    Thank you Dr. Mike, I am 5 months Post Op of my Left knee TKR. I am coming along nicely, I do my home exercises 2x a day 7 days a week. Squats and Chair Squats are part of my routine exercises and I enjoy doing them, but now thanks to you , I am going to start doing them properly. Thank you Dr. Mike!

      ok I am 5 months post double knee replacement (last FEb 3 weeks apart) I have good mobility, but strength still feels weaker. will try this

    Thank you. For me personally, this is the most helpful video I've seen for where I'm at in recovery. I'm four months out of my tkr. I'm still only barely squatting a little bit but now i know what I'm working for and how to do it.

    I’m doing powerlifting at 64 after a TKR and an ankle fusion. I’m not doing huge weights yet for my squats and deadlifts, but I have just benched 225lbs so there’s hope!

    Another timely video Dr Mike, just had similar advice from my physio post TKR but not as detailed. We are practicing mini-squats at present my TKR is doing fine with those but unfortunately it seems to be flaring up my other knee which has a lesser degree of OA so I might wait a little till that settles a bit before trying again. The band seems a great idea I’ll get one of those, my physio really emphasised the power of the glutes too! Now doing more work on that area than the quads, I never really understood the necessity for core strength etc too so this journey has been an eye opener 😳, I’m keen to prevent a 2nd knee replacement so I will be following your channel for expert guidance along the way, Another great video thank you & best wishes from a very wet and windy Scotland! 😊🏴󠁧󠁢󠁳󠁣󠁴󠁿🇬🇧

    Dr Mike, I’ve been training since I was 17 - 78 now . I trained as hard as I could pre surgery, lots of leg work . Both knees done in 2020 - 6 months apart . As quickly as I could I slowly started training again and hiking . My question is , no matter what I do I can’t seem to get back to the strength I had pre surgery, nothing to do with pain . My guess is I replaced the joint but obviously not the connecting tissues, tendons etc . Very frustrating to me but I’m probably expecting too much 🤔

      Thank you so much for watching and supporting the channel! I know it must be frustrating but I do think that there is always opportunities for building more strength even after surgery! I don’t think you’re expecting too much but maybe you might want to think about adding in isometrics and eccentric training! This is a great way to build foundational strength especially if you’re hitting a plateau. Focus on tempo training which basically means you need to slow down the movement and add in pauses. This helps build stability/control throughout the full range of the exercise. This is often harder and requires you to go down in weight but can be very effective! Hope this all makes sense ! I believe you can still do it!

    Dr. Mike, I am three months post-bi-lateral, and I am on my own with the physical therapy so your advice is priceless. My question is about ice therapy, How important is it?

    I’m enjoying your videos and I wish I found them sooner but today is three months and the stairs I was able to do with the assistance of the handrail since the beginning but the operated knee all of a sudden is just hurting when I try to walk up the stairs, so I’m still with the handrail and I’m not happy about it because I want to be able to go up the stairs and down the stairs without assistance. I’m not sure why my knee hurts so much. I’ve been massaging it and even more so since watching your video and I do still go to therapy not sure what is wrong, but I did message my doctor today to see if he can get me in for another x-ray or something because I just don’t feel like this is Normal.

Congratulations on your surgery and if it makes you feel any better many of my patients are not able to ambulate up and down stairs without assistance at 12 weeks. The knee has most likely not developed adequate stability and strength yet but with practice and continued strength training this activity will get easier. The stairs are so difficult for many people because you’re basically transferring your entire body weight to one leg. This is not an easy task even for someone who has had no surgery at all. Stay consistent and don’t let the pain discourage you!

      That is a great question and I’m glad that you want to be proactive about your health! The research also shows that preoperative exercise specifically for a knee replacement can be beneficial in improving recovery and outcomes! Many of the exercises prescribed after knee surgery will be fair game to start implementing and I’ll link a couple of those videos below so you can check them out! Also focusing on quadricep strengthening is going to be key in preserving the strength in one of the largest muscles that supports the knee joints.     Thank you for this video. I am almost 13 wks post surgery on my 2nd knee replacement. I find that my knees and just above my knees experience incredible pain lifting myself up from sitting or lowering myself to a seat. I get the same pain when trying to squat and kneeling is very difficult. What do I need to do to get past this?

      Thank you for stopping by to watch this one and for taking the time to comment here! Unfortunately without evaluating you in person it’s hard to say what is actually causing your pain. I will say though that typically many of my patients deal with stiffness specifically in the quads after surgery. This is due to lack of flexibility which can cause pain over the top of the knee. Here are some suggestions that could help: 1)work on some soft issue massage over the top of the thigh with tools like a lacrosse ball or percussion massage gun (I made a video about this that I’ll link below) 2) continue to work on your range of motion in the knee to restore full knee flexion and extension 3) don’t neglect your hip flexibility because often tight hips can lead to more knee pain.     I lose total range of motion and back to step one so if I bend my knee to 70° tomorrow morning is back at 40

    i have had a totalknee replacement 2 years ago. 4 mounts ago i squated 220 kg . So you can squat after tkr.

      @@MovewithDrMike at this stage, I really can't get my knee to the floor with split squats. is that a goal set of mine that I should work towards? I'm finding that the non-surgical knee isn't strong either, I have arthritis in that knee as well. Thanks

      @@jeffkia808 You don’t have to reach your knee all the way to the ground in order to receive the benefits of the split squat! Just go down as far as your knee will allow. What matters is that you get some single leg strengthening exercises in to strengthen and balance out the weaker legs!

    HI Dr. Mike. I cannot go that far do with squats. I am almost 8 weeks PO. 128 flexion and 4 for extension. I was disappointed with the extension, but my surgeon reminded me that for 2 years I have spent my knee in a bended position to compensate. He said just to always keep in mind to bend knee backwards when standing and walking. He's not worried as he said I am walking well. Also, have you had patients to complain about the tight band feeling around their knee? Sometimes, between pain and the tight band I feel like it limits me. I've heard of people who say they wake up and one day it's gone. I have done Physical therapy every day since surgery including outpatient and at home. Next week is my last outpatient. How long after do you need to do a lot of exercises as I have been doing going forward? Thanks. Love your videos

Moving forward I would treat your rehab program as any other strength program. For strengthening I would aim for 2-3 times per week. For knee flexibility I would focus on stretching at least 4-5x per week which can be very beneficial. Consistency is the most important factor! Hope that’s helpful!

    Hola Mike. tengo un par de dudas. La primera es en relación al Squat. Cuando desciendes, hay que sacar el "culete" o es mantener la espalda recta. La segunda, justamente tiene que ver con el ejercicio de peso muerto, sea con kettelbell o mancuerna., hay que sacar el "culete" o se mantiene la espalda recta. Creo que falta una toma en el video completamente de perfil para despejar dudas. Gracias por tu respuesta. Un abrazo.

      Thank you for your question and I appreciate you stopping by to watch this one! So with the squat whether it’s with or without a weight like a DB or kettlebell I would focus less on what’s going on with the back and focus more on what’s going on at the hips.
      The instructions that seem to help people the most when talking about the descent (down portion) of the squat is:
      1) start with feet hip width apart
      2)hips drop down and backwards
      3)knees tracking over feet
      4)chest tall to prevent your hips from rising too high.

    Dr Mike, With the help of a trainer, I am trying to strengthen my legs ~6weeks prior to left TKR
    b/c of bone on bone. Right knee is not as bad but hurts during exercise. Squads aggravate my knees. Should I try to ignore pain? Do you have suggestion of what to do pre-surgery? My ROM isn’t great & I’m trying to improve it pre-surgery.

This is very important and can definitely speed up your recovery time! I would recommend that you try to exercise as much as you can tolerate trying to work through some of the pain. You want to perform exercises that are going to allow you to keep moving the next day not exercises that leave you in pain for days!     Hi Doc, can I ask as I’m about to go in for surgery in December. I love physical activity. One thing that has been great for my mental health is boxing training. I have been told I may have to give this away. That hurts a bit. What are your thoughts?

As for boxing I don’t consider this to be a high impact activity when compared to others like running or playing basketball so I wouldn’t count it out just yet! You’ll be surprised how good you’ll feel 6-8 months after surgery as long as you do the proper rehab!

    Why is it when I bend my leg during exercise right after I lose what I gain and have to start from the beginning

      I totally understand your frustration and I get that at times it may feel like you’re going backwards. But this is pretty typical and at the early stages it’s very easy for the surgery knee to get stiff and tight. The connective tissue (muscles, tendons, ligaments, fascia) around the knee are healing and swelling can vary day by day which can greatly affect your flexibility. Good positioning and daily range of motion exercises are important and over time your flexibility will improve. Keep up the work I’m rooting for you! Thank you for watching!
