I’ve done the QR code on a big screen, it resizes nicely, and people manage to use it. I suspect lighting and contrast might cause more issues than the quality of the QR code image.

How do you make the form in which you can submit each question's response at one time? For example, if everyone answered only question 1, press submit and see those results, then later in a presentation is question 2, the audience now does that question and press submit, see those results. I don't want the people to have to do all 4 questions at one time.
I’m not sure. You may have to create several different forms to achieve that.
