The hobbling scene in the movie is actually toned down from the book. In the book, she cuts off his foot with an ax, and then seals the wound with a blow torch.
I prefer the method in the book just for the horror of it but that didn't matter as much as the other stuff they left out. The slow deterioration of the typewriter and then the electric knife. Also the difference between the sheriff in the movie and the trooper novel. The movie version with the shotgun seems more realistic but again the book's version induced more horror.

Annie Wilkes in Misery defined the term “toxic fandom” for all time. Fun fact: Stephen King actually wrote this as a horror story for himself; this was his own personal horror, as he was a famous writer and he did have his share of crazy fans!

The other character is a DJ in The Stand 4 part Miniseries. Kathy plays a character named Ray Flowers. Her role is short but very good.
Actually, @MaryCherryOfficial, you got two points from the book spot on in your end commentary. In the book, the hobbling is by sawing his foot and cauterizing it with a blowtorch. Incredibly graphic and gore was not the direction Rob Reiner wanted to go with this film which is why it was changed. The scene in the film is still chilling, even without blood and I don't know anyone, even on repeated viewing who doesn't instinctively cringe when she breaks his foot. A testament to the effectiveness of the film. The other thing is, and you were spot on, in the book, it's made clear that while she was spying him at the Silver Creek Motel, she took the occasion to tamper with his car breaks which is why she knew he'd have the accident. Book Annie is straight off the bat a violent and unhinged psycho, Kathy Bates did a much more nuanced portrayal of her and she absolutely killed the role. James Caan was also outstanding. So impressed was Stephen King that he wrote the 1992 psychological drama Dolores Clairbone with Kathy Bates in mind and she did appear in the cinematic adaptation in 1995 (worth watching). Another movie you should watch with her is the one she starred in after Misery, in 1991, Fried Green Tomatoes. Beautiful movie, I'm sure you'll love it. Her role is so different in it that you'd be hard pressed to believe it's the same actress which is just another proof of her immense talent.
James Caan wasn’t first choice for this role. It was offered to a lot of leading Hollywood actors but they did not want to play second fiddle to what would be an Oscar winning performance to Kathy Bates.
I think every major actor turned it down. I think they didn’t want to be cooped up in bed forever either. It’s a tough performance to take on, a lot of the movie relies on Caan, who did a terrific job.
Even her persona to most didn't feel that huge. People would describe Bates as someone you'd see at school PTA meetings. And I must say, after seeing a few of her roles before this, I'm shocked how well she pulls off not being type-casted.
