She's been dealing with depression. It might be it too. It's like sometimes she can't control her self, and no matter what being a celebrity she needs to act normal or ok in front of everyone. But when she not, she can't control it. The scene with cameras might signifies that everyone is watching every moves she does. The roses that the audience are throwing might be the "comments" that whatever she does good or bad they still say something hateful about her and it cause wound or more pain to her.

So for the theory that is circulating now is : this mv is actually portraying taeyeon’s real life experience, she fell in love with someone and was in a relationship with him but he actually only loved her in front of ppl ( mostly audience) and made it clear for her that they’re only a couple in the play not in real life but she couldn’t control herself that’s why she went through depression she got crazy, also when she was screaming ( that represents all her sad songs about heartbreaks ) the audience were throwing flowers to her that were hurting her face and were clapping and happy about her performance thinking that she was just acting while she was in real pain then in the end while she was washing her hands notice the ring that fell ( that ring was also in the lyrics) and is representing the relationship of them , then she just took it off bc she wants to be free then when the curtains were closing she made that sad smile because nobody understood her feelings and pain so she just abandoned expressing herself.

Thank you so much for your Taeyeon reaction , For me the MV talks about Taeyeon's depression The actor and the relationship she has with him represent that, when they are fighting represent the bad days, When they are happy the good days
That's why when she's in front of him she can't control herself, it's not that she doesn't want to, it's that she can't and she feels like it's going to explote, In the scene with the birthday cake he appears behind all the other people Implying that even on her special or happy days he (depression) continues to appear and doesn't leave her alone That's why when Taeyeon says that she will return to him, she doesn't smile because it's not a good thing, but she knows perfectly well that she can't control when to feel bad or when to feel good she can't control herself indeed
The last scene is the deepest for me because although she has already taken off her wig and is no longer acting but rather showing herself as she really is, showing her true feelings people continue to see it as a great performance, she is collapsing but the public is still indifferent thinking that it's just another performance and they throw roses at her, which fill her with wounds Those wounds are the ones tae is cleaning at the beginning of the video.

I believe it's about toxic relationship. And in this case she thinks she's the toxic one and she can't control herself, so that's why she said "it feels like heaven, it feels like hell".
I think the combination of MV and the song itself is pretty deep, how do you get out of a toxic relationship when you're feeling in love with that person?
