  • Do you have questions about the effect of testosterone on psa or your prostate

    so this video is going to answer the question does testosterone affect psa and this comes from a specific question i got from a viewer they were asking do high levels of testosterone from testosterone cpn affect your psa levels so the answer to this question is yes testosterone almost always raises psa given enough time testosterone increases
    the body's production of two important hormones that have an effect on prostate tissue and those are estradiol and dihydrotestosterone also known as dht both of these hormones can cause the prostate cells to individual cells to grow and then divide and turn into new prostate cells since psa is a protein psa stands for prostate specific antigen an antigen is a protein since it is a protein and the amount of that protein is a reflection of the
    amount of prostate tissue available or present it only makes sense that more prostate tissue means higher psa we also know that with age the psa and prostate size generally does increase and in part that's thought to be a reflection of the prostate tissues
    exposure to these hormones testosterone dht estradiol while there are some small studies showing that tsa does not rise upon administering testosterone there are plenty of studies that are more well designed that do show psa going up upon initiating testosterone therapy you do have to give it enough time because it's not going to happen after one shot or one month of application in some cases it can but it's over time those hormones have an effect on the prostate tissue also if you're applying testosterone
    topically and it's not actually getting into your body absorbing which is a
    common problem with topical then you may not have any effect because it's not
    actually changing those hormone levels in my practice i commonly do see
    increased tsa levels when people start testosterone replacement therapy there's
    usually an initial spike after a few months of being on it and then it kind of stabilizes and then you sort of fall into the normal trajectory for age it might might rise over a couple two three years again after that so it's typically going to stabilize after the initial rise in psa when you start the testosterone replacement therapy then you know there's a whole question about does psa even matter what is the value of doing the test that's not really what this video is about but if you do have questions about things like that or other questions about testosterone in psa rising.

    I am 70 i have been on testosterone replacement therapy for 9 years taking 2 ml testosterone shots a month during this time. My psa test at the beginning my therapy was 3.5. 9 years later i applied for insurance and had to take a medical evaluation . This included a psa test. My psa was 7.0. I immediately stopped my testosterone therapy and waited a month tested again and my psa went down to 6.0 my testosterone level went to 207. I waited 5 more months with no testosterone and tested again. My psa level was 2.7 and my testosterone level was 217. My suggestion for someone who has been on testosterone therapy for a long time is get off for a few months and retest. I went from 7.0 to 2.7 all i did was alow the effects of the testosterone to clear my system.

      Doesnt mean is cancer and psa is not specific to prostate cancer all it means is you bph and trt since increases it, you stopped and dht reduced, it shrank

    On TRT for 6 months..blew my psa up from 3.2 to 6.8..got off immediately went back down to 3.2...but unfortunate the TRT probably sent off prostate cancer...I start radiation next week....As my doc warned me..TRT will not cause cancer...but if it is lurking somewhere..it's like pouring gasoline on the fire.

      It's been shown by other urologists and specialists in recent years that it's actually estrogen and to some extent DHT that "pours gasoline to the prostate cancer fire" rather than testosterone. This makes sense to me as young men with naturally higher testosterone levels and low estrogen/DHT levels never get prostate cancer , but on the other hand older men with naturally declining testosterone levels combined with rising estrogen/DHT levels are more proned to get prostate cancer. Of course genetic , environmental and DNA damage come into as well. Why would a hormone that gives you strength , muscle growth, energy, confidence and high libido when you're young turn around and give you prostate cancer when you have a lot less of it ( testosterone!) when you're older???

    Hello doctor,
    I have been on carefully monitored testosterone therapy for about 3 years. For the first 2 years I think my PSA was relatively stable. This last year however my PSA had a double velocity going from 2.4 to 4.8!! My endocrinologist advised I drop my dose from 2 squirts to 1 squirt a day (not very technical I know!!😂) and to also see a urologist.
    In the meantime after about 6 weeks of seeing my endocrinologist I did a new PSA test before seeing the urologist and it came back as 2.1? So it’s more than halved? As an educated guess do you think there is a correlation in your opinion?
    I have to see the urologist next week for a DRE. No conversation of an MRI or biopsy as yet.

      Well it is common for PSA to spike for various reasons so sometimes it is better to just recheck if u get a spike. U will have to check it again later to know if it was the testosterone

    I had a question...what if you have a high PSA level to start with due to BPH? Would you still be able to do a trial of the hormone replacement therapy or does that make matters worse?

    Hey Doc, any info on whether or not testosterone supplements such as Tonkat ali can elevate PSA levels?

      I was taking tongkat ali and gaining muscles and better in the bed but my PSA was 6.1 recently 😮

    Very useful thank you. I am micro dosing 12mg of cyp daily, 2 months ago my PSA was a healthy 1.0, yesterday it came back as 9.0! A 900% increase!! I’m not doing anything different and hoping this is an error! I’d be grateful to get your view of this? Thank you.

      That must be an error I have been using 300mg cyp every 2 weeks for years my PSA is 2
      If your micro dosing why along acting cyp you should be using propionate

    Doctor. I would appreciate if you could offer any opinion in regards to my situation., I am a 51 year old male that went to the doctor's office in May '22. I had a workup of blood and specific to PSA....
    I had a PSA, Total of 3.0 ng/mL
    I had a PSA, Free of .05 ng/mL
    I also had a consult with a urologist and a rectal exam... all checked out fine. No concerns.
    I was then placed on TRT (Cypionate). I started this with 0.25 mL 3 X's a week. (200 mg/mL)... Starting 8/3/22.
    I just went back to have a 6-Week Blood Work Done.... and my PSAs have gone crazy. I'm just looking
    over the labs.
    I now am experiencing..... a PSA, Total of 4.9 ng/mL and PSA, Free of .09 ng/mL.
    Any opinion? Other than this big spike in PSA.... the TRT has been a good experience as I feel
    more healthy, more alert, more focus, more energy and more motivated. I'm not experiencing any of the hallmarks of prostate problems.

      I can't give individual advice on something like this. sounds like a very high dose of testosterone if you wrote it correctly.

      @@swintegrative Good Morning Doctor. That is my bad, I am taking .25 mL three times as week.... not 2.5 mL. I hope that helps. Is there a way to arrange a consultation with you? My concern is that although I've been checked by a urologist.... all appeared fine in July. My PSA has spiked 1.9 ng/mL
      higher in less than 4 months. I don't know what is going on here and I'd love an opinion....

    I'm 32
    In may my total T was 300ng/dL
    Psa 1.0
    Started cream for one month, then went to injectable for 2wks
    Today my Total now is

      It's typical to see PSA go up when on TRT and may even go down without stopping. It is a test with lots of variation.

    Hey Doc I have a question. So I have been on testosterone cypionate for 4-5 years now. I have foloowed my PSA for years and it has definitely increased to a touch over 4.3 over the course of 4 years. This would not alarm me, however my Free PSA is 9%. My question is this. Have you ever seen exogenous testosterone replacement increase the PSA while the free PSA remaining below 10%? I have found no research to support or refute this. I have even looked through Up-to-Date and have seen nothing. I only see that if < 10% there is 56% probability of prostate cancer. That leaves 44% that is non cancer related and I cannot find any research that points to what the possible 44% is. Thanks a bunch. Oh, and I am scheduled for a biopsy tomorrow. So I am ruling out cancer, I just wanted to pick your brain at this possible other causes of lowered free PSA. Thanks again, and thanks for the videos.

      and have a very low free PSA of 6. I'm scheduled for a biopsy on Feb 14th. A bit worried here.

      @@luketimber4679 I ended up having 3 cores positive. 10%, 10% and 20%. Gleason 6. I go for an MRI and having gene testing. If everything looks good I plan on doing active surveillance. It isn't a death sentence and if you have been keeping an eye on things and have caught it early, it is very curable. I quit the TRT and feel great. I will say a prayer for you. I understand what you are feeling. Just know that it is very slow growing and if still organ contained it is very curable with multiple modalities.

      The lower the PSA the less accurate the % free psa will be.

    Hello doctor, I am 49yo and have been receiving TRT for over 2yrs now. These are my labs as far as testosterone levels
    Start 2yrs ago: test 245 psa .08
    Year ago: test 935 psa 1.1
    6 months ago: test >1500 psa 1.5
    2 days ago: test 1475 psa 1.5
    It seems the trend is a direct correlation with increase testosterone it has increased my PSA over time as you stated I the vid. It seems to have stabilized over the last six months. Do you have any suggestions for me going forward?

    My Primary Care Physician reported my PSA test result was "479"... aren't PSA #s reported using decimals? Please explain this 3-digit number. Thanks.

    Trama to the boys has left me with freet below range but total test is 500..my lh is top of range and have been told by 2 hrt clinics its testicular failure since lh is maxed out..but hav a high psa of 5.5 so they sent me to a urologist who told me nothing is wrong with me lololololol

      The trauma could have raised the psa too depending on how severe and how close to the psa test. Total 500 is not bad. Maybe you should focus on raising free testosterone.

    Need your advice brother. I'm 35 years old lifelong Natty. I normally transform my body way faster than the average person with proper food and decent sleep training hard of course. However when I stop training and eat like trash I also get out of shape just as fast. So i can work out for 12-20 weeks 5 days minimum a week and literally look like I been training for year or years. I been working overnights for the last year and not sleeping well I been training again from taking 5 months off got out of shape and now am some months into training now but this year has been weird I'm not transforming as fast. I just had my hormone panel blood test done for the first time in life would like your input on it. How do I reach out to you???
