    People wonder why healthcare in the US is so expensive. Well, this by itself would not be enough to move the needle, but it is an example of the myriad of issues, which combined, ARE the reason for expensive healthcare. And they are nearly all driven by two factors-Administrative Bureaucracy and Fear of Lawsuits.

    While it isn't as expensive, this is in nursing as well. All of these advanced degrees just want your money to keep the letters after your name. CEUs are a joke and the standards make it impossible for mothers to take a couple of years off with their babies and then return to nursing. Instead they have to go back to take retraining courses. The fact is that a nurse working in geriatrics can go get a job in pediatrics and it is the employee and nurse that makes sure they get the on-site training before working on their own.

    The Moc part 4 is not even something most pediatricians would agree to do if we had a choice. We are told we have to do mini studies to improve our practice, which (conveniently) allows the board to show they're improving practice with the certification. And if we can't show improvement, it's difficult to get credit. Not all studies should be successful. Sometimes, we have no control over the things they think we do to change and study. We oft cant tell nurses how to do their jobs, etc That's is quality controls department or management and not the job of a pediatrician! Most doctors are no longer the boss of where they work. What good doctor doesn't practice to their best abilities all the time? Who in their right mind would wait to make chages until their next recertification period to get MOC 4 credit? But smart docs that want to remain certified will need do just that.

    It is a mistake if you do not change your opinion over time during the Covid era. Lack of transmission protection post vaccination, importance of natural immunity, lack of understanding of age risk stratification etc. etc. all were very significant dynamics that evolved are our understanding of Covid. VP and ZDogg are to be respected for recognizing and changing their stances. They are both deep thinkers who have advanced the way we should think about these turbulent times.

    Same thing for EMT's and Paramedic's. Recert every 2 years. You need 24 ce for EMT. You need 42 ce for Medic's. You also have to test and prove the skills. Recert a CPR card which does not mirror what you do in the field. Some of the ce are free, others are not. None are worth jumping hoops to acquire because rarely does anything change in our field. So 99% is a waste of our off-time. The test and skills we do in the field so no need to test for them. The CPR card is a joke and cost money. I said years ago it was a mafia scam. If we didn't have a union it would all be out of pocket instead of just some of it out of pocket but all on our off-time. Our medical field is borderline archaic the way it is ran.

      @@reginamemoriesforever-vc8ql Because the whole thing is set up like a business including our EMS agency, CPR cards, test with ridiculous questions, medical examinations before you can take a Ambulance test, both out of pocket. All on your own time. It doesn't include the 10 test on company time that we have to take every year to keep our jobs. I have a EMT card, County card, Company card, CPR card, medical examination card, and Ambulance drivers card. All cost time and money. Its is a mafia racket.

    For ours it's not even a written, lengthy test anymore. It's 10 questions every quarter. They are easy, have typos, and still cost like $500/yrly.
    Also I can't check my CME tally without paying the AOA $200
    I almost want to call my board and say "hey can I just pay you an extra grand and do nothing, clearly that's all you care about"

    Thanks. Any chance for a discussion of the monoclonal antibodies currently offered for COVID? I'm immune suppressed and they would be offered to me if I had to be hospitalized (early stage CLL on watch and wait). I am vaccinated with one of the non-MRNA vaccines and would refuse any MRNA shot offered to me. Thanks so much if you think you could discuss the monoclonal antibodies. 😊

    It’s all for profit and all BS. I’m grandfathered in to ABIM and the time and money I would have wasted on this I spent on getting an MBA and I know more medicine than the test takers because all they know is study to the test. I study what my patients have. And you don’t forget what you see. And I buy my state CME hours from NEJM for a couple hundred bucks every 3 years. I’ve got it down to ~6 hours. My wife thinks it’s a joke because it’s open book with the citation given. Hey, what’s a guy to do? And I’m not burned out like the MOC victims. But I have only a couple remaining NEJM TESTS to do. I’m thinking in retiring at 85.

    Good discussion. One of the main problems is these academics make up these tests and they don’t have a clue about the day to day practice of medicine.

      last time I took peds board they had three questions about Hib and epiglottis even though it’s been vaccinated out of existence. I studied with 20 year old practice exam questions and some on the real exam were exactly the same.

    There was a lot of speculation from both of them, but they did not look into the topics discussed much. There is research showing that board scores correlate with care outcomes. If they do not agree with this research, they can discuss why instead of saying it does not exist. It costs a lot per question to make exams since they are written, attached to citations, edited for grammar/clarity, reviewed by multiple physicians, revised, pgiven a test run, pyschometrically evaluated and differentially weighted. That is on the level of individual questions, not including the cohesiveness and comprehensiveness of the test as a whole. They are speculating about increases in testing being for money. ABMS made rules requiring increased testing. Individual boards then responded by increased testing. Many of the boards charge annually, not per test, and so are not making more money but are incurring a cost. Many boards have not increased dues in years despite inflation.

    Do you two plan to work to get the MOC eliminated? A good start would be to eliminate the MOC and have the ABIM simply keep track of CME credits or maybe require attendance at a board review conference or something like that. Maybe not try to eliminate the ABIM, but simply eliminate the MOC.

    MOC is a classic example of Pournelle's iron law of bureaucracy: "In any bureaucracy, the people devoted to the benefit of the bureaucracy itself always get in control and those dedicated to the goals that the bureaucracy is supposed to accomplish have less and less influence, and sometimes are eliminated entirely."

    I've been a tech admirer and appreciator my entire life. I remember a long time ago, a computer program for the masses that could provide a quick and dirty diagnosis from health prompts. Cool, but merely a novelty.
    Flash forward to now and we have AI chatbots doing much better diagnoses.
    So now every profession is subject to AI replacement or augmentation.
    I predicted long ago that we would get here with physicians. However, for it to work, the AI bot has to be given the proper input from the patient. Are most patients capable of that? Probably not. So I predict a system staffed by nurses or NPs or PAs that interface between the patient and the AI bot. We already have Urgent Care clinics. They will expand to include Routine Care services. And established physicians' practices will stratify into basic services (nurse with AI bot) and concierge service with a human physician.

    My Lung Dr got mad when I said no to the jab. I told her I will not take any MRNA jab so she posted that on my chart. I feel she has now put my life at risk & I'm afraid to go to the hospital for fear of an angry nurse or Dr.

    NPs recert every two years but it isn’t a test. We have extra CME for the board certification. It is expensive and it doesn’t prove anything other than your employer or you pay money for a piece of paper. If they made it a retest or speciality based test things would change fast glut of NPs not practicing evidence based medicine would drop. I say this as an NP. We need to police our profession better.

    The exams are a joke. What’s really onerous is logging CME while reading while on the toilet . Same with PI projects for maintenance of board certification like doing projects on hand washing.

    More and more people I used to spend hours listening to are changing their format to a paid version. This is very sad. You may think that $3 a month is not much but, if you add everybody, it's a lot of money. We used to have access to excellent information on you tube, unfortunately now, it will only be accessible to a few. I understand people spend a lot of time doing their show, but it's not like it's a job for them, it's more a hobby. Now we are left with 9 min of information. Money, money... Destroy everything.

    Unexplained care variation (or actually explained but never fired or addressed) - a sit down test doesn’t change bad behavior. You guys are so correct - medical school, residency and fellowships in general do not fire bad doctors. We were able to get a super dangerous doc fired from residency - she almost killed multiple people. The residency had so many complaints they could not ignore it.
    Medical boards are overwhelmed with bullcrap complaints by overly sensitive people - I don’t like how my doctor told me I’m obese. (See the comments - I don’t like z because he said I had to get vaccine.) Hearing something you don’t like doesn’t make a doctor a bad doctor. I see bad doctors in practice (harming patients physically and financially) and the medical board has its hands tied w archaic laws in some states. In other states it can go overboard - the only examples I’ve seen of this is California. We have so much work to do protect the public from bad doctors. And it starts in medical school.
