should be able to focus on living your life, not navigating the health care system. You can rely on your Case Manager to help keep your health care on track.
Your Case Manager can help you by:
-Coordinating your health care
-Assisting you in navigating the often complex health care system
-Providing education specific to your health care needs and concerns
-Supporting you and acting as your advocate to help improve your health care experience and outcomes
-Helping you reach your health care goals
-Assist you in finding possible opportunities for lower costs and savings on your health care costs.
How does Case Management work for you? If you are hospitalized or diagnosed with a new condition, such as but not limited to Diabetes, Circulatory Disorders, Cancer, a WPS Case manager may call or write to offer their help. You can also begin Case Management by calling WPS at 800-333-5003. Just ask to speak to a Case Manager directly. Your Personal Case Management Nurse will be available to work with you over the phone. You'll be able to discuss health care concerns and get the information you need to make the most informed decisions about your care. These conversations are confidential, and your privacy will always be protected. WPS values your privacy and security. There are times when managing your health care and navigating the health care system can feel impossible. We developed our Case Management Program so you won't have to face these challenges alone.
