I agree with this presentation. Expensive to buy, but more expensive to be without coverage. Both time and money.
I recently learned that many American Express cards are eligible for "premium car rental insurance." You need to enroll your eligible Amex card, but, if you're enrolled, your Amex coverage for damage and theft becomes primary..Amex will charge you a fee each time you rent a car, but I think it's well worth it. The fee is per rental (up to 42 days in most states), not per day. Anyway, if you are an Amex card holder, you may want to check it out!
I travel been renting cars with my AMEX card with premium car rental insurance. I'm a safe driver, but unforeseen misshapes can occur anytime. Last May, while driving a freeway north of Denver, CO a large rock bulls-eyed the windshield of my rental car I was driving. Over the next couple of days, the windshield began to crack. AMEX insurance benefits was prompt and very helpful. The problem was with Fox Rental Car (slow response and no updates, etc). After 3 months, Fox finally provided me with a $1,200 bill for windshield replacement. I paid it and, in a few days, AMEX reimbursed the $1,200 to my AMEX credit card account. I highly recommend AMEX Premium car rental insurance. The AMEX premium rental car protection does NOT include liability. I am looking into Allianz Rental Car Insurance coverage, I heard they have rental car coverage.

What advice do you have for foreign travellers? People who dont live in the USA?
Just be aware that some states here in the US require you to have an International Driving Permit (IDP) in addition to a valid driver's license from your own country. As far as insurance goes, if you have a credit card that includes insurance coverage whenever you book a rental car use that. Otherwise, I would recommend getting the car rental's insurance especially if you have never driven here in the U.S.
