- Everyone gets free dentistry up to the age of 18.
- Those with low income continue to get it free.
- Everyone else pays a limited amount.
It's the same as how prescription medicine isn't always free on the NHS but is subsidised such that people don't have to worry about being bankrupted by chronic medical conditions.
If a UK resident didn't get braces as a kid it is not because they couldn't afford it. That's a valid choice btw. Look at Japan. Many of their celebrities have "crooked" teeth and no one cares. Maybe in America you can tell someone grew up poor by looking at their teeth, but that's a specific combination of fashion and economics that is far from universal.
