GFR: Understanding Glomerular Filtration Rate & Kidney Disease with tips to improve kidney function

  • Episode 60 - GFR: Understanding your Glomerular Filtration Rate (GFR) & Kidney Disease with tips to improve GFR. If you have kidney disease, then you have heard about a measurement called GFR. Did you know your GFR is NOT a measurement of kidney damage? Let's take a look at what is GFR, how it is used with kidney disease (CKD), and how to improve GFR levels/rate.
    • GFR stands for Glomerular Filtration Rate. This measurement helps determine how well your kidneys are functioning. It measures the volume of blood needed to pass through the kidneys in a given amount of time in order to filter a given quantity of a substance, given in mL/min. The easiest and most common substance to measure is Creatinine and that is what we will focus on in this video.
    • The GFR looks at the rate at which your kidneys are filtering your blood - not how much damage you have. Those with damaged nephrons (filters), blood vessel damage within the kidneys or reduced blood flow to the kidneys will have a lower filtration rate.
    • GFR can’t be measured directly, so estimation is done based on one of several available formulas looking at your age, race, gender, the amount of creatinine in your blood, and possibly other factors. It is also possible to do a creatinine clearance test using a 24-hour waste collection to get your estimated GFR or eGFR
    • The eGFR test may also not be right for everyone. It may not be accurate if you are younger than 18, pregnant, very overweight, or very muscular.
    • Your GFR tends to decline naturally as we age, even in people without kidney disease.
    • The GFR scale goes from 0 to 120. Normal will vary with age since it declines naturally, but typically it is: person in their 20s GFR 116, 30s 107, 40s 99, 50s 93, 60s 85, over 70 75.
    • If your GFR is not normal, your doctor will do other tests. Doctors look for abnormal leaking of protein, sugar, red, or white blood cells.
    • There are 5 stages of Kidney Disease and the stage you are in is determined by your GFR.
    • Since the GFR scale goes from 0 to 120 - it is not a direct conversion to the percentage of kidney function remaining. A GFR of 60 would mean your kidney is functioning at 50% of a young adult. And remember, it is natural for your kidney function to decline as we age.
    • While it is not possible with current medical science to repair the damage to your kidneys, it is possible to improve your GFR in many cases. Remember, GFR measures how well your kidneys filter, not the amount of damage done to them.
    • Since the kidney damage is always present, it is possible that you can improve your GFR through diet and lifestyle changes, then quickly lose those gains in your GFR through diet choices and other lifestyle factors.
    • Our goal is to remove stress, inflammation, and the workload placed on our kidneys so that we can improve our GFR as best as possible for as long as possible. Find that goldilocks zone of diet and lifestyle choices to maximize our kidney's ability to filter waste and toxins from our blood. For most people, the right diet and lifestyle can also minimize or eliminate symptoms when a GFR is between 20 and 30.
    • Some prescription medications, kidney infections, urinary tract obstructions, abnormal muscle breakdown and eating a large amount of meat can elevate creatinine levels in your blood, temporarily lowering your GFR.
    • Tips to improve your GFR include eating a low inflammation kidney-friendly diet, staying properly hydrated, getting regular exercise, avoiding supplements with creatine, taking a probiotic like Renadyl ( www.dadvicetv.... ), and avoiding the use of Nonsteroidal anti-inflammatory drugs like Ibuprofen. Also managing the underlying cause of your kidney damage to prevent further damage - such as managing your blood sugar and blood pressure (take all prescriptions as prescribed by your doctor). These are areas your doctor and a dietitian can really help. They may also recommend a renal multivitamin like ProRenal+D ( www.dadvicetv.... ).
    • Last, don’t focus too much on your GFR. While it is an important number, it really doesn’t tell you much and is only one of several indicators of your health. You need to focus on your overall health and your labs. You can’t easily tell someone what to do with a GFR of 59 or 49. But you know what to do if you are suffering from anemia.

      I too was nervous. Mine was 51, I have a young doctor who’s said I may have been a little hydrated not to be alarmed. So I did another blood test 3 months later making sure to drink plenty of water and it was 55 again he said not to worry. I’m going to see another doctor. I do have high blood pressure that I am managing with BP medication.

    Thank you James for more info on the gfr. I have a better understanding of my kidney disease as I was concentrating on the GFR. I understand it is the stuff that remains in a person's system after the kidneys does the filtering. I am eating so much healthier now and I am feeling much better. I will find out what all my results will be in a few weeks. Thanks again and yes I fall off my diet every once in awhile but not where it causes more damage.

    avoided red meat and exercise for two days before blood draw - and cut my diuretic dose in half for a week egfr went from 54 to 85 in the 10 days between blood draws.....

      Same...7 days before blood draw...I cut diuretic in half, avoided red meat, avoided sweets completely....egfr went from 40 to 58...had also previously switched to a loop diuretic (no more triamterene or spironolactone)

    will a kidney stone 4mm lower the egfr number? It may have JUST passed right before i took the test. Still felt inflamed and irritated.

    I’m 39, gfr in 60s. Creatinine is 1.4, all other tests like BUN ratios are normal. I do have high cholesterol, could that be affecting my gfr? Currently on Lipitor

      Mildly high Cholesterol or Cholesterol meds don't effect kidneys much

    my skin feels tight and dry, scratches from itching don’t heal well they leave scars, my gfr is 81, i’m 25, creatine was elevated above normal amount, creatinine kinase was 438… folate was too low too, pls help i need advice, doctors don’t seem to care about low gfr and symptoms

    My creatine is elevated, not very much but still, my eGFR is 52 but my cystine-C is very good. I dont know what to think about this. Im athletic and traning almost every day, I go real hard in gym. Been lifting over 30 years. Half of my other kidney is removed when I was 1 years old, something wrong when I was born and it hurt to pee or something and my mother to me to hospital.
    I dont what to think of this. I use creatine suplement. I took about 5 month pause from creatine supplement but after that my creatinine actually got bit up. I took cyctine-C and it was pefect. Now I got this really bad eGFR and am really confused.
    I have zero symptoms and am athletic especially in every exploding fast power sports (which I guess is since genetics of lot of Type 2 muscle cells + a lot of gym action).
    Any opinions?

    My GFR went from 90(2017) to 60(2021). I am 32. My doctor told me it being at 60 is nothing to be concerned about....I feel that that is totally incorrect. :/ should i be concerned?

      Keep an eye on protein in your urine - if you have any talk to your doctor on how to correct that (it's done with certain medications that are affordable). Stay active and live a heart-healthy life. If your eGFR stays steady for another 5 years and you have no protein in your urine, your kidney function will be considered normal for your age, so don't let it worry you too much. Your kidney function is high enough for your age, and your family doctor should be able to help you manage it - just keep an eye out for protein in your urine; that is a serious indicator of future problems.

    I am suffering from low back and knee pains bcz of arthritis and muscles stiffness . I know anti inflammatory medications are not good for kidney. But homeopathy medicines was helping , my family doctor says no scientific evidence, advised not take it . I stopped taking homeopathy after that . But I was almost cure after long time, unfortunately I couldn’t finished my homeopathy course. Took only 2, weeks.
    Do have any idea , please share your thoughts opinions .

      Check out Fruity Lou youtube channel. Search for testimonials. There is a man on there that did a water fast (can’t remember how many days) then he went on all fruit diet and reversed (cures) his CKD. Also check out Dr Robert Morse TH-cam channel. He has a wealth of information on kidney filtering or lack of.

    My GFR is 10 and has been for last 6mo and i have felt great and i been eating salads but my dr is telling me that my blood is sticky and acidy and i should be sick but i am sorry i am still peeing like a champ and i feel great and my dr told me if i am not sick he wont put me in daylises until i get sick well i dont know from oct to dec i lost 42 pounds i feel awesome but all blood work sucked lol but idk what to do but any way im trying

    You’re doing really good, it’s great to see renal patients talking about kidney disease because they’re always more passionate and positive than renal doctors/ specialists.
    I’ve had stage 5 for 32 years, spent 18 years in total on dialysis & have received 2 cadaver transplants. So I have quite a lot of knowledge & experience on the topic- love your comment about how a person is feeling as a ‘whole’, it’s so true. I’m in Australia and despite the science being forward thinking the specialists are always so negative. It’s really hard to find a good renal doctor. Luckily I keep on top of all my own blood results, because they don’t, they can’t even find them on the system most the time 🤦‍♀️

      My dad is 73 is GFR is 24 .. this all started when he got heart stent.. the creat was 1.6 a day before stent was placed, and after stent operation he had pain in his arm so the dr have two strong pain killers back to back in 30 minutes of time.. which caused damage to his kidneys.. Acute kidney damage.. we came to know on March 2024 that is creat is 3.5 .. so we worked on it and in May and June 2024 it remained at 2.5 ( present ) . So his present creat is 2.5 and gfr is 24 .. Is it possible to increase gfr .. if yes can you please tell what diet he can take ? What fruits he can eat, also can he drink coconut water ? Also one egg per day ?? Also how much normal water per day ?? I shall be thankful to you if you can help us.. or can we talk over call ?

    UPDATE: So…I’m 38, now and my egfr has been the same for 4 years now: 89-90. I do work out a lot, IT fast, increasing more fiber and drinking water (not overload), clean urine and my creatinine was at 1.09 while the UREA BUN is at 19. Also, I’m a little anemic but not terrible, was high on b12 and vitamin d while having high cholesterol but not fatal (eating a lot of eggs).
    I plan to get tested, again. Any advice?

      George Brill Consider yourself lucky to catch it earlier than most - this gives your more time to make diet and lifestyle changes and greatly improve your chance of slowing down, stopping, or even reversing some of your lost kidney function. Make sure and find the underlying cause of your kidney damage and work with your health care team to manage that and prevent further damage. Managing your blood sugar and blood pressure is also very important - those are the two most common causes of continued kidney damage. Stay positive - you can do this! It may seem like a lot of changes at first, but living a kidney friendly lifestyle will help you to stay healthy. When I was first admitted into the ER my GFR was 8 - and I had NO CLUE I had kidney disease. The doctors got me up to GFR 13 and stable - then from there it has been all diet and lifestyle changes that gave me back my life, energy, and my future. I am just one example - many others are managing their kidney disease with diet and also seeing amazing results. Best of luck to you - and we are all here to support each other and help!

    I am 25 my GFR its 76 my doctor says it's normal but according to my knowledge i know that i should get ready and fix my diet and start to eating veggies only

      Hi don’t trust you Doctor, they told me that years ago and 2 years ago they try to send me to dialysis because my GFR was 18 yesss dropped off and 5 years from the first time they told me so I changed my diet I do exercise I eat well sleep well and now is 23 GFR but I’m feeling better much better I just listen to my body ,, so focus and you health reading here and there look for a lot good information like this channel ❤❤

    My urine tests are negative
    My protein test shows trace g/L but reference range is negative

      Don't eat protein rich diet .....eat vegetables .... become a vegan....avoid milk products.....pickels,lemons,orange, all citric acid fruit, avoid non-veg, drink atleast 2 n half lt water daily.....check BP daily basis....walk fast every day half n hour , do yoga daily.....I too have GFR 63 % ....too scared .....Doctor prescribed few medicine and above restrictions

    HI James, I just had two stints placed in heart area and the contrast took my kidneys down again. Back on dialysis. Have you done a video about the danger of the contrast in operations. If not it would be worth doing. I knew the danger going in. But fixing the heart was worth it.

      yes they use high doses of iodine in the contrast to see these physical state of the kidneys *** for people with frail kidneys the test is often contraindicated their are many minerals that are large and difficult for our kidneys to excrete. Best to avoid them if possible. always remember that u have the right to refuse. But it is important to have a sound understanding in making these decisions via discussing them with professionals healthcare specialist.

    people , i might add in Australia, some blood test laboratories DON'T take into account the persons muscle mass in their creatinine/albumin ratios etc...-
    So, nephrologist use a formula to work out your filtering function using the labs eGFR/ creatinine numbers (different labs use different measuring scales), so be aware of such details people...

      @DADVICETV I am also in the process of changing my medical team. My primary care doctor and nephrologist appeared to be grooming me for dyalisis. Neither of them were willing to discuss the reason for my high parathyroid numbers. I found a doc that took time with me and explained things.

    James, you are so positive and your information is great. In WV, and southern OH the 2 major dialysis centers will not let you have a renal dietitian until you are on dialysis. The nephrologists will not send you to one either, just any regular one who doesn't understand they have to tell you what you can eat. I moved across the state after having 5 nephrologists. I've been a 3b for 10 years and I have been to #6 one time. Boy, I hope he is helpful, the others -not so much, especially the University Professor, #3, who would have made me have a vein mapping years ago. I can see hope, thanks to you. My age is 62.

    In January this year, creatinine level was 1.12.
    then a eye angiography was conducted which basically was adding colour in the blood.
    Afte 3 months of that creatinine level was 1.80 and now in August2023 CL-1.79
    And eGFR 33ml/min
    Along with diabetes, hypertension, hypothyroidism and heart disease.
    Now what should be done. Please help
    I'm feeling so helpless , please

      There was a urine infection a few months ago. And doctor said protein discharge with urine has identified.
      new addition: anemia ( hemoglobin levels dropped to 9)
      It was the evaluation of last September 23.

    This is useful; my GFR plummeted from 100 to 81 in four months. I've been obsessed with change, even though my doc says I'm fine. I'm 56 and have diabetes and hypertension. I've been working on my weight and completed a 5 day fast less than a week ago.

      I am Mr. Positive - but any drop in my GFR has me concerned. A 19 point drop (nearly 20%) is something I would want to look into the cause of. Your GFR will fluctuate but for most people it is within a 2-10 point range. I am a huge fan of intermittent fasting - but be careful of fasting and when you get your labs done. During a fast your GFR will artificially be lower due to how your body reacts to fasting. Also stay properly hydrated - being dehydrated can cause an eGFR to be up to 20% off. See if your doctor can do another kidney function blood test to verify this wasn't a temporary drop - you can have your family doctor draw blood and send it to a lab like Quest, you don't need a specialist. Good luck!

    I stopped taking statins after reading they can harm kidneys, I had protein in my urine, after stopping there was no protein and my egfr rose to 104, I'm 52 and type 2 diabetic (controlled, no meds)

      Like he said it can jump up or down depending on various factors. I wouldn't worry about GFR unless it dips below 15.

    I have recently had a full blood count test, my GFR came back at 58, my GP requested another full count blood test and it came back at 54 3 weeks later. My GFR in the past was always 60 she said. I do have high cholesterol and was prescribed statins for it but for the last 3 months I havent been taking them and she said that may be why my GFR has come down so I'm back on them for the time being. Do you think my GFR will go back up when my Cholesterol comes back down? I had got my Cholesterol down to 5.5mmol and it is now 8mmol. Im also taking high dose vitamin D3 and K2 as this is good for decalsifying the arteries. Thank you.

      My friend was taking vitamin 3 with k2, it had sunflower oil in it. Make sure there's no sunflower oil in the one you use.

    All of another test is normal
    Even i do kidney ultrasonographic...the size of my kidney is normal..no kidney stone and no infection...
    Blood sugar is normal
    And blood pressure is normal...
    No other symptomps
    Only my creatinin and GFR...

I checked labs from last year and saw one reading in December of 2021 it was 87 then in July of 2022 last year it was 101. Since it was 80 today being a 36 year old male can this be alarming?

      7. Apples
      8. Berries
      9. Extra Virgin Oil
      10. Melons

    thanks Coach James! my GFR as of 2 weeks ago is 57. thanks for giving me some guidelines. helps me put it all together and motivates me to do more.. I also find out why my thyroid function has deteriorated.. Bottom line was I am taking too high a dose for the normal changes in my body. It is good to monitor the own blood test and seek advise and understand the changes and modifications needed. they of course was discussed with my Nurse Practitioner last night. She really cares and I admire her dedication.

    My Nephrologist thinks I may not have CKD and is sending me for tests. despite having GFR at 52 and last reading at 45.
    I am very athletic for my age and at the fitness level of someone in their early 40's and have a lot of muscle mass due to calisthenics and athletic activities. I am 57 years of age. I am hoping the test come back showing that he is right.
    Here is hoping. I now feel confident to sign up for my usual big bike ride. 360 k.m. 2 day event.
    Thanks be to God I was able to get an appointment with this Nephrologist, my g.p. was hesitant to send me saying we do not normally send anyone to a Nephrologist until they hit 30 GFR. but thanks James I have become my own advocate with a little research and some help from your videos.

      24 hour urine test showed I have 2.5 times the average male my age. He said I do not have Kidney disease. And am closer to 100% I have a lot of muscle mass and workout a lot e.g. I am at 1155 k.m of bike riding. Phew.

      Managing your blood pressure is critical and your doctor may need to try different dosages or medications to get you to 120/80 or better. Make sure you are eating a heart healthy diet and watching added sodium. Good luck!

      My blood pressure is much easier to control, but I am still on BP meds. There was a time I just took a couple pills a day, now my BP meds require 9 pills a day. I need to loose weight (I was doing really good, then wasn't as careful with my carbs and exercise this year and have gained some weight). Once I drop some weight I should be able to get onto less BP meds and still keep it under control - that is one of my future goals.

      Hi Chaz , first you need to reverse your diabetes . Do intermittent fasting , only eat 1 or 2 meals per day . Strictly low carb high fat diet . To be more precise , subscribe beat diabetes channel n practice what Danny taught . Hope it helps .

      I agree on the fasting. I fast daily at least 16-18 hours with 2 meals low carb. Add in some moderate exercise
      and you can reverse type 2 diabetes. I did.

    What should i do i am worried

      First, don't panic. Now is the time to make sure your blood sugar and blood pressure is in check. Also, talk to your doctor about hydration to make sure you are not slightly dehydrated (very common). This is a great time to kick any bad habits, like smoking or drinking. If you had no protein or blood in your urine, good chance your kidneys are ok and you can address any issues with diet or lifestyle changes. What you shouldn't do is jump on the fake supplement cure bandwagon or make any drastic unhealthy diet changes.
