  • Trevor Siemian suffered a rare "pure" dislocation ankle injury during week 2 of the NFL season during Monday Night Football against the Cleveland Browns. He somehow didn't have a fracture which only happens in less than 1% of ankle dislocations. Also he managed to walk off the field! In this video i'll review the footage of the injury to explain the mechanism and what is going on in the ankle joint to cause this injury pattern.

    This was interesting to watch as I had a similar injury about 8 weeks ago and was never really told what had happened to my ankle. Hope Siemian gets better soon.

    I watched that play, at first when it happened it didn't seem that bad but after slow mo you could see the defender and Trevor Simiens weight landed all on that ankle as it rolled.

    Good video: one comment - these ankle injuries are not really eversion injuries but actually have rotatory mechanisms and Siemian based on what I can tell this video had a supination external rotation injury, which is the same mechanism and similar type of injury that Gordon Hayward had a while back (except he fractured). The injury begins with an injury to the ATFL and the force propagates rotationally then next to the fibula, then the posterior malleolus and then medial malleolus/deltoid ligament. The fibula does not really break because the talus is moving laterally into it but rather because it is torqued/rotated during the injury event, which is why the fibula fracture is typically oblique (a direct lateral hit from the talus would result in a transverse fracture). The talus in this instance is not actually moving laterally in relation to the tibia but rather, it is the tibia which moves medially away from the talus.
    If he did indeed dislocate and spontaneously reduce his ankle, his deltoid ligament would have to be torn and treatment would certainly involve surgery, which with a deltoid injury would not involve ligament reconstruction but rather fixation across the syndesmosis (either screws or Arthrex tightrope) to hold his tibia and prevent it from subluxing medially while the deltoid scars down. Or if he was a normal person...you could theoretically treat it with strict non weight bearing in a cast for 8 weeks if the reduction can be maintained in a cast but ain’t no NFL player gonna wait for that.
    My two cents from an orthopedist’s perspective...

      At 5:15, how is that not eversion? Appreciate the Ortho input as you all certainly deal with these more than I would. I can see the rotational component playing a role but just looking at the freeze frame that certainly looks like more eversion and pronation, not at all supination. Supination is present with inversion injuries right?

    I had this same injury years ago (ruptured deltoid ligament). The way my surgeon explained it was that you can't just create a synthetic replacement like an ACL since there aren't opposing bone masses to attach to. The best they could do was try to reattach the remaining ligament, stick me in a cast for 2 months and hope scar tissue glued it back together.
    Anyway it felt pretty good after 6 months, and good as new after about 9. Plus a sick scar

    For me, I tore my ATFL, and according to everyone around that I was playing basketball with, they said my ankle flexed to about a 110 degree angle, which is absolutely insane. That put the radiologists and docs into a deep set of shock, and they were lost as to how I could have possibly avoided fractures completely like I had. When I went in to a foot & ankle specialist, they wanted to get their own xrays on it too (even though it was a year later) just because they wanted to have their own verification that there was no signs of any previous fractures. And they found no signs of any fracture history. I got the MRI, and again, they looked to see any signs of previous possible fractures but found nothing but a heavily scarred up ligament. Even they could not figure out how my bones defied the forces completely.

    2. Is this injury any different to a syndesmosis injury? These come up fairly regularly in Australian football.

      Liam Briggs sydesmosis injuries are high ankle sprains, they are sprains or tears of any of the four ligaments that connect the tibia and fibula; they do not involve the talus. -orthopedist

    My right ankle has dislocated numerous times. The pain is breath taking plus I have to suck it up and put my ankle back into place knowing that pain is going to be so glorious that I’m going to get nauseous and maybe even see stars. Thankfully it doesn’t last long. My ankle isn’t repairable. My only option is to have my ankle permanently fused in a right angle position. Something I’m not ready to do. I hope this young QB gets excellent repair of his ankle. Having an unstable ankle would end his career.

    So weird question. About 5 inches above the inside ball of my ankle I get severe pain about once every couple months. When I put weight on the outside part of my foot it gets unbearable and it's difficult to walk. It lasts for about a week and goes away. It's been happening for about 5 years. I don't think it's a bone problem but I could be wrong. It does hurt when I put pressure on the spot but I got to dig my fingers in deep to hit the spot. You ever heard of reoccurring pain like this every couple months?

    I'm a HS QB coach and I teach them to keep their knees straight to avoid this type of injury in this circumstance. I'm also a Mathematics teacher and I educate them that their leg will break with the load and force applied to it. This is when the lineman hits them in the front and driving them to the ground on their backs. I guess Siemen missed that day of practice.

    I broke my ankle(foot trapped while falling down a stair case and turning around while it was trapped) anyway 2 surgeries later, I think I can conceptualize how Siemian was extremely lucky, with this freak sort of compression load and rebound without a major fracture...99 out of 100 times the Fibula snaps and reconstructive surgery is required, I don't think he realizes just how lucky he was, although I'm certain it was excruciatingly painful,lol!!

    I dislocated my ankle, bone broke through the skin, but I didn't break anything.
