I think it might be because earlier people were being too general: He mostly seems uncomfortable around surgery.
Columbo had him with the clock....he knew this was his guy. Same happened in “A friend in Deed”....calling in Columbo when he didn’t know there had even been a murder. It’s the little things. A murderer has 100 different ways to get caught. If he can eliminate 50 of them...he’s a genius. That still leaves 50 ways to get caught.

Usually his nice, well mannered and fumbling persona makes people open up so Columbo can identify those inconsistencies and loose ends to finally catch the murderer. However with Dr Mayfield, who was particularly adept at covering his tracks, he realized he had to make him very uncomfortable to force him to show his hand. In the end, it was Mayfield's fear of being caught that ultimately led to the Columbo getting the physical evidence he needed.

To be fair to Mayfield, he probably hears a lot about deaths / accidents in his line of work so a doctor is typically going to be calmer about these sorts of things than the average person, so adjusting a clock is probably no different than pouring a cup of coffee. I thought this was rather shaky to identify him as a suspect to be honest.

Dr Mayfield was even more cold-blooded than Spock. But he didn't drive Columbo nuts. Columbo feigned that burst of anger to draw Mayfield out. When he didn't, and then Mayfield had his outburst in the operating room, then Columbo had him.
He's also probably the closest of all of them to getting away with it. Columbo knew it was him from the beginning but was perplexed about the proof until the very last minute of the show.
Yeah man. Columbo may be able to be respectful towards and even empathize with killers who are remorseful, but he's got zero patience for ones who are genuinely remorseless.
I would like see more comment sections on different Columbo episodes, not on the same episode over and over again, this being the 4th comment section on just this one episode in the last 2 weeks. Smoking in hospital rooms must still have been allowed when this episode was made, as the smoke from Columbo's cheap cigar can clearly be seen rising when he is talking to the doctor in his office.
Dr Mayfield's big mistake here was trying to frame her ex-boyfriend for the murder when Columbo was hot on his tail. That was a big no no, especially after we find out the boyfriend had been injected into his left arm when he was left handed.
It's not lost on me how methodically the tracks were being covered. He stages a robbery, doesn't break the windows, takes maybe 10 things out of the cabinet, as he walks away from the hutch he lazily knocks down that book instead of strategically placing it away from the rest of the pile, he pushes one chair over but not the other, he also doesn't mess up the items on the desktop. He does this all while still wearing gloves AND does it in little more than 40 seconds. It looks just like a room that actually got ransacked for jewelry.

Most cold hearted killer but least successful, he didn’t kill his intended victim. He made things too complicated and they snowballed on him. Being a doctor, don’t kill people using the the method that reminds cop of your profession.

And they were able to do that in AITF. So besides being a groundbreaking landmark show that tackled a wide range of issues there also was a political objective to the show..

That is some type of skill to be able to take an old video, some old TV show, and put it on the Internet. I don't even know where to start to do something like that. Mrs. Video wants to take a class at the junior college to learn all that, but I don't know, that seems pretty expensive, and this whole thing is beyond me.
I sure would like to know how to do that. Maybe you could just give me a simple step by step. That is, if you have time. I don't want to take all your time. I know you're very busy.
