Regarding fireballs - You CAN block all 4 if you time it properly. In fact I recommend it as this means you don't take the fireball damage but you also don't take the following fire DoT. I've been running this dungeon over and over. I rarely take damage from those fireballs as a result of timing. Regarding geysers - Below 50% HP he does it twice in a row. It's not random, it's not a bug. To your defense, you posted this 4 days ago which was shortly after the content released. Devs might've cleaned it up since then. Aside from those, solid guide.
Only under 50% the boss does the second puddle. The bug that I found is sometimes instead of landing the second fire and then going to the puddle phase, the boss ignores that and goes to the rat phase without anyone getting in the rat form and then he gets stunned and you can whale on it for a few seconds. Another bug, after a wipe, the boss still remains agrro towards a player that had dots on him and that player must move away from the gate a few to properly reset the boss otherwise the door to the boss won't open even with all the team dead.
