I'm pretty sure what happens, at least on Fearful Harmony, is that the PS can't identify the disc but tries to read it anyway. This takes up a bunch of resources, meaning the PS logo plays through but at an extremely slow speed due to lag. The result is the sound. EDIT: Apparently this guess I made was mostly right. Neat.
Yeah, it is basically a MIDI playing but the notes keep getting played over and over because the PS1 is lagging so bad. The notes themselves seem to play at the correct speed.
FluxGigawatt Reading discs uses up no resource whatsoever except drive calls which are worth nothing. Fearful harmony is either the result of a corrupted/modified BIOS or a homebrew disc on a modded PS1 which somehow employes code hijacking which in turn edits the BIOS RAM data anyway.

That is exactly what the PS1 does, it plays the sound of wind chimes to ward off negative energy in hopes that the disc would eventually be read. Note: There was a bit of plagiarism involved there.

I know what fearful harmony was made!: Is a combination of a sound of BIOS_STARTUP_8 and BIOS_STARTUP_9, they extended the sound to a loooong sound. Then, added to the ps1.
Why fearful harmony happens: The PS tries to read data, but can't find it. Because it is trying again and again it lags the PS a lot. The sound is the PS intro but slowed way down because of the lag. Scared me shitless when I was younger

I feel like the harmony wouldn’t be as fearful if it displayed a normal error message and not a bunch of graphics vomit. Well... and if it sounded less like an army of wind chimes.
The thing is, the visuals are different every time, so sometimes you can get a bunch of graphics vomit, the PlayStation diamond thing but cut in half or you could even get the visuals to be a “Forbidden Image”
I must correct something :) 1. Personified Fear happens also when you insert PS2 CD-ROM based game and wait about 2-3 minutes on SONY logo screen. I've tested it many times on many consoles. 2. Insert Disc/Force Restart is not a "rare" screen. It's just the same screen like "Insert PlayStation format DISC", but this version occurs on PAL models from 1002, to 5552. US and JP consoles have only "blue BIOS" from 1000/1001 to 9000/9001, but EU consoles are a little bit different. 1002, 5502 and 5552 models have "old style" tiled background BIOS, which has this "rare" error screen with black disc and PSX console pictures. In Europe only 7002, 7502, 9002 models have the "blue BIOS" same as in JP/US models with "Insert PlayStation Format DISC" error screen. Greetings from Poland :)
BIOS basically manages boot up of the console. Stands for Basic I/O (Input/Output) System, every single console or device that get its BIOS corrputed/bricked literally has no hope, not only the PS1.
the reason you can't easily get the force restart error is because you have a modchipped system. non-modified systems will always get the INSERT DISC background

Hi random person in the comments. Sit down, take a load off, and listen to my story of horror. It was a night like none other. I would pop in Final Fantasy Tactics, playing it for the fifth time. But I was pretty young at the time. I accidentally put in the wrong disc, and basically exploded in fear at this tune that played. My parents were both out of town, and my sisters both slept upstairs. I was petrified. I had no idea what this screen was. Luckily, I hear steps coming down the stairs, and my sister walks into my room. "Hey dude, do you hear some high pitc- what the heck did you do to dad's PlayStation!?" She yells at me. I don't respond. Instead, I break out in tears. She assures me I'm okay, and then, she unplugs the PlayStation. Now in 2018... That PlayStation still is under my bed now. I still haven't touched it at all. Anyone wanna buy it? 200 bucks.
Basically there was the ps1 logo in the corner, tiles across 70% of the screen, and a big old streak across the screen. And yes, it happened when the screen turned black.
Am I the only one who gets on edge, sick to my stomach, and paranoid, whenever I hear fearful harmony? This shit used to play a lot at 2:00 AM because my old PS1 had an error where it would turn itself on, and I had an unreadable disc, the open latch was hard to get, and I was too scared to unplug it... I was like 4, when it happened. It still creeps me out just as much as it did back then.

What Fearful Harmony does: the console tries to read the CD-ROM data, but it appears to be scratched. The PS1 then gets corrupted by the invalid CD-ROM data and ends up glitching the Playstation audio driver, thus making glitched corrupted Harmony from the startup but looping it due to a error, and the console still wants the data so it continues to read the disc.
I never had a PS1 so I cannot confirm, I don't do disassembly of consoles anyway so I cannot tell exactly what happens during the personified fear error.

Fearful harmony is really scary like imagine being a kid and you accidentally puted the wrong disk in and you see the screen glitching and making a weird sound like kids be screaming and crying for help
List of the PS1 errors that I find to be creepy: 1. Fearful Harmony (It used to scare me when I was little. Fearful Harmony, the PS2 red screen of death and the PS2 memory card screen used to give me nightmares.) 2. Personified Fear (This used to startle me and give me goosebumps. Both Personified Fear and the PS2 memory card screen used to make me jumpy. The PS2 memory card screen still makes me jumpy now.) 3. Forbidden Image (For some reason, I find this to be twice as creepy as Fearful Harmony. Unlike fearful harmony, the forbidden image doesn't make very creepy sound effects that slowly increase. Also, on the forbidden image, the orange diamond doesn't appear on there. It's all black or a very dark gray.)
BK4 Fearful Harmony's very creepy sound effects that slowly increase remind me of very creepy music from a scary movie. That's why I put fearful harmony as number one on the list.
2:24 This is the sound that should originally play in the menu but has been corrupted and never worked. Instead, it's trying to get all the animations and textures on the screen as "main menu"
Ah, that’s why it sounds so ethereal. When the files got corrupted, it probably couldn’t just be deleted, so it makes sense with the over lapping and repeating sounds.

The Huskgamer I got that once when I put a Japanese disk in my English PS2. I didn’t know there was a border for countries and it scared me pretty bad. I knew what it was though and I wasn’t so scared that I didn’t play for weeks or anything.
The problems were fixed by PlayStation 2. These uncontrollable errors are the scariest. I wonder if the fearful harmony is remixing the Sony or PlayStation 1 start-up screen or if it made it's own music for some unknown reason. Is Fearful Harmony always random? Every video I see of it, it always sounds the same, or maybe it's different, but I'm not catching onto that, I'm hearing them incorrectly, or I'm not remembering them correctly. What about the PlayStation 4 beeping error? Is that a glitch?

Personified fear is the scariest error ive ever heard of/seen. The unease you feel after that REALLY hard noise is just disturbing. After that noise, You only see a black screen. Nothing. I always turn down my volume incase i get scared. So yeah.
Not just that, it keeps going and going and going, at least Personified Fear is just a bit weird, going black and silent when you expect to see a logo.
GeoNeilUK I find the software terminated error way creepier. Not only does it show a big sign, but imagine it happens on, say, spyro 2. It sends me a strong sense of unease because if you played it in a modchipped ps, than you basically lost everything on the game, if you already had stuff in it. And to top it all off, It makes me think that pretty much every character is gone. Hunter is gone. Elora is gone. The professor is gone. Ripto is gone. Crush & Gulp are both gone. Even Spyro is gone. Also, the text can go completely black.
I can certainly see where you're coming from with the Software Terminated screen. Smash cut to *DEATH!* And the screen basically accuses you of wrongdoing too!

i can get it very easily. Console shouldnt be modchipped and you need a disc that is not ps1 game its that easy

Is a combination of a sound of BIOS_STARTUP_8 and BIOS_STARTUP_9, they extended the sound to a loooong sound. Then, added to the ps1.

Why fearful harmony happens:
The PS tries to read data, but can't find it. Because it is trying again and again it lags the PS a lot. The sound is the PS intro but slowed way down because of the lag.

@Camel King Clarith That's Personified Fear, Fearful Harmony is the "pring" sound that when it starts up normally is along with those 2 sounds.
The thing is, the visuals are different every time, so sometimes you can get a bunch of graphics vomit, the PlayStation diamond thing but cut in half or you could even get the visuals to be a “Forbidden Image”

1. Personified Fear happens also when you insert PS2 CD-ROM based game and wait about 2-3 minutes on SONY logo screen. I've tested it many times on many consoles.
2. Insert Disc/Force Restart is not a "rare" screen. It's just the same screen like "Insert PlayStation format DISC", but this version occurs on PAL models from 1002, to 5552. US and JP consoles have only "blue BIOS" from 1000/1001 to 9000/9001, but EU consoles are a little bit different. 1002, 5502 and 5552 models have "old style" tiled background BIOS, which has this "rare" error screen with black disc and PSX console pictures. In Europe only 7002, 7502, 9002 models have the "blue BIOS" same as in JP/US models with "Insert PlayStation Format DISC" error screen.

the reason you can't easily get the force restart error is because you have a modchipped system. non-modified systems will always get the INSERT DISC background

It was a night like none other. I would pop in Final Fantasy Tactics, playing it for the fifth time. But I was pretty young at the time. I accidentally put in the wrong disc, and basically exploded in fear at this tune that played. My parents were both out of town, and my sisters both slept upstairs. I was petrified. I had no idea what this screen was. Luckily, I hear steps coming down the stairs, and my sister walks into my room. "Hey dude, do you hear some high pitc- what the heck did you do to dad's PlayStation!?" She yells at me. I don't respond. Instead, I break out in tears. She assures me I'm okay, and then, she unplugs the PlayStation.
Now in 2018...
That PlayStation still is under my bed now. I still haven't touched it at all. Anyone wanna buy it? 200 bucks.

Am I the only one who gets on edge, sick to my stomach, and paranoid, whenever I hear fearful harmony? This shit used to play a lot at 2:00 AM because my old PS1 had an error where it would turn itself on, and I had an unreadable disc, the open latch was hard to get, and I was too scared to unplug it...
I was like 4, when it happened. It still creeps me out just as much as it did back then.

The power button was sticky and would short because as a kid, I spilled my juice box in it. That's the only reason I can think of for it doing that.

What Fearful Harmony does: the console tries to read the CD-ROM data, but it appears to be scratched. The PS1 then gets corrupted by the invalid CD-ROM data and ends up glitching the Playstation audio driver, thus making glitched corrupted Harmony from the startup but looping it due to a error, and the console still wants the data so it continues to read the disc.
So, Sony did not do that?
I never had a PS1 so I cannot confirm, I don't do disassembly of consoles anyway so I cannot tell exactly what happens during the personified fear error.

And it usually occurs when you insert a pirated/damaged or unreadable disc.

List of the PS1 errors that I find to be creepy:
1. Fearful Harmony (It used to scare me when I was little. Fearful Harmony, the PS2 red screen of death and the PS2 memory card screen used to give me nightmares.)
2. Personified Fear (This used to startle me and give me goosebumps. Both Personified Fear and the PS2 memory card screen used to make me jumpy. The PS2 memory card screen still makes me jumpy now.)
3. Forbidden Image (For some reason, I find this to be twice as creepy as Fearful Harmony. Unlike fearful harmony, the forbidden image doesn't make very creepy sound effects that slowly increase. Also, on the forbidden image, the orange diamond doesn't appear on there. It's all black or a very dark gray.)
BK4 Fearful Harmony's very creepy sound effects that slowly increase remind me of very creepy music from a scary movie. That's why I put fearful harmony as number one on the list.

The problems were fixed by PlayStation 2. These uncontrollable errors are the scariest. I wonder if the fearful harmony is remixing the Sony or PlayStation 1 start-up screen or if it made it's own music for some unknown reason. Is Fearful Harmony always random? Every video I see of it, it always sounds the same, or maybe it's different, but I'm not catching onto that, I'm hearing them incorrectly, or I'm not remembering them correctly. What about the PlayStation 4 beeping error? Is that a glitch?

Personified fear is the scariest error ive ever heard of/seen. The unease you feel after that REALLY hard noise is just disturbing. After that noise, You only see a black screen. Nothing. I always turn down my volume incase i get scared. So yeah.
Fearful Harmony is far and away the creepiest error.

Not just that, it keeps going and going and going, at least Personified Fear is just a bit weird, going black and silent when you expect to see a logo.
GeoNeilUK I find the software terminated error way creepier. Not only does it show a big sign, but imagine it happens on, say, spyro 2. It sends me a strong sense of unease because if you played it in a modchipped ps, than you basically lost everything on the game, if you already had stuff in it. And to top it all off, It makes me think that pretty much every character is gone. Hunter is gone. Elora is gone. The professor is gone. Ripto is gone. Crush & Gulp are both gone. Even Spyro is gone. Also, the text can go completely black.
