Shit happens. Sometimes it's out of your total control. You can do what you need to help the fish but for the most part it's up to the individual fish's immune system. No one is intentionally letting HITH take over. It just happens.

Hang on, I don't mean to be rude and I have huge respect for you. However, I've just watched the video and it doesnt identify what's causing the ulceration and holes in the head. You state that it isnt bacterial and you found no parasites other than the gill fluke.
Surely it HAS to be hexamita to cause those kind of holes? I know you didn't find any in the faeces but the evidence from the holes points thaf way.
One other thing I wasnt going to mention was the nitrAte. You suggest that it's under 50 so the owner must be doing enough water changes. I would suggest that ifs under 50 because his tank isn't fully cycled. Hence the ammonia and nitrite readings.
Despite what I've said, I do admire your work tremendously. Thank you for everything you do.

Thank you so much for your educational videos. I learned what I DESPERATELY needed to know to hopefully save my Oscar.
    Just FYI.....
During the part where you were explaining the dosage for the bacterial infection treatment, it SOUNDED like you said 14mg of Oxytetracycline instead of 40mg. I really thought I heard "fourteen" ...but, after doing the math...I realized you said "fourty".
However....someone else might not catch that.....and dose their fish ineffectively. Would you consider putting the dosages up on the screen in writing as you explain it? It would really help those of us with different accents and enunciation than your own.
    Thank you!

Hi, I'm not sure if you're going to see this but I wanted to determine whether or not my betta fish has hole in the head disease. So I just noticed she has pits on her head that aren't too big looking but they vary in size so I immediately thought she had hole in the head because of the pits and her momentary lethargy. At first I think she was just constipated because of how lethargic she was and then I noticed the pits in her head. I bought medicine right away but then I noticed she stopped being lethargic but the pits are still there. She is as energetic as ever but because of the pits, I have no idea whether or not she has hole in the head. If any of you can help me determine whether or not she has hole in the head I would be very thankful.

In line UV will only take care of suspended algae that pass through the UV, so is good if you have that issue. An electrolysis unit may be more beneficial for algae growing on surfaces within your aquarium.

Jalal Jalal medicines used and their dose rates are customised to each situation. In this case we used praziquantel and oxytetracycline.

Hey doc I need your help!
I have a 1.5 year old green terror cichlid which is about 7" long. Very healthy looking, active, eats great too.
I believe he has hith which was triggered by a small injury above his eye that happened after he hurt himself on a branch in the tank. I small pit formed. Maybe 1mm in diameter or smaller.
He also has the occasional clear or transparent green/brown poop. But often times it's normal.
    Treatments he has recieved.
    1. Epson salt soaked food for 5 days, followed by feeding new life spectrum hex shield for 5 days. Keep in mind he didn't eat much of the new life spectrum food. Broke it down in his mouth and out the gills mostly.
    2. Metroplex with focus sprinkled on his favorite small disc pellet. His dosage during this time would of been on the high side of the recommended amount of metro. I fed him twice a day for 2 weeks.
My concern now is that even after all this he still has some poops the odd time which is not normal. So I don't know if he is still infected with the parasite or not. I don't want to extend and keep medicating for long time which will cause harm.
    Help please

As discussed in the video, the causes of HITH can be varied, and treatment is aimed at addressing the primary causes.

do you find prazi is the best medication to treat hith, as there are many other meds such as metro and flubendo, and all the other hith aquarium treatments, why did you choose prazi, is it a fast acting treatment or is it because its readily avail to you. thanks

If I were to take out the fish from the infected tank and left the tank empty of stock for several months will The Hole in the head disease die off since there is no host fish? I also thought of removing the fishes to a quarantine to treat with metronidazole. My infected tank is 260gallons with fancy goldfish. I plan to move them to a 20 gallon to treat the fish separately. It's only 3 fancy goldfish left the rest died. Also for the infected tank I plan to overdose it with Salt hoping the satiny level will kill the HITh disease. Please respond to my comment ASAP

ThatOne Guy theoretically, obligate parasites will die out. however, recent evidence in other parasites suggests some parasites may survive longer periods, living on slime of the fish! so i can't say for sure. how long you'd need to spell the tank

I think Treating with metronidazole for the fish in a separate tank will eradicate the parasites for them. I'm just curious if leaving my stock tank empty of fish will let the parasite die off without a host.

Its either emptying my infected tank and letting it dry out or just leaving it alone without a host fish hoping the parasite dies

chlorination and drying is routinely used in high biosecure facilities. remember that some fish naturally carry Hexamita, so it may come in with the fish. best is to maintain healthy water and fish.

ThatOne Guy Metronidazole everyday and change water 50% percent everyday. Till the hole heals. I have recovered my Flower horn this way. Requires lot of patience. Good luck.
