Often Dr. Fruitman is asked" How do you know what medication to choose?" Here is some insight to the thought process behind choosing the right medication for you!
Both Ritalin and Adderall belong in the same stimulant category. Once key factor in choosing is the patient's preference and attitude toward each drug. If a patient doesn't take their medicine, it won't work!
It is also important to know the patient's previous history with each of the drugs. Typically, Dr. Fruitman says, it is best to start on a low dose in an extended release form, and Adderall may be best for motivation purposes, while Ritalin may be a better choice for over-stimulation.
Both of these drugs have proven effective in treating ADD/ADHD. The most important thing to consider while deciding a course of treatment is finding the right dosage.

I want to stress an important point he makes multiple times in the video after I [male, 25] have now taken prescribed medication for 6 months: Dosage matters. Picture your proficiency of different traits (motivation, patience, hyperactivity, etc etc) as different numerical stats or bars, akin to a video game. Medication will boost some stats and nerf other stats. If your goal is productivity, too much nerfing of stats can actually lead to less productivity, even if all the important stats are off the rails high. My ability to focus can be crazy good, but it won't mean anything if my stomach is killing me and I can't choose what to focus on.
If you're watching this because you're struggling in school/work or perhaps want the extra edge in school/work, I understand your struggle and respect the grind. Illegal adhd med use is rampant in wealthier American high school and college campus cultures and I don't endorse it. I'll level with you and say recreational use is relatively low risk (physically, not legally). No matter what you do just please know what you're talking about when you discuss adhd. You may be using medication to get your headspace from a 9/10 to a 9.8/10, but many people take it to get from a 3/10 to a 7/10, or comparable. ADHD is a serious thing, and I just ask that you respect those who have more of a need for it then yourself. If the need is important enough to you, consider getting a screening for a diagnosis if you are in a financial position to do so. In any case, be safe and best of luck with everything 🤙🤙🤙

@@cbreazienutz Yeah i used to take 30mg of ritalin and it made me an introvert and quiet and im an extrovert. I lowered my dosage to 20mg and its perfect. If you feel ritalin is hitting you hard the lower your dose and see how you feel on that

Great video for advice on properly medicating myADD. I had my concerns and questions for my own doctor after being prescribed a stimulant. I feel more comfortable about this medicine now because of Dr. Fruitman.
Really good doctor for taking the patient’s account into consideration. One of the doctors I had in the past would see this as drug-seeking for recreational purposes. It’s understandable to be suspicious and cautious as a doctor, but that doctor was so paranoid about it, that I never understood why he even bothered to prescribe this medication.
Ritalin makes me anxious and excited. Adderall makes me calm and focused. Anyone else?

Same. Ritalin lasts about 2 hours for me and then I crash so hard. Adderall is much more smooth and calms me down. I personally did not do well on Ritalin. Everyone is different but adderall all the way for me.
I normally take adderall but it is almost impossible to find currently. I asked my dr. For a substitute and was prescribed Ritalin. It doesn’t do crap for me other than increasing my heart beat. Adderall actually gets rid of anxiety for me and I feel like I can sit still and in peace so to speak. Took Ritalin for the first time today and I hope they get this supply issue sorted out soon. I can go a long time without medication but I drive everyone around me nuts and I can’t sit still,( that drives me nuts.) When I take adderall I don’t need anti anxiety medication. When I don’t take it I feel extremely anxious and sometimes a massive panic attack can happen. Those things suck because I honestly feel like I am about to die. One day the panic attack might actually be a heart attack. Anyway Ritalin sucks for me.
@@allEyezOnDelphi at least my tolerance will be mostly gone. Will try to take half my normal amount and see where I am because I haven’t taken any for almost one and a half months. Not easy, I came very close to quitting my job and made it well known because without medication I can get mad very easily and have told many of boss to go fuck them selfs without thinking about the consequences after. I also get extremely bad restless legs at times and is miserable. Make me want to chop off my legs.
the same thing happened with me, vyvanse is definitely a better alternative to adderall in my opinion than ritalin
@@charvolute I was prescribed Vyvanse recently but unfortunately without heal insurance, it’s like $380+ and I can’t afford that so Ritalin it is. Hopefully it’ll help me out some.

Thanks for taking the time to explain the drugs but I'm a little unsure whether you understand the irony of your music selection. what I noticed with videos about ADHD. They made sure they not too long.... so u don't loose focus.
If never taken stimulants before, perhaps methylphenidate first. If inadequate, then perhaps amphetamine. If patient cannot tolerate, then switch to non-stimulant.
In the USA we have many as well. Dextroamphetamine is Dexedrine and was a bit more widely used before Adderall hit the market in the 90's.
Idk why they switch to adderall, Dexedrine has a lot more benefits to it than adderall like lower blood pressure and heart rate. To me it seems safer and more effective and I’ve tried both and def prefer Dexedrine

I was prescribed methamphetamine xr in the past. The pharmacy has been having trouble getting any in stock. Since they don't have any my doctor switched me over to Adderall. I have been taking it for 2 months now and have noticed that I get set off and become very agitated and borderline aggressive on the Adderall. I'm a adult that takes Ritalin 20 mg. twice daily, would that be equal to 40 mg.? I take it on a empty stomach. My psychiatrist selected Ritalin for me because it is milder than Adderall. I take for ADD/ADHD Symptoms. I also take Zoloft for depression and anxiety. My question is, because I take the generic prescriptions for these medications do the work the same as the name brand or as effective?
The generics by law have to work just as effectively as the brand name since it's medication, there can be only very minor difference.

I'm 18 and am prescribed concerta 36mg. It allows me to concentrate very well but I'm still procrastinating. I don't think any form of amphetamine medication is prescribed here in Iceland but I wish I had that motivational kick in the butt amphetamine medication seems to provide.

I have been diagonsed with ADHD since i was in my early 20s. I have taken both ritalin and adderall now, immediate release and extended release for both. i've tracked the effects carefully and feel its slightly more than 2x ritalin for the equivalent effect of 1x adderall. there isnt much of a difference in regards to their effects in my personal experience. benefits of both are they make me wired and able to concentrate on a task for several hours at a time. i've studied and learned so much thanks to these drugs. side effects: dry coughs, especially with ritalin. the worst was biphentin (an extended form of ritalin). the coughs were so bad throughout the day that i felt pain down to my lungs. crashes (feel terrible next day if not taking any) is another side effect. surprisingly, the immediate release taken throughout the day had MUCH less crash than the extended release drugs. maybe having these drugs constantly in your system for too many hours at a time is the reason why you experience withdraw symptoms later?
I got diagnosed at 6, those are rookie numbers!

I am understimulated. I need adderall to get things done. Now 6-7 months later my dosage of 15mg no longer works. Is there any other alternative medication that works with motivation like Adderall does? I want to switch to another medication so my Adderall tolerance goes down.

@@josiahbicelis304 No I am off Adderall now. Dumb doctors do not know that I have idiopathic Hypersomnia which makes me sleepy 24/7 and even when I try to show them proof they deny seeing the paperwork.

I would gladly take sttratera if it was available in my country, but not a stimulant. I already have too much anxiety as it is, so yeah, I'll pass on the stimulants, even though I have severe ADHD.
I have anxiety too, but I find Dexamphetamine reduces my anxiety. Anxiety is mainly from too much norepinephrine. Now stimulants like Ritalin or Adderall increase serotonin, norepinephrine and dopamine. But dopamine itself, reduces the production of epinephrine. An ADHD brain has a deficit in dopamine, so theses stimulants bring up the dopamine, which will reduce epinephrine and slow down the brain, but at the same time, the brain becomes primed to form reward pathways with boring tasks.
Strattera operates similarly, but it takes awhile to build up. I don’t know why your country doesn’t have Strattera?

Ritalin is much better. I took 10 MG extended release addaral one day a few weeks ago on a day off and boy was I stoned af. Now I stick to Ritalin. I feel like my focus is sharper than the knives I use at work and I feel much more alert too with motivation. Adarall is yucky to me.

Amphetamine has two isomers: dextroamphetamine and levoamphetamine. Dextroamph is the active dopaminergic CNS stim. Levoamph acts on norepinephrine (which increases wakefulness and alertness). Adderall is just a combination of dextroamphetamine and racemic amphetamine (both dextro and levo) salts. You guys just get straight up generic dextroamphetamine.

Actually, they are not in the same category. If you look at the molecular structure of Ritalin, the “backbone” of the molecule is related to cocaine, whereas, adderall is purely an amphetamine. Although they are both psychostimulants, they are actually at opposite ends of the spectrum

The boost of energy and the feeling it gives is 100% addictive to anyone with addictive personality disorder but, the patient has to be honest if they have had issues in the past with addiction. You know and I know that isn't the case. They are drug addicts and will not upfront answer yes to the clinician when they ask them. That is how people get really hooked on these medications. They may not be the same as Meth off the street but, I agree they can/do ruin a life just as easily and do so much more than reported.

Probably too high of dosage or took your dose too close to bed time.
You can’t sleep while stimulated.
Or you are like me and have CYP2D6 gene dysfunction which is super important in breaking down amphetamine. Where as Methylphenidate is broken down by CYP3A4. Just because a medication has weird reaction in your body doesn’t mean your illness doesn’t exist or most importantly cause significant distress. I just wanted to let everyone know why people have polar opposite responses to ADHD Treatments. Your genes literally control your brain more then your mind. That is all.

That's common, you cant just quit it, you need to lower the doses and skip days gradually

Adderal is great but since it’s basically straight up meth the government made legal, you forget to take it one day and the come down can be hard. It’s addictive but good, Ritalin is another good one, more focus (on what ever you think is the easiest task distraction) and less burnouts

I was diagnosed with ADHD/ADD today at 23. Doc prescribed me 10MG Ritalin per day once per day. I am 200lb male.

I hate Ritalin it makes me a spaced out catatonic zombie. NOPE. I'm on adderall but I need Vyvanse and can't afford it so this is all I have to treat my severe adhd. Had it all my life and have never properly functioned. Since my brain is naturally adhd wired too, the meds make my brain feel so weird since I'm not use to actually functioning. I'm so mentally drained all the time but have learned to get use to the exhaustion. Memory is getting so bad and my mind is always everywhere. Basically an adhd brain is the literal definition of living chaos. I wish people weren't so negative and mean about it and to people that have it like me. I'm constantly apologizing for being my hyper and chaotic or soaced out self. The world literally makes u feel bad for TRYING to function as best u can. Even so, I'm still as kind and loving as possible bo matter what. I can't help it I'm wired to be positive and nice even when I shouldn't to some. It's a gift and a curse. My psychiatrist prescribed me Ritalin. Based on this video, I think Adderall will work better. I haven't gotten the insurance for the Ritalin yet, but someone gave me a couple Adderall to try in the meantime... well, it was phenomenal. Adderall works very well for me, and now I'm nervous to find out about how Ritalin affects me because I read that the side effects are a bit different... that Ritalin can make you nervous (I'm already a nervous person) or lethargic (already there).
Same issues here, been using Ritalin for a few years now. It works fine when it's basically coming up or at its peak, right afterwards? I turn into an unmotivated emotionless zombie that only knows irritation and lacks all motivation... sure my head is calm but the downsides outweigh the positive.

You shouldnt be high after taking this medication, the purpose of taking this medication is to be more productive and focus, if youre using it to be high then you should talk to your doctor cuz they mightve prescribed with too much dosage.

Plenty of people do. They say it’s just like taking meth and that it’s for lazy or stupid people. But I don’t think that’s true. Neither are the stigmas against antidepressants. Do you take of these? I’m considering a stimulant after having been diagnosed with ADHD recently.

@@sandizz That's great that you feel fine. Some people have different experiences with them. They'll be okay for a while but then they won't. Or they need to try different ones. I see what you mean though. I've heard a lot of horrible things said about antidepressants and about depression in general. It's not right. It should be taken more seriously.
As a practitoner you should know, that it is impossible to generalize this in the way you do. The answer to the question Ritalin or Adderall depends on the patient and most of all on his comorbidities.

Thank you for your input, I believe your intentions are pure! However, asking someone with ADHD to sit quietly for an extended period of time, unmedicated, is at best counter productive. If someone said "OH MY GOSH, I MEDITATED AND MY ADHD IS GONE!", then they likely did not have it in the first place. I cannot clear my mind. I cannot find a nice quiet space to get grounded and be one with the earth and nature. Please keep encouraging people to be healthy and seek alternative options for mental and physical wellbeing. But keep in mind that the one thing you ask is basically impossible. It's like asking someone with advanced emphysema to take long, deep calming breaths..in and out... Your answer to the problem is just not having the problem.
