    This Doctor Reacts video is me, as a psychiatrist, analyzing another episode of It's Always Sunny in Philadelphia. I'm watching PTSDee and it will come as no surprise that its all about trauma, post-traumatic stress disorder (PTSD) but also we discuss sleep deprivation, Freud's Oedipus complex, whether dream analysis is a thing, and an fascinating but rare disorder called familial fatal insomnia.

    I love when you do these. An episode that would make for great commentary would be "Flowers for Charlie" and you could dissect the playceebee effect.

    I know this is more about PTSD and sleep, but I'm also fascinated by what this ep adds to Dennis and Charlie's childhood SA backstories from The Nightman Cometh and Dee Made a Smut Film. There's a lot of thoughtful character work for such an irreverent show. Love to see more IASIP inspired videos!

    I was once woken up by a fire alarm while i was in a hotel on a school trip staying with 2 other girls in the room and i DEFINITELY has confusional arousal. I was convinced the alarm was something just coming from our room and was going to wake everyone up so i started unplugging the telly and everything else in the room that i thought was making the noise. The others were like what the hell are you doing, its a fire alarm.... then i sort of properly woke up and realised 😂 i do remember it all though. I was in such a mad dash.
    My partner also does this all the time where he "wakes up" from a noise he has heard or if i roll over. And he thinks i am falling out the bed and grabs me in a confused and panicked state (to stop me falling out... when i wasnt falling out). He rarely remembers it the next morning because he goes straight back to sleep. I think I only remember mine because i had to fully wake to go downstairs for the fire alarm.

    Another treatment is DBT for PTSD. Blends some of the beautiful and effective DBT skills (mindfulness, emotion regulation …) with CBT exposure.

    Thanks for teaching me the term sleep inertia! Maybe with an actual term under my belt, doctors will start taking me seriously when I tell them that when I wake up in the morning, I'm so tired that I literally cannot keep my eyes open. They haven't taken me seriously when I tell them I have Delayed Sleep Phase Disorder though, so who knows. I love all of your videos, especially the Always Sunny ones! Do you think maybe one day you can do a video about little-known side effects of ADHD, like DSPD and iron deficiency? Those are two things I struggled with for years, and it was only after I got my ADHD diagnosis that I learned that these conditions disproportionately affect people with ADHD. Apparently we're even more susceptible to seemingly completely unrelated conditions like PCOS (something I also have)!

      Do you live in Virginia? I used to and had similar problems with lousy doctors who don't listen and/or just give me the brush off.

      Yeah, delayed sleep phase disorder is v comorbid with ADHD, other things that can cause that include sleep apnea and narcolepsy.

    the world record of not sleeping won't be officially broken because the world record people refuse to put the people who try it in danger as it is very dangerous

    I've been in the situation where a then friend was trying to help me by drinking me underneath the table, because getting flat-out drunk was what she did whenever she felt sad. I reckon that some level of projection might have been at play there.

    Great to see another sunny video! An episode which i think would be very interesting to analyze is the last one that came out, dennis takes a mental health day. I think it gives some cool insight on what goes on on the inside for him

    Thank you for the video. I connected with the content. I suffered from sleep deprivation and craved sleep. I slept anywhere my body would let me. Now that my dog guards my bed, I can actually sleep. 4 years of being able to sleep like a baby. Whoop whoop!

    When I was getting my doctorate, I had a nightmare that I was taking my comprehensive exam and had forgotten to study for it. This dream wasn't very hard to interpret.

    It's Always Sunny is one of the most hilarious shows I've ever seen but they still manage to at least touch on serious subjects but still ultimately, not solve any of their issues and learn nothing or learn very little, if anything constructive/positive 🤣🤣 Also, Doctor Elliott, I was wondering what your thoughts are on people who do not ever have any dreams. The general consensus is/was that there was maybe a correlation between those who do not dream and it leading or being a symptom of becoming/being a serial killer or some sort of other drastic negative behavior. 🤔

    When I have my manic episodes I go 5 days without sleeping and then usually go 2 days sleeping then 5 days no sleep again... its generally how I know I'm manic

    The thing you didn't mention about Fatal Familial Insomnia is it's more or less confined to a single family in Italy, incredibly heritable to the extent that being born into that family is more or less a death sentence unfortunately :/ horrendous disease.

    Had to stop watching when you start describing fatal familial insomnia lol. I've been hospitalized for my insomnia before. Word to the wise - do not read about FFI when you are experiencing extreme insomnia.

      Yeah was going to say, I don't know if I have had insomnia but I have had periods of insomnia like feeling. I also have lots of health anxiety. When my friend told me about FFI I was convinced I had it for a bit(he was also a nurse and said he had saw at least one patient with it).

    What do you mean "sleep deprivation is used as a form of torture in parts of the world"?! It is right now commonplace in US prisons, not to mention the likes of Guantanamo Bay. I refuse to believe you are not aware of this.

    Amir give this to you and you analyze it to your best, factoring melee warfare those with PTSD we're taking out of the gene pool but seeing that they probably made it and had at least one child their genetic code which continued. Where lifespan and population numbers have changed you can put into an argument why same-sex relations were frowned upon in these days as your population would be wiped out like the Spartans
