    What if since the 1950s the experts said the best way to get something into orbit was with a giant rubber band (and consequenty there was never any spaceflight) and anyone who suggested large rocket boosters had their career ruined? This is the state of modern medicine and research.

    My fasting insulin was low ,waist is 34 and my height 76, 178lbs because I eat high saturated fat and cholesterol and low in carbs diet. BP low.

    Marsha for your BP get your am and pm cortisol and your extended electrolytes.... if your potassium levels are within normal limits try taking a bit of potassium like 99 mg of potassium chloride per day, and if the potassium is still in normal limits and you not get signs and symptoms of high potassium then take 99 mg 2 times per day...I take 99 mg 4 times a day....but I often water, coffee fast only a few days per week....for cancer.. I am born in the 1950's and am phenotype XX.... Keep checking your blood pressure about 4 times a day.... if your BP is high in the morning that might be the dawn effect so take it every two hours one day and see if it drops... Do not take potassium blindly or randomly as you could give yourself cardiac arrhythmia and a muscle cramp in your heart--a fatal MI.... I tam medically trained and it took me decades to find out that when the blood work says it is within normal limits it might not be normal or healthy for you....but potassium too high or too low and kill... if y.ou are in ketosis, and eating a lot of fat, and less protein, you might want to know that there is not much potassium in fats, it is more found in the meat... look it up signs and symptoms of high or low electrolytes in the blood serum electrolytes... sodium, potassium, calcium, magnesium, chloride, phosphate..... also look up how many grams of potassium you might need per day.... and one more thing for all the other people listening to this....low adrenal function can cause facial flushing, tremors, crying uncontrollably...something that a GP might mistake for anxiety..... eating carnivore with lots of healthy fats should help with this... by helping you to make more hormones and neurotransmitters of all kinds...hope this helps..piujuq@gmail.com

    Please can you talk more about glucagon? Does low insulin increase glucagon secretion and so increase blood glucose levels? If so by how much? Enough to increase anxiety? If you have already talked about this please point me to it. I'm on very low carbs now.

      Aren't insulin and glucagon the two levers that the pancreas manipulates to maintain blood glucose at the right level? So low blood glucose would stimulate glucagon release.

    Why some other doctors say from 60 on, one should not fast? In order to not to loose muscle...

      Eating healthy fats and protein [animal in both cases] is similar to fasting. Ben says won’t lose muscle unless body fat all gone. Others, like Ted Naiman seem to disagree. You could check in with insuliniq.com.

    Dr. Birman, I understand why insulin is higher in the morning, but why would it normally be elevated in the afternoon?

    Could I ask for a favor? I help newly diagnosed type 1 (LADA) and they already know insulin makes you fat. But it is insulin in excess, insulin also keeps them alive. The statement "First Cut Insulin" is going to be misunderstood as *THEY* can simply cut insulin to their detriment. We cannot cut insulin, we have to stop raising it by eating the foods that raise it in excess.
    Rather than "Cut Insulin", I'd say "Stop elevating it with how you eat". That is the only option for non type 1's but a dangerous concept for type 1. there is something called diabulimia where type 1's try to lose weight by cutting insulin and peeing out a bunch of calories. They usually succeed, then they are discharged from the ICU.

      @@doctorstotrust Oh I have. Major Ben Bikman fan here. Unfortunately, many react much more to titles than content. I have had to address this particular one a few times. You cannot "cut" insulin, you can only reduce the demand for it.

    I'm ketovore for 1year and 9 months. Love it but my a1 c goes up and down. 5 months ago it was 5.5 and now is 6. My fasting insulin has always been on the lower side. Now is 2. My triglycerides are 67 . my hdl 545 and hdl 94. I have had an ileostomy for 2 .5 years, and eating anything other than meat and fat makes it difficult to digest. My MD says I need to spike my insulin for my a1c lo lower and my lipids to go down by eating more carbs,75 to100 grams.
    Do you agree?

      Glucagon allows the liver to release glucose into the blood when blood glucose gets low.....like during the night when you're asleep. You know glucagon is doing its thing if your blood glucose is low. Glucagon is the reason you don't die overnight from a hypo.

    My doctor is pushing osempic and other drugs for weight loss. I haven't been able to lose any weight for 9 months. I'm fasting eating primarily one meal per day. What else can I do to get weight loss moving again? I'm swimming 40 laps 3 to 4 times, per week, I'm using bands for resistance as well as a cable machine and using a treadmill in a walk sprint mod 2 to 3 times per week. My A1C is at 5.4. I'm using ageless muscle to increase muscle mass. I'm 70 years old female. What else can I do?

Check with Gabrielle Lyon perhaps, she is strong on protein and resistance exercise for muscle gain. Building muscle should help many areas, including fat loss

    The guy who gets dizzy after swimming might also have water in the ear that will throw off your equilibrium.

    My doctor told me there was no way to measure insulin. Glucose is the standard. Please explain.

    I know they say salt doesn't cause HBP but I had to quit the salty electrolytes & replace with potassium electrolytes to get my blood pressure under control. I've always needed more potassium which helps me a lot with muscle cramps too. I also use small amounts of magnesium 2x a day to help but can't use large amounts or I get cramps. I think the high mag pushes my potassium down.

      Great points. Many of these doctors say too much salt can be unhelpful. Sodium between 4 and 7g seems best. Hard to overdose for most people. In my opinion.
