    Leonard Nimoy was chillingly effective at playing a sadistic sociopath murderer in this episode. He could have made a career out of playing bad guys in the movies if he had wanted to, he was that good at it. He was scary in this.

      Reminded me of “Mirror, Mirror” in the original “Star Trek.” The novelized version of the episode implied Spock had a devilish appearance at Kirk’s first meeting, seeing his mirror universe counterpart brought those memories back to Kirk.

    Dr Mayfield was TOO smart for his own good. Killing Sharon & taking her purse would have looked like a simple mugging with no real evidence. Even if Columbo found out about her calls to the company and other small things it never would have led as far as this complicated frame-up over drugs which was just too obvious a setup. But more than a few killers felt a need to flex their genius to set up a complex situation that just didn't add up and made it clear something was planned.
    Also he left that syringe clean of fingerprints. A junkie with no gloves dosing himself yet the needle is wiped clean? I bet Columbo checked that- it was a loophole that he'd check in later murders, too.

    This is easily the most heartless and cruel murder in the whole series. Mayfield is also the most ruthless and remorseless killer in the show, and the casting of Nimoy could not have been more perfect. The experience of playing a emotionally-void Vulcan made him a spot-on choice for such an icy, remorseless doctor.

    This was the most evil of all villains on this show. Unlike most of the Columbo killers, he engendered no sympathy from the detective or the audience.

    The way Mayfield waited to ambush the guy was creepy. Sitting in the dark, looking straight forward, hands folded, knees crossed like a girl: the posture of a snake ready to strike.
    I loved the psychodelic imagery to show the guy was stoned.

    The saddest part about this murder is Columbo literally knew IMMEDIATELY that Sharon's ex didn't OD. So Spock goes through the effort to frame and murder this poor guy and the ploy doesn't even remotely slow Columbo down.

      @@TheSuzberry Agreed. Columbo could have solved this case very easily by having Mayfield followed. He wouldn't have had the problem in finding the suture later in the episode and it would have been much stronger evidence in court if he could catch Mayfield attempting to incriminate the boyfriend.

    This guy had no alibis that I can remember. Just plain confidence and position to fend off Columbo. That in itself is unusual.

      He did a lot of footwork though, to try and make it look like Harry did it. And as Colombo got closer, he planted more evidence. Many Colombo villains just sit back and try to stomp Colombo with their ego alone, but Dr Mayfeild was more, I guess, proactive. As the narrative shifted, he had to change strategy. His one attempted murder, led him to commit 2 other murders, just trying to cover his tracks.

      Very true - and Columbo all but tells Mayfield that he is onto him within seconds of meeting him when he points out that he was callously winding his desk clock while supposedly being told of the nurse's murder for the first time and didn't react to the news at all. Yet the Surgeon figured he was smart enough to outmaneuver the Lieutenant regardless.

    I would have to say that this was pretty good acting. Nimoy went from playing an emotionless alien to an evil murderer that everyone despises and hates.

      I admire the way that in these crime dramas every little detail is there for a purpose. This guy takes him out a cigarette while Columbo is talking to him. Does the viewer take any particular notice of that? Probably not, but they remember it. Then when Dr Mayfield (aka Mr Spock) comes along to do his dirty deed, thinking he's being so clever, he jabs the needle into the wrong arm - the left arm. Cos Columbo, who misses nothing, has noticed by virtue of the cigarette business, that this guy is left-handed. I'm not being cynical, I really admire this kind of plotting. I'm an amateur writer of sorts, but I could never produce plots like this.

      This episode is sadly marred by suspect audio. Check the clip when the surgeon mentions the different suture colour. The soundtrack triggers about 3 to 4 identical reveal jingles.

    They should’ve had Michael Caine play as one of the villains in one of the episodes. He was awesome in a shock to the system as a serial killer so he could’ve played a character similar to mayfield.

    The mondo Columbus episodes are too short. The old 1950s non Rittenhouser mondo Chawin himself found guilty of 6 counts of racism mondo TVs couldn't hold longer ones. They used old CRT Catholic Ray Tubes with flypaper control. They used to use Central Vacuum Theory with CRT or Catholic Ray Tube technology and Motorola involvement to try to make a non racist product.
