Thank you for the explanation of the c-peptide test. Even my family doctor admits that he has never prescribe this test because he does not understand what it is for. The subject came up because I was attending a metabolic clinic and the staff there asked my family doctor for a requisition for a number of tests every 3 months. He complied while my sessions at the clinic continued but when they stopped he did not want to keep prescribing the c-peptide and fasting insulin tests. He was worried that the insurance people would flag him for prescribing too many unnecessary tests. Now I am in another metabolic program but this one includes medical doctors who can prescribe lab tests. My present metabolic doctor is suggesting that I am headed towards lada, not type 2. At this point my numbers are still at the pre-diabetic stage. Even she did not explain the relationship between c-peptide and blood sugar as well as you did. Thanks again.
If you're still insulin resistant, you need to eat whole foods only (wide variety of veggies, meats, greens, nuts, SOME fruits)... And STRENGTH TRAIN 3-5 days a week, gradually increasing diet & exercise but not way too much all at once. Get good sleep 7-9 hours a night, low stress, enjoy the sun safely 😊
The pancreatic beta cells secret C-peptide and insulin in equal amounts after splitting proinsulin to form these 2 components. They enter the blood and head to the liver. The half life of insulin is about 3 minutes and that of C-peptide about 5. When blood glucose stays high as in T2D with bad insulin resistance the beta cells will be overwhelmed and proinsulin will be secreted along with much reduced amounts of insulin and C-peptide. The endocrinologist will conclude that your pancreas is dead and order insulin for injection!!! They are not research scientists and they follow the book/protocol to prescribe insulin. They have too many things to be updated but have no time. You can say an old doctor is very experienced but MAY be outdated.

My A1C is 4.8, fasting glucose is 72, but my endocrinologist won't check my fasting insulin levels. Because one can have normal fasting glucose levels and a normal A1C levels, but a real elevated fasting insulin levels...which means insulin resistance and a predictor of diabetes

Great bro. So how do we decide if the patient with 150 glucose and 0.8 c peptide is type 1 or type 2? And more broadly what're the criteria of differentiation between type 1 and type 2 diabetes? Because as you said high bg suppress beta cell function and they fail to produce insulin temporarily showing low c peptide despite high bg. I just did my lab work. My A1c was 5.3. My fasting glucose was 70. And my c-peptide was .61......and flagged on the report as 'low'. However I've seen several websites that describe normal range as .5 to 2.0.

Three months later test reading were - HbA1c-5.8, c-peptide-0.75, fasting insulin-2.3, fasting sugar-118 In subsequent test c-peptide kept going up from 0.75 to 2.0 with fasting insulin 4 and fasting sugar 100, HbA1c 5.4 My fasting sugar rarely come below 100 can this diabetes be reversed. What should I do? My glucose was always fine (did do regular tests) It was still fine in may and in august it was first elevated, in september I was diagnosed with diabetes. Because of my wheelchairlife and a few pounds overweight I was diagnosed with diabetes type 2. Metformin doesn't seem to do much unfortunately. They did test for 2 antibodies, my local hospital doesn't do any other antibody tests and advice me to take the c-peptide test. I do have an underlying condition that goes hand in hand with auto-immune disorders. Also I have another condition that causes unbalanced cortisol levels. I would like to know if the high glucose (but I also have low levels) is caused by diabetes type 1 or 2, or by my dysautonomic dysfunction or cortisol levels. I would like to have the other antibodies tested, but I don't have a referral for this. Would c-peptide at this early stage, and with cortisol problems, also be accurate?

Hi Doc Eric! I am at panic mode right now. I got my C-Peptide test. I am not diabetic. Sometimes I'm experiencing low blood sugar that's why my Endocrinologist have this OGTT FBS and OGTT 2Hrs qnd C-Peptide. C-Peptide Reference Interval is at 1.10 - 4.40 ng/ml and my result was 4.51 ng/ml. OGTT Non-Pregnant 75gms FBS 70.10 - 98.93 mg/dl and my result was 89.20 mg/dl OGTT 2hrs < 140.56 mg/dl and my result was 195.16 mg/dl HbA1C < 5.70 % and my result was 5.60% My question is will I patter the C-Peptide result to the OGTT 2hrs after loading glucose 75gms or the OGTT FBS? because the C-Peptide Reference Interval is from 1.10 - 4.40 ng/ml. I am very oqnic at this moment because before rhebtest, Doctor told me if my C-Peptide is high, there's a problem with my pancreas. I am afraid. Some here in google and TH-cam, I saw that the normal values should be 0.5 - 2.0 ng/ml and it's too way far from the reference interval here. Please help me doctor.
you are insulin resistance with those numbers. You are barely able to keep down your glucose levels. Your fasting C-peptide is also very high. Your 2 hour OGTT test of 195 is super high, mine was 101 at 2 hours. My C-peptide is 1.03, your value of 4.51 means you are producing way too much insulin. You need to cut back on carbs/sugars.
My Fasting Glucose is 67 in lap , but 97 at home on personal glucose meter. And my Hba1 c shows GF 77.all in same day. My C- Peptide Fasting is 1.92 is it okay And Homa IR is 1.47

you should always get an insulin or C-peptide test to make sure you are not producing a bunch of insulin to keep your A1C and glucose levels low. optimally a 3 hour glucose & insulin test to see just where your numbers go after drinking sugar.

Dang, what about a fasting glucose of 104 and a c-peptide of 1. What does this mean It was weird because I got a Fructosamine test and it was 254 which is a A1c around 5.9 But my A1c is usually around 5.4 -5.5
Fasting glucose of 104 & c-peptide of 1 could mean different things. Best way would be to get in touch with your health provider. They would be able to provide best advice based on your medical history.
When I went to the Diabetic Specalist my sugar was 77 & sensor showed 77 C-Peptide showed 1.1 she said your Thyroid is ok. My Kidney is ok & my liver is ok but my insulin production from the pancreas is Measurable but low. My TSH is 2.790 My T4-FREE is 2.17 What got me she talking about insulin pump therapy? I am like are you serious? She just started me on a Ozempic. Im diabetic as well. Truth is we don’t really need doctors if we have classic T2 diabetes. You mentioned you have 30lb to lose… which means you are still in the process of reversing diabetes (still a way to go). I do agree with you that you lose the weight first… then see how things improve. Maybe you can share your age and how long you have been actively reversing this?

I just did my lab work. My A1c was 5.3. My fasting glucose was 70. And my c-peptide was .61......and flagged on the report as 'low'. However I've seen several websites that describe normal range as .5 to 2.0.

Thank you so much for informative video. When I was diagnosed with diabetes I lost the weight with HbA1c 12 and fasting sugar 225. Based on doctors advice started on low carb diet.
Three months later test reading were - HbA1c-5.8, c-peptide-0.75, fasting insulin-2.3, fasting sugar-118
In subsequent test c-peptide kept going up from 0.75 to 2.0 with fasting insulin 4 and fasting sugar 100, HbA1c 5.4
My fasting sugar rarely come below 100
can this diabetes be reversed. Is my beta cell intact. Am I still insulin resistant

What should I do?
My glucose was always fine (did do regular tests) It was still fine in may and in august it was first elevated, in september I was diagnosed with diabetes. Because of my wheelchairlife and a few pounds overweight I was diagnosed with diabetes type 2. Metformin doesn't seem to do much unfortunately. They did test for 2 antibodies, my local hospital doesn't do any other antibody tests and advice me to take the c-peptide test. I do have an underlying condition that goes hand in hand with auto-immune disorders. Also I have another condition that causes unbalanced cortisol levels. I would like to know if the high glucose (but I also have low levels) is caused by diabetes type 1 or 2, or by my dysautonomic dysfunction or cortisol levels. I would like to have the other antibodies tested, but I don't have a referral for this. Would c-peptide at this early stage, and with cortisol problems, also be accurate?
Is fasting blood Suagr 95 and c-peptide 1.90 considered normal ?

I am at panic mode right now. I got my C-Peptide test. I am not diabetic. Sometimes I'm experiencing low blood sugar that's why my Endocrinologist have this OGTT FBS and OGTT 2Hrs qnd C-Peptide.
C-Peptide Reference Interval is at 1.10 - 4.40 ng/ml and my result was 4.51 ng/ml.
OGTT Non-Pregnant 75gms FBS 70.10 - 98.93 mg/dl and my result was 89.20 mg/dl
OGTT 2hrs < 140.56 mg/dl and my result was 195.16 mg/dl
HbA1C < 5.70 % and my result was 5.60%
My question is will I patter the C-Peptide result to the OGTT 2hrs after loading glucose 75gms or the OGTT FBS? because the C-Peptide Reference Interval is from 1.10 - 4.40 ng/ml. I am very oqnic at this moment because before rhebtest, Doctor told me if my C-Peptide is high, there's a problem with my pancreas. I am afraid. Some here in google and TH-cam, I saw that the normal values should be 0.5 - 2.0 ng/ml and it's too way far from the reference interval here. Please help me doctor.
you are insulin resistance with those numbers. You are barely able to keep down your glucose levels. Your fasting C-peptide is also very high. Your 2 hour OGTT test of 195 is super high, mine was 101 at 2 hours. My C-peptide is 1.03, your value of 4.51 means you are producing way too much insulin. You need to cut back on carbs/sugars.
My Fasting Glucose is 67 in lap , but 97 at home on personal glucose meter.
And my Hba1 c shows GF 77.all in same day.
My C- Peptide Fasting is 1.92 is it okay

you should always get an insulin or C-peptide test to make sure you are not producing a bunch of insulin to keep your A1C and glucose levels low. optimally a 3 hour glucose & insulin test to see just where your numbers go after drinking sugar.

It was weird because I got a Fructosamine test and it was 254 which is a A1c around 5.9
But my A1c is usually around 5.4 -5.5
Fasting glucose of 104 & c-peptide of 1 could mean different things. Best way would be to get in touch with your health provider. They would be able to provide best advice based on your medical history.

When I went to the Diabetic Specalist my sugar was 77 & sensor showed 77 C-Peptide showed 1.1 she said your Thyroid is ok. My Kidney is ok & my liver is ok but my insulin production from the pancreas is Measurable but low. My TSH is 2.790 My T4-FREE is 2.17 What got me she talking about insulin pump therapy? I am like are you serious? She just started me on a Ozempic. Im diabetic as well. Truth is we don’t really need doctors if we have classic T2 diabetes. You mentioned you have 30lb to lose… which means you are still in the process of reversing diabetes (still a way to go). I do agree with you that you lose the weight first… then see how things improve. Maybe you can share your age and how long you have been actively reversing this?
