      Hi Michelle. Can you do a video on why monofocal iol's is recommended for eyes with commordities ( hope I got this correct ) like diabetic retnopathy and glaucoma? Have you ever seen success with multifocal iol's and these conditions?

      Dr. Lee. Excellent video on "unhappy after cataract surgery." I am in this camp but may be influenced by a massive amount of floaters (asteroid) that need to be removed through a vitrectomy. In one of your future videos, could you address vitrectomies, their pros and cons and what impact floaters have on the quality of vision after cataract surgery. Many thanks. Herb

      when you have a patient who only wants a monofocal (same focal point in both eyes), how do you pick what distance to set the focal point?

    I am thrilled with the results of cataract surgery. Have high myopia and wore contact lenses since 1968 (first non-gas permeable and later rigid gas permeable). I did quite a bit of research before cataract surgery and also discussed my expectations with the doctor. Up to 95% of my life is reading books and documents and computer work and I wanted to be able to do that without glasses. I said I would be willing to wear glasses to drive, if necessary. I went with monovision IOL implants with one eye corrected for near and the other for distance. I knew I would have no problems because my contact lenses were fitted that same way and I had worn them for decades. I now have 20/25 OU and I don't need glasses to drive. Every day I wake up and don't have to reach for eyeglasses or insert contact lenses is a blessing. My life now is so much better.

      Was there any consideration as to which eye went for distance and which for near? It seems having the same set up would be successful but my surgeon doesn't seem to take that into account: wants to do my weaker eye first, and set it up for distance (this eye was actually better for near) and do a mini-monovision with the second surgery by setting the stronger eye (naturally better for far) for near.

      ​@ckp2ator389 the dominant eye is set for far vision. It sounds like you don't know what that is. Please research it. My left eye is weaker ie had a higher minus prescription but it is actually the dominant eye. So the left eye would be corrected for distance vision

      @@shobhnakapoor1399 Yeah, I later saw that dominant eye is not necessarily the one better at distance. It's the one that the brain chooses, generally they say for right handed people it's the right eye, but not true in every case. I'm going to do a couple of the self-tests (e.g., look at centered object with both eyes, then cover each eye separately, which eye sees object). Thanks for the correction. Did you end up getting monovision with your surgery?

      @@deborahd8651 Left eye: Alcon SN60WF; Right eye: Johnson & Johnson DCB00. My right eye (my dominant eye) is corrected for distance and my left eye is corrected for near. Had left eye surgery first in 2018 with right eye surgery in 2021. Still absolutely thrilled with results. Your situation may be different as each patient must be evaluated on a number of different variables.

    I just watched your video. I had cataract surgery in 2008. Things went well at first but soon led to a detached retina. Cataract surgery is the leading cause for this not MMA or Cage Fighting. Candidates for the surgery need to know. Thank you for a presentation 😊.

    I had my cataracts removed and after consulting with the doctor and making the decision I went with the standard lens that insurance pays. All I wanted was to have good distance vision and I did not mind reading glasses. I got results far better than I ever had as I had extreme far sightedness. At +6.00 in the right eye and a +6.25 with +3.00 on the bifocal to my last exam at 20/20 in the right eye for distance and 20/25 in the left eye at distance I am blown away by the improvement in my life. I have also been rather light sensitive and to degree I am still a bit but nothing a cheap pair of sunglasses won't handle. This is so strange for me as I had glasses since I was 3 years old and at 66 years old to suddenly not need then 90% of the time is just something I never expected. I always figured if the operation could achieve a reduction int he weight of my glasses to a +2.00 to +3,00 I would have been completely satisfed but to get the clarity and having NO issues with glare at night I have with my rebuilt eyes this is so awesome. When I had the final exam last week and the doctor told me see you next year I was so happy I shook his hand.

      I'm currently +5.50D in both eyes, and I am considering having cataract surgery. I was wondering if you were bothered by the loss of the magnification that your glasses used to provide after you had cataract surgery?

      @@urbanimage No I was more bothered when after the first eye done and the waiting to have the 2nd eye done, the loss of magnification in my left eye was just so wonderful as it was the worse eye and it instantly was the better eye until the 2nd eye was done. After the 2nd eye was done I was amazed by the eye sight I have as the only time I now need glasses is when reading. Any good doctor that does cataracts should easily get the correct lens implant.

      @@mikeske9777 That's interesting and rather reassuring to hear. I asked because
      years ago, when I tried contacts, I found the loss of magnification compared to glasses was very noticeable. I asked my ophthalmologist about it when we were discussing cataract surgery, and he said he'd never had a patient mention it, which quite intriguing. Of course, it's probably just a question of getting used to a new 'normal' sharper post surgery.

      @@urbanimageI missed being able to take off my glasses to read or be on my phone. My first eye was corrected to 20/20 and I quickly realized that was way too small for me to be able to read. I opted to have less correction to the second eye for a monocular fit and am glad I did. Towards the end of the day, I sometimes want readers but for the most part this worked better for my preferences.

      @@susancarrick5510 It sounds like you were shortsighted (Myopia) before surgery, rather than farsighted ((hyperopia). So you would have lost your ability to see close-up., but overall, assuming you had warn glasses, you would have seen things a little bit bigger after surgery. The opposite happens if you are farsighted and wore glasses before surgery.

    I had cataract surgery in 2018 and I'm extremely happy with the result. I was a contact lens wearer from the mid-60s and I'm beside myself to not need glasses to see. I went with the lens' to see in the distance. I had been using readers prior to this and didn't mind still using readers. My vision is nearly 20-20.

    Appreciate Your channel. I am happy again with my cataract surgery from 2 years ago.. I used a good younger doctor here in Arizona. We did the upgraded Toric lens. Best vision I have had since I was 8 years old. I am 55 now. No longer need glasses for distance or reading which is wonderful. Unfortunately 3 month ago I had a detachment of the Retina. I got another surgeon to fix that with a vitrectomy.
    She did a good job.
    I have the good post cataract surgery vision once again..

      Oh dear, that is my biggest fear - is a detached retina, especially with my glaucoma. Was it very painful? I'm sorry you had to go through that!

    I had cataract surgery 18 months ago. I am very dissatisfied with my vision. I have much difficulty with my ability to read and my distance vision is much less than I had before surgery. My doctor says everything looks fine but I can't see very well. I don't know if it was worth it. So many people say they can see much better. I cannot.

      Very unhappy with my cataract surgery my vision after surgery is 20/40 I fill very unsecure driving at night time can't see well anought. Also can't read they prescribe glasses but is worse😢

    My eyes re adjusting to the light has been the worst part of my surgery, because nothing is consistent. My doctor said that everyone heals differently so I take it day by day. Nothing else I can do.

      My eyes were extremely photosensitive after keratectomies and cataract removal. Outside I was almost totally blind. Even the darkest of sunglasses didn't help enough that I could walk independently on a sunny day. This lasted weeks and I was becoming very anxious that I'd never adjust. Now, almost a year later, my eyes have readjusted to light. Hang in there.

    This is a very brave action to allow folks who are unhappy with Cataract surgery to be able to voice their extreme dissatisfaction. Frankly I'm happy with my early diagnosis and since I'm still 20/20 with glasses, I will leave well enough alone. That said, I'm cutting all added sugars and carbs from my diet and have heard so many positive results that includes eye health.
    I am upset mostly that the VA Eye Clinic never mentioned anything to help avoid worsening or improving symptoms. This is why we go to TH-cam and channels like this one.
    A MAJOR thumbs up and of course I subscribed to this channel.

    I had cataract surgery a couple of years ago and my optomoligist is a totally awesome surgeon. I had him for many years before the surgery so he knew my eyes very well and my surgery was a total success. He is still my optomoligist to this day.

    I had surgery several years ago and soon after had vision problems. Doctor never really addressed. I finally went to Boston Palmer Eye Institute in Miami. They told me that the lenses of the cataracts were dirty and I need to have laser surgery to clean the lenses off. I’m very disappointed that nobody has ever addressed this before. Thank you so much.

    I had my cataract surgery around 20 years ago. I’ve had no issues. My left eye I see up close and my right eye is for distance. It does take getting used to. I didn’t want glasses and I got used to it. I have no regrets.

    I saw so many ad experiences on the internet that i didnt want to get my cataracts done. Finally did them on 12/03/23 and couldn't be happier. Just did the basic lens that is covered by Medicare. No issues, other than some wierd flashing the first 4 to 6 weeks from bright light entering from the sides behing my glasses lenses. I did lenses for close and distance with glasses, just like I've been since 3rd grade.

      it sounds like you have a good doc - verifying what the patient prefers (in terms of being able to see close or far without glasses) before surgery is huge!

    I have severe dry eye syndrome. I had cataract surgery in 2019 and I'm not happy; I wanted my CLOSE up vision corrected, not my far away. I read a lot and was struggling to see the ingredients in my groceries. Well everyone said oh, no, you can always wear reading glasses/keep them on a chain around your neck. Well I tried, but they kept dropping off or getting tangled in my seatbelt. I finally had to get progressive lenses I couldn't afford. Moral is, stick to what you know is right for YOU.

      People need to ask questions about the surgery before and after. My surgeon constantly asked if I had any questions everytime I saw her. You should never leave the surgeons office without any doubt. The only stupid question is the one that is never asked. If you must your mind, get a 2nd opinion. My results are better then I expected. My right eye is 20/20, my left eye is 20/40. My left eye has always been a problem for me since birth. But I do see better then I did with my left eye. I had a asthmatism in my left eye for just about my whole life but it is clear and focused. I do need reading glasses but I've worn glasses my whole life so it doesn't bother me.

    Had mine this past Monday and I have been diagnosed with negative dysphotopsia. Dr said it should work itself out within 3 months. I hope so because it's very distracting. Had a retinal detachment a little less than a year ago and my vision is better now than it has been since that, so I'm still glad that I got the cataract surgery

    I had my vision corrected for cataracts in July 2022 (multi-focal IOL in both eyes - July 2022), age 50. After cataract surgery my vision was terrible, not clear/crisp at any distance, blurry, full of obstructing floaters (like looking through soup), nighttime halos and starbursts so bad that I cannot drive, and overall not great. I saw better before the surgery while wearing glasses or contacts. It took a year of complaining to my doctor before being assured that Lasik would “correct” my already “corrected” vision, but not he halos and starbursts. After having Lasik, nothing has changed. There is no consistency in my vision, it just sucks all the time. I now have to wear 3 different glasses (prescription distance, readers 1.25 & 3.0) after being told I could "throw my glasses away after cataract surgery." There was no "wow" moment for me. My eyes always feel strained, they burn every night, and I always feel like I'm wearing irritating contacts that I just want to take out...but I can't because this is just my crappy vision now. It has been a frustrating year that has negatively impacted the enjoyment of my life. I feel like I've had some horrible accident or illness that has left me with impaired vision. I regret having the surgeries and I wish I didn't do it. I'm going to other doctors to hopefully fix this disaster. So many promises not fulfilled...I want my money back.

      identical story here. I spent close to $8k and told I wouldn't need glasses at all. I cant see up close, I can't see clearly at a distance. Shadows, Daylight, etc awful and I no longer can drive at night and during winter, that is 4:30 pm. I am 57 and really messed up. Why is there no recourse available? or is there?

      @@jerryknapp6175 Gosh, I feel the same way...my depth perception is terrible - can't see close or far in one eye, because my doctor told me that I should get the multi-focal lens , even though it was only in my left eye....can't drive at all! 😡 I am hoping to have my lens taken out and a straight lens to correct nearsightedness (like my other eye), be the replacement. But, I am really concerned about the risks...oh, also, see almost double at times with my left eye..I feel like I got duped, because they talked me into this lens...wish I had listened to my gut and not gotten the multi-focal lens... 😢

      Well at least I'm glad to see I'm not alone I had my left eye done in September and I wish I would not have I was seeing better before the surgery now I can't see far or close up and reading is a problem along with the sunlight and my eyes constantly burn I have an appointment coming up I'm almost sure they got the power wrong on my lens I was supposed to get my right eye done but I canceled the appointment I feel so lost it wasn't what I was expecting...

      I have the regular cataract surgery done. Not the lasix. I was able to see things close up really well but things far away were blurred after about 6 ft. After the surgery, My close-up vision is horrible. I have to use a strong pair of reading glasses and my far away vision is better but it's still blurry. I'm sorry I ever got the surgery. Now I have to wear regular glasses and readers. It's a pain in the arse.

      @@Crash911CPR I totally understand what you're going through. It is a really terrible situation that could have been avoided if your doctor(s) was completely honest with you. Doctors don't tell you of the awful dark side of cataract surgery, they only tell you how wonderful your life is going to be after the procedure and that you'll be so happy and can throw your glasses away (that's what they told me). None of it is true. I still need prescription glasses that still aren't great AND I also go between different power readers depending on what I'm doing: 1.5 power for general up close and 3.0 power for anything really small that I'm working on right in front of my eyes. It is truly a frustrating way to live considering I was perfectly fine prior to surgery. Having cataract surgery, multi-focal premium IOL implants in both eyes, has made my vision worse than before and my doctor just doesn't give a crap. They got their money (mostly insurance $) and they'd rather "complaining/unhappy" patient just go away rather than deal with them. What they're doing should be illegal.

    Had cataract surgery 7/12/2024 on left eye. Even though I have 1.4D astigmatism. Never needed distant lenses. Only 1.75 reading glasses. Surgeon promised me I would see clearly again at a distance and be able to drive again at night without the headlight glare. Intraocular lense J&Johnson DC 800 implanted. She decided I did not need Toric for astigmatism. Now vision is at 20/60 without corrective lenses. Everything ghosting, blurry eyes constantly trying to focus. Referred to Lasik surgeon. He wants to wait 3 months to hopefully get rid of astigmatism. I am MISERABLE. Optometrist examined eyes. Prescribed glasses to help with the 1.5 astigmatism that I now suffer with due to the surgery. Hoping that I will be able to even drive again in the daytime and see things at a distance clearly, No one seems to know what went wrong. Make sure your Dr chooses wisely the right cataract lense. I would give anything to be able to go back and get her to put in the Toric. They cannot redo this surgery. Lasik is my only option.

      @@janathomas4203 The cataract surgeon had originally said I had the options of replacing the lens she had just put it in a week prior. Or prescription lenses and or Lasik surgery. Immediately that same day as the post cataract eye exam she called me at home stating urgently that after concurring with the other Cataract surgeons at her clinic she cannot replace my lense with a new one. Referring me to a Lasik surgeon within their system.I need to talk to her again about all this prior to seeing the Lasik surgeon for the second time. 3rd eye exam with his office. He is monitoring my eye to see if it stabilizes enough for lasik to supposedly fix the astigmatism that the cataract surgery made much worse. He claims that is a minimum of 3 months following cataract surgery. I spoke with an optometrist off the record about all this. He said Lasik is not always the answer and is very invasive. I wish I knew what is the best path to getting my vision restored properly and safely.

      Updating. After several visits to Eye surgeons. They think that Lasik is the best option. Not sure why the lense replacement with Toric they do not prefer. The glare I have at night from headlights is still awful. They do not know if Lasik will help with that. They do claim that with Lasik I will be able to see at a distance without glasses as I was prior to cataract surgery. But that was promised prior to cataract surgery. Hoping this time it improves my vision.

      @@silkrose44 just a tip: you might have posterior capsule opacification. Especially if you had posterior cataract this can develop very quickly after surgery, and the effect is very similar to a normal cataract (dimming, positive dysphotopsia, blurriness, all of it), depending on how opacified the membrane is.
      I have posterior polar cataracts which meant my doc couldn't even clear all of the cloudiness on the capsule, and it has only gotten worse in the 6 weeks since surgery in the first eye (he confirmed this under slit lamp).Today I'll go for a yag laser capsulotomy which will remove that residual haze and increase acuity, and it should finally eliminate huge nighttime starbursts which are "new" in that eye.
      Note that "new" is relative as prior to surgery I could not see anything clearly out of that eye, and I had horrible glare day or night, and quadruple ghosting around lights at night, which actually downgraded the vision I had in my other eye by quite a lot. I never had starburts before though, and it appears likely the cause is actually small visible creases in the capsular membrane, which will also be eliminated with the capsulotomy.
      tldr; have them check for PCO before getting into lasik

    I had my surgery done last moth and it has been such a good change for me. Being so near sighted for over 57 years and now being able to see at a normal distance and just have readers for glasses has been something very special for me. I go see my surgeon tomorrow and want to thank her for what this has done for me I am just so grateful I was able to have this done now and enjoy life more

    I thought we needed a new tv and nearly bought one but my cataract surgery appointment came up and yo and behold it wasn't the TV that was out of focus it was my eyeballs. Fantastic get it done.

    in may 2023 had cataract surgery. my left eye was the worst and was told i was almost blind with the cataracts. both eyes were done about 2 weeks apart. surgery felt as if it was about 15 minutes. i am 71 years old and my vision is just about perfect. nothing hurts at the operation , get it done you will love the results

      I am 67 now. Had mine done in August. I have 20 20 now. The colors and details of what I can see now are great.

    Hi folks, it's a big decision and I'd like to help. I had my surgery 1 day ago and I can see brilliant. Everyone's luck will very, but I did all I could to maximize my chances. I picked the top surgeon in my area, I paid as much extra as he said he wanted for the extra stuff to maximize my chances. I spend may be 100 hours on research, I made them triple check all the measurement's, I figured out exactly what I want from my surgery and triple communicated it to my surgeon. Things can still go wrong, but that's when you want the best surgeon at your side. If everything goes perfect a monkey can do this, but when there's a problem with your eye only a great surgeon will save you. And don't forget about the greatest surgeon of them all watching over you!!! Good luck and I hope your surgery turns out better than mine.

      I also had the best surgeon and hours of research and here I’m after a cataract surgery 38 hours later and I still have a blurry vision and cannot focus on anything and I was promised 20/20 vision, my eye pupil is still dilated after all this time, still have pains in my eye, gritty eyes. I use drops and hope for the best. Distance vision is not good so as close, at middle distance I see something like contours of items and just wish they weren’t blurry.

      5 days after surgery: eyes are still blurry most of the time, cannot read with that eye, distance vision fluctuate and depends if eye got hydrated or not, not happy with my vision at all yet. Letters don’t have a focus at any distance. Occasionally in the morning I can see some flowers outside if the fresh air goes on my face, noticed a number plate outside. But inside any sharpness disappear and mostly blurred. Maybe because of dexamethasone and antibiotics drops, I don’t know. Seeing doctor in a day.

    I MUST SAY I need up close vision for fine details at work and did not know I would LOSE it after he put in lenses that restored my far away sight. Someone really needs to think and talk to their Ophthalmologist before surgery about it, I would not have minded wearing glasses to see at a distance versus struggling at work trying to use magnifying glasses and such! Believe me when you can't see up close that matters too!

    I had cataracts at birth and even though I had surgery at an early age, my brain was never developed and just decided to focus all its power on my good eye and I’ve had perfect vision in that one eye my whole life

    Good video! I opted for fixed focus lenses set for distance and had a good result. Removing the cataracts greatly improved my night vision. I was already used to wearing glasses with progressive lenses, so continuing to need glasses after cataract surgery was not a problem.

      That’s what I plan to have done for my eyes. I don’t mind using glasses since I’ve been using them since I was a kid. I just want the cataracts removed.
      Multi focal lenses look so enticing but after reading comments about them such as halos and such, I’m starting to think they’re not worth it. If it sounds to good to be true, it probably is.

    When I found out cataract surgery lenses were permanent I decided to get basic uncorrective lenses and do my vision correction with eye glasses just like I've done all my life. I'm pleased with my results. I do have dry eyes now which are easily helped by preservative free OTC eye drops. I was legally blind before the surgery and can now see again. A true miracle for me.

    Very informative video. My 80 year old husband had cataract surgery and has been told he has 20/20 vision in both eyes. However since the surgery he seems to be having some difficulty walking and claims his sight is now affecting his balance.

      Please do research. My surgery was a disaster for me. In Houston, with a well regarded doctor. Had I known, I would never have done. And $6K out of pocket. If people tell you it's 99% effective - that is a lie.

      Me too age 72. I am going to do more research before getting cataract surgery. I still see ok now without problems but my eye doctor says I need cataract surgery now.
      Lots of possible bad side effects. Glare, lifetime dry eye issues caused by cutting nerves in cornea, possible floppy iris if you have taken even one dose of Flomax.
      Picking a lifetime lens is one of my biggest issue I have currently. There are no re-dos. Far, intermediate, near mono lenses do not have loss of contrast, halos and glare issues like like multi focus lenses do.
      There are many more problems with cataract surgery not mentioned here in my comments that I found on the internet.
      Eye doctors do not tell medical patients about the bad side effects of cataract surgery.
      New laws should require a print out sheet or internet link to all possible cataract surgery complications and issues in writing.

    I'm late to the party. I had cataract surgery in 2022. I had severe astigmatism on both eyes. Even though Medicare did not consider correcting astigmatism medically necessary opted to have it corrected, so that portion of the cataract surgery cost was on my nickel. I'm very happy with the results. I no longer need corrective lens for distance vision. This has been a boon as I can use cheap safety glasses when working and generating a lot of dust. I have bifocals but find I take them off a lot now and occasionally forget I took them off until I try to read tiny print on my phone.
    My wife has been advised cataract surgery is in her future, probably next year. As a patient having someone hack and whack on your eye is intimidating but the procedure is amazingly quick, the prep getting you ready takes longer then the half hour or so the surgery takes. I find the procedure amazing, that something this complex has become routine and fast.

    I had cataract surgery in 2017 and I am very happy with the results.
    I opted for the multi focal lens. In addition to being nearsighted i also became reliant upon more and more powerful reading glasses.
    Prior to the surgery, i already struggled with glare when driving at night. Was especially bad if the road was wet. Part of this may have been the type of cataracts that I had. They were a main cataract surrounded by a ring of other cataractd.

    I like this doctor's candidness, but need to deal with one misnomer. It isn't "night time" glare - it is "all the time" glare! If you work on a PC, you've had it. My only recommendation to would-be cataract patients is PLEASE DO NOT opt for a multi-focal lens or even a so-called "comfort" lens (aka "EDoF" or AT LARA lens). The glare I have 4 months on from surgery is impairing my ability to function as I did previously. If I could go back in time, I would choose a monofocal lens. The glare off the EDoF is nothing like what was described in the Carl Zeiss product brochure. I am suffering with the awful levels of glare that was shown for the Tri Focal 'AT LISA TRI' lens. I generally regret having this surgery. Even the YAG capsultomy, after 10 weeks, made things slightly worse. I was fine with glasses, but surgeons seem to assume you want to be glasses-free. I just wanted to be cataract free so I didn't have to change my glasses prescription every few weeks. I hope this advice helps someone.

    I am very unhappy. They asked me if I want long or short vision. I chose long vision. I didn’t realize my close up vision would be so bad I can’t see anything on my phone with that eye. So now the eye that I didn’t have done has to see close up and the other is long distance only. I don’t even trust anyone to take the cataract out of my other eye. My eyes get so tired now. My doctor says he can not replace the lens without tearing up my eye. They have a long way to go to perfect cataract surgery.

      Please get the other eye done asap..I went for 2 yrs like that and it everything got progressively worse..my equilibrium went crazy at times..I fell quite a bit..I was seeing big and little..terrible ..after that was done my vitreous started detaching in my eyes...pulling on my retinas..still seeing Retina Specialist..took 4 years to heal..I see rays of God in my right eye..surgery did not go as well as it should have..I am now a candidate for the Yag..laser to break up another type of cataract that has formed..now I have it in both eyes..it's the luck of the draw...some patients will be fine.. and some wont..post Cataract Surgery..😢

    I harbored no illusions about wearing glasses before and after surgery, as I was born with Albinism. I did my research, but knew my sight would only worsen over time. I have problems with light and glare and was warned by the retinal specialist. Even so, I am thrilled. As an Albino I could have it so much worse!

    Thank you doctor for being available to guide us.
    Just over a month back, I was talked into undergoing cataract surgery on my left eye on which I believed I had very good distance vision and could read fairly satisfactorily.
    Now my left eye is not providing me with vision as clear as I enjoyed prior to the surgery! Reading through the left eye has certainly improved and colours seem to have better resolution and brightness.
    My problem is that after surgery, I find it very disappointing when it comes to watching TV, reading shop name boards and deciphering the LED count down attached to traffic lights.
    I hope you would advise me on how I could at least revert to the vision I enjoyed prior to my surgery.
    I am in Sri Lanka.

    Post Op I have blinding glare from any overhead light or natural sunshine now. I have to wear glasses with the attached solar shield to block the glare. Night or day. Maybe this will help your listeners. Thanks Dr Lee for your informative videos. I’ve learned a lot from you.

    I am heartbroken with my vision after cataract surgery. I was sold Crystalens, which were supposed to eliminate my need for reading glasses. Now I need bifocals, too! I didn’t need glasses for distance. Now I do! I have been to other doctors to see if the lenses can be removed. I was told the lenses cannot be removed “from the bag”. I paid over 5k for lenses that made my vision worse!

    I had PanOptix in both eyes about a year ago. I'm very satisfied now, but the first two months were hard and I had days I wish i hadn't done it. I didn't have cataracts, but decided to get the lens exchage so i could just see better without having to have multiple pairs of glasses. My vision today in 20/20 far and near, but it took 10 months to get there. I was testing 20/30 distance after the surgery, but close was already 20/20. I had a negative dysphotopsia in my left eye that took 6 to 7 months to completely clear up. I also had YAG to both eyes at 11 months - it was very easy and helped get that last little bit of distance back. Dry eye was a slight issue as well - i still use Systane drops in the morning and before bed. I still see some halos at night (especially red LEDs), but its gotten much better and doesn't bother me at all now. Best wishes to everyone out there going through the healing process.

      Hi I just had PanOptix done in both eyes 3 weeks apart, second eye was 4 days ago. I was at the beginning of cataracts but was fed up of wearing contacts and reading glasses. Vision from far was -7 and -5.5. I am not happy right now. Distance is bad in both eyes. You say that you weren't happy after 2 months and it took 10 months to improve . From your experience does the distance get better with time? How long? I am not sure what YAG ( I did look it up) . Did your eyes improve with the Panoptix or was it because of the YAG surgery? I feel like I am making myself crazy lol reading too much. I will see my doc next week for a follow up but need to hear from someone who has been through it!! Thank you!!!

      I too had Panoptics in one eye so far. Near vision is almost perfect while distance vision is blurry. I can't see leaves detail on trees 20 feet away. My nonoperated eye has better distance vision than the one with Panoptics . I'm hoping my problem will clear up. It has only been a month since surgery.

      I had Panoptix toric lenses implanted 10 months ago. Even though this lens has considerations, the upside is well worth it. It seemed to me as time went on my vision and the ability to deal with the halos got better to where it's no longer an issue. Give it time, good luck.@@mindyluba9668

I had the yag done , I got glasses and used my eye drops, I even bought expensive mascara, and my eyes are blury 90% or more of the time. I'm very unhappy with the surgery!!!! I wish there was something I could do to make my eyes better. 😢

      @@SM-sj6lj In the early months, i considered going to Shannon Wong, MD to see what the options might be. Look him up here on youtube. He has a lot of cases where he has replaced lenses that people aren't happy with. I know it wouldn't be cheap, but might be worth it in the end.

    As you mentioned, I think my dissatisfaction is due to unrealistic expectations, BUT I blame that on my doctor and his staff. I understand their desire to promote the positive aspects of the surgery. But it is wrong to overpromise, and not to prepare the patient for how difficult the adjustment period might be, or that the results may not be perfect. I also blame myself for believing them 100% and not doing my own research and asking them more questions. I will be going back to progressives just like before and honestly, I feel a bit cheated that I paid for premium lenses and should not have.

    I had cataract surgery last summer in July, on both eyes two weeks apart. I paid an extra $$1,400 to have corrective lenses surgically inserted into my eyes. At first I wasn’t happy with the results since my vision was blurry, but after a month I had perfect 20/20 vision after my eyes completely healed. I’m extremely pleased with the outcome.

    About a year ago, I had cataract surgery with the PanOptix lens implanted. I had HIGH expectation, but my vision has been so blurry that I have had to move my chair closer to the TV and increase the type font whenever I use the computer to send or receive e-mail or read anything on my iPad / Kindle reader. I had YAG surgery, but that didn't help. I've seen a couple of eye doctors who examined my eyes and said everything looks good with the PanOptix lens implant. My ophthalmologist and optician have told me that my blurry vision is from dry eye. I use eye drops, but they don't help much. Unless I want to go through the risk of lens replacement, there is nothing more that can be done.

    Just had 2 Toric lenses done with my cataract surgery. Although it is not perfect, I wish this technology would have been available in the early 80’s My left eye astigmatism was so bad that I was discharged from the US Navy for medical reasons. Not even Lasik in the early 2000’s could fix my left eye. Again it’s not perfect but it’s such an improvement I am not going to complain. Good information Thanks

    I had cataract surgery 16 years ago and it was a drastic improvement of my vision (I was -14 before) with pretty much no side effects. I did have to have the YAG a few years later. Also I got the monofocal IOL, so can't comment on the multifocal ones.

    I had initial IOL replacement surgery Dec 2022. Both eyes within 2 weeks. I was upsold to the Panoptix IOL, $6250. These lenses did not work for me, super glare from headlights at night, weeping and terrible vision. Had both replaced with monofocal IOL’s with astigmatism correction. The basic lenses Medicare pays for.
    My experience, do not go for any IOL other than the basic replacements even if you have to wear glasses after.

      Hi I was talked into the panoptic lens. They are horrible. The glare at night with Starburst. Can't drive at night anymore. I went to see a top specialist he told me don't have anyone go into your eyes. It can make matters worse. I was so disappointed. My quality of life has changed. He told me that the Panoptic lens have many complaints from people.

    Thank you for the video. Had cataract surgery in March 2024. My issue post surgery is I have glare/cloudiness problems indoors/outdoors/reading a computer screen due to floaters. It’s like looking through a spider wed. I did not have this issue prior to surgery and did not use a multi-focus lens. If I it to do all over again, I would have held off until it was absolutely necessary.

    Thank you for the video. I had my Cataract surgery done 20 months ago. Unfortunately, the new lens is clouding over and seriously affecting the vision in that eye. I am waiting for the Yag Laser treatment in the hope of this helping. I am very disappointed that this has happened.

    Reason 1 for me. I was recovering fine until the medicated drops (Imprimis) ended. Now it’s been 5 weeks of debilitating dry eyes. Yesterday was the first day of a slight improvement. Glad to catch your video and hear it’s somewhat normal😅.

    Had cataract surgery many years ago. As soon as they took the bandage off, I saw a bright half-circle in that eye. It is light reflecting off the edge of the lens. It's still very annoying, especially at night when street lights or headlights are present. I was told nothing could be done.

    Doctor Lee, I've seen a couple videos on new accommodating lenses that are in trials. As I understand it, the muscles that allow for natural focusing atrophy by the late 50s. So what good does it do to have accommodating IOL lenses that depend on those muscles? It seems that such lenses are ripe for unhappy results.

    I dislike glasses and decided the best option was multi-focal for both eyes. Three weeks ago I had cataract surgery on my left eye. It took only two days and I was seeing very well. Two weeks ago I had cataract surgery on my right eye. My vision remains blurry in the right eye and doesn't seem to be improving. With the first surgery (left eye) the doctor was done in about 5 minutes. The second surgery (right eye) the doctor spent a lot more time on and I was in quite a lot of pain for 2 days post op. So not sure if there were any complications that he did not tell me about. Now that they have my $7100, it seems they're in no hurry to follow up with my concerns except to say I need to give it more time. I hope they're right.

      Oh wow...I'm in the same boat...because of my glaucoma, I was only able to have the multi-focal (Apthera) lens in my left eye (thank goodness)! I am definitely not a fan of the multi-focal...nothing is in focus - not near or far (in my right eye), which was my good eye and did not have as bad of glaucoma- so, now, I am hoping to be able to have a replacement lens. It has been 2 months and no change..my doctor told me to give it time, too - but, he also told me that if I want to have the multi-focal lens taken out and replaced with the same kind of lens I have in my right eye, I have to have the surgery within 6 months of the original surgery. Ughh...so, I will probably do that - I have terrible depth perception now, that I never had before..best wishes to you! Please keep after your doctor! I still may get another opinion.

      @@sunnydee154 Thanks for your comments. It's been a few weeks and it seems the doctor was somewhat correct that my right eye would improve. My left eye has 20/20 and my right eye has 20/30 vision. I have another appointment mid November to see if there has been more improvement. I was told that for what I paid, they want me to have the best vision possible and if my right eye is still at 20/30 they will set up another appointment for a laser procedure. Not sure what a laser procedure entails but it's supposed to correct the vision to 20/20. I asked what the cons are to the laser procedure and she said none.
      I think a multi-focal lens in only one eye would be difficult to adjust to. I see your point about depth perception as it's taking some time for me to get used to especially while driving. It takes a lot of nerve to lay on that table and let someone poke around in your eyeballs. I guess I'm lucky and have no other issues with my eyes. My dad had glaucoma and needed to use eye drops 4 times a day. When we're young we take our sight for granted. It's not until we get older that we realize how much we rely on our sight.     Dr. Lee, thanks for your clear explanation on the topic of post-cataract surgeries, patients' expectations, and outcomes after the surgeries. That's right, disappointments could arise from reasons of high expectations, types and suitability of the intraocular lenses patients choose, the health of one's eye. For example, if the eyes are going to have monovision intraocular lenses but with different vision depths, then consideration should also be taken which lens go on the dominant eye, which can make a difference in your vision after surgeries. Also, choosing the lenses in accordance to your lifestyle or work (if one is still in the workforce).
    Therefore, do some research on the different types of lenses available, what lenses are most suitable. Write down all your questions and discuss them with your ophthalmologist in depth, to minimize the surprises after surgeries.

    My doctor put a lens that sees close up and the other lens that sees at a distance. Now I can’t focus on anything. I don’t drive so I don’t have to worry about that. I get frequent headaches. This is up high on the list of horrible things done to me that I have to live with. My vision was so much better before my surgery.

    I had both my eyes done about 6 mouths ago. I have difficulty to see and it feels as if I have hair in front of both my eyes, sometimes it feels as if the hair is pricking my eyes. The doctor told me she knows about this problem it will go away in about a year’s time. This is the worst thing I’ve ever done and I have had many operations with no problems.

    I just had my left eye done 2 days ago and couldn't be happier never been so excited to go get needles and scalpel in my right eye to get it done in 2 weeks

    I had cataract surgery in my right eye - I went from needing glasses to correct distance vision issues to legally blind in just months. I was in my late 40s - so unusual. My main complaint is that I can see the lens in my right eye, not only do I see a moon shape, my eye feels gritty. I know this is a known issue, I was told by my doctor that it would be like that for a few weeks, then I was told maybe 6 months. I never went back, it's permanent. I wear glasses full time because my brain is tricked into not seeing the lens, it sees the rim of my glasses and I don't get that gritty feeling. it's been quite a few years since this happened and I have a very slow developing cataract in my left eye. Not causing issues yet, but I can see the dull colours developing if I cover my right eye.

    Nope. I had cataract surgery 10 or so years ago (I"m 77 now). I am perfectly happy, I have 20/20 distance vision in both eyes and rarely need to use reading gasses.

    I had my cataract surgery about 2 weeks ago with multifocal lens on my right eye. I am only getting about 80% far, 40% mid and 10% near range vision as of now. The halo effects of lights really bothers me. I might have been at a very early stage of cataract where surgery might not have been necessary. I wish I had seen your video before the surgery.       My Dr advised against this lens bc of halos and my increased risk after Lasic.

    I'm 80 and had cataract surgery done last Jan. I got the standard lenses. My left eye now is 20/40 and my right eye is 20/30. Not perfect but I'm ok with it. I do need glasses for close up work. Long distance is not as good as I thought it would be so I probably will be getting bifocals to be used when driving and reading. Overall, I'm happy I did it.

    does stigmatism complicate the cataract surgery? And when we talk about multi-focal lens and the glare at night, do we mean in driving at night or just lighting at home ? Than

    Not unhappy, but surprised. My right eye was operated on last week and the left will be operated on next week. I was happy with the new lens, but surprised that I had a larger field of vision with my new lens... like a wide-angle lens vs. a normal one. My surgeon said that I was extremely far-sighted, so I am not certain if that is the reason. Should be interesting after the second surgery.

    I had cataract surgery in 2016 in IL, only 1 eye my Left eye did everything correct and true until I was released as instructed, eye drops, I followed directions and read everything I was given. The result was my left eye was worse than pre-surgery. Still to this day now 2/2024 my left eye remains worse than before surgery. I will not and have no intention to go back to where they performed and was promised my eyes would be fixed, I would have been better off never having had the cataract surgery. Oh and I wasn’t given a choice as to what lens for whatever reasons, I didn’t need surgery in my Right eye. Moving to Nashville at end of this year, I may go to a more qualified eye surgeon.

    I had cataract surgery for both eyes. Distance vision is perfect but I still have to wear glasses all the time to see anything close up and for reading. I got sick and tired of putting on readers when I wanted to read printed material. Oh well. At least the blur is gone.

    Post surgery I was told I had scarring which 1 in 3,5 experience.
    I didn’t know this. If I did I wouldn’t have gotten it so early.
    I started putting castor oil ( hexane free, cold pressed, organic and in a glass bottle). It worked miracles. No more foggy vision.
    I used to use it to for cataract before the surgery. Wish I had continued and not given up. If you read some of the testimonials it really works for dry eyes, floaters and cataract.
    It has helped me tremendously.
    I may not need laser (yag) surgery now.
    Give it a chance and see if it works for you. 🙏🙏🙏

      I use heritage brand castor oil, it’s hexane free, cold pressed, organic and in a brown glass bottle.
      I take a dropper and put it directly in my eyes at night before bedtime.
      Now w surgery, I do it am and pm.
      It has helped w the scarring post cataract. I see better and it has helped w dry eyes that’s common after surgery.
      I wish I had continued using it and not have surgery so soon.

      You can also rub it on your eyelids and under the eyes too. It’s absorbed rather quickly. If you search Barbara O’Neil on TH-cam she talks about many benefits of castor oil as well as others but I trust her the most.

    Hi Dr. Lee, Thank you for this video. I got my cataracts removed about four months ago. I chose to go with monofocal lens, adjusted for distance. I I did not want to deal with halos/glares. And also monofocal lens is one clear lens, which is more simailar to our natural lens. I don't know if this makes any difference. I just did not want to deal with the imperfect multifocal technology. I expected perfect vision, and would not mind wearing glasses for reading. I do have very dry eyes, although things appear to be improving. My vision is not great, in that I get blurred vision. My near vision is quite poor. I never realized how much I read. I do have to wear glasses all the time just for that. I thought reading was just for reading books. But, it is for much more. My optometrist told me that dry eyes can last up to 9 months after surgery. I really do hope that the dry eyes improve, and that my vision also improve. I guess I expected too much as I heard from so many people how great their visions are after cataract removal. But, your explanation helped me to understand my feelings about the results better.

    Just had my cataract surgery on my right eye. December 2, 2024. Now is December 28., I am experiencing a blurred vision after 3 weeks. I was suspecting that I might accidentally scratch the eye and the lens was dislocated. Kindly enlightened me Doc.. I will see my medical provider again this January.

    Dr. Lee: I had a really bad experience with my cataract surgery. First of all the lens was ordered less than a day before the surgery. On the day of the surgery, it turns out no one checked if the lens ordered was the right lens. It wasn't and the lens vendor (Panoptix) and the staff had to scramble to get a lens for my surgery, and my surgery was delayed almost three hours. On top of that, my eye was scratched at the surgery center. Now, one week later, my vision from the eye that had the surgery is terrible. I can't read anything after the surgery with that eye. My vision from this surgery is very blurry. It's basically useless - almost I am blind in this eye.
    Previous to surgery, the Doctor mentioned he "might" have to adjust my eye (with lasik), which I had a lasik procedure on 25 years ago, doubling the cost of the surgery. Once I paid, I was concerned about the additional lasik procedure he "might" need to perform. Several times I tried to contact the Doctor about this to find out if this was absolutely necessary and I was told no you don't need it, and no we won't let you talk to Doctor about this. That was a warning sign I should have paid more attention to (meaning if I can't talk to my surgeon about an important issue like this, I need to go to another surgeon).
    My question is: how would you approach this if you were the patient in this case? Obviously, I am very very unhappy. I am not rich and going to another Doctor is going to be very expensive. What's the best way to approach my surgeon and get this repaired? Or maybe I'll just have to get an attorney? I don't want to do that, but I am deeply worried.

    A news personality in my city had cataract surgery 6 years ago. When she tried to come back to work, she said the pain was extremely bad 3 weeks after surgery - so bad, she couldn't work at all. Two days later, she hung herself.

    Had cataract surgery seven weeks ago.
    I used to have one eye with near signtedness and the other with intermediate. My brain interpreted, and I used spectacles only for road driving or movie/theatre going. These glasses did not prevent my seeing at closer ranges (for example the dashboard).
    Now the ex-intermediate eye, the one operated, doesn't see correctly at ANY distance except precisely 1 foot. It doesn't hurt, but feels bloated, with floaters and a side bar. And I'm desperate, not being able to see my cat's face or my bookshelves.
    I amm feeling so desperate that my older thoughts of suicide have come back again. Only, this time, by stabbing through my destroyed eye...

    I had Cataract surgery about 20 months ago. I am both happy and unhappy. Happy because a) the cataracts were blurring my vision at all focal lengths, and now vision is crystal clear, and b) I was very nearsighted since about the second year, and now can see very well at distance with no glasses, including to drive, watch TV, swim, or just walk around. Unhappy because: my near vision went from perfect to not good at all. I need reading glasses or a magnifying glass to see anything small, where before others asked me to read the fine print. Without my glasses I could read things as tiny as anyone.

    Did not realize that one eye was corrected for distance and the other for short until I saw your video. It has been a month since my surgery and my night glare (Halo) effect seems high and I am told that the eyes will get adjusted to it eventually. I really hope that is the case. I also have issues seeing my monitor even as I type this with blurred text. Both near and far vision is good it is the medium one (monitor 2 to 3 feet away) is my issue. Will report back in a few months. Thanks for the informative video

    I had surgery back in January 2022. My left eye had a lens implanted for distance and in my right eye for close up. With the combination I can see clearly at any distance. The brain is amazing at compensating and interpretation. Maybe it helps that I'm a microbiologist and can handle monocular microscopes, simply by ignoring the input from the open, non-observing eye, and concentrating on the object of interest. It's (almost) all in the brain.

    I was asked if I want to remain near sighted or far sighted. I told them I wanted to be near sighted because I had been near sighted for the majority of my life. They badgered me to have it be far sighted and I finalled gave in. They said I probably wouldn't have to wear glasses. That was a big lie! I now have 2 eyes that don't work together. if I don't wear my glasses I get a headache.
    I hope everyone who reads this will make their own decision.

    Michelle, I have been following your Channel for a long time. Your content was a great help and informed and educated me prior to my cataract surgery...
    I learned a lot from you and felt reassured and i opted to, and had very successful cataract surgery on both my eyes. Thanks to all your content!.
    I have a question relating to my wife's condition and I hope you could shed some light on it.
    She had a detached retina and had to have two surgeries on the same eye. After that her eye developed a severely dense cataract making her blind in that eye. Her Retina doctor told her that if the Cataract was removed, she "may develop double vision". From your experience, and all the patients you have treated, have you had cases of "double vision"? In your opinion, does it make sense for her to have that cataract removed to restore Vision in that eye?
    NOTE: I am aware you cannot answer definitively without seeing a patient, but I would just like to hear your experience based on statistical data and your experience with cases of double vision. Your advice / opinion will be much appreciated!...

    Great video. I got the Medicare generic lens several months ago in my left eye which worked great for distance but left me disoriented in mid range and hopeless as far as reading or working with my hands: essentially blind without glasses. I wanted to avoid total dependence on glasses so last week I got a multifocal lens (Vivity) on the right. Don't go for this lens if you expect perfection but my walking around vision is very good and my near vision, far from perfect, is adequate for basic needs. My distance vision is "good" in daylight but I'll definitely need corrective glasses at some point for driving at night if things don't improve. I am dissapointed that my distance vision isn't as good with Vivity as the generic. I don't feel that I'm having cognitive dissonance between the two lenses but my brain is definitely having to adjust to the cascade of new visual information now available. Finally I wish that I could have been better prepared for my current outcome. Given the out of pocket expense of multifocal lenses there should be a better way.

I was blind with cataracts and could not get prescription glasses because of the cataracts and now I am having Cataract surgery and I see so wonderful in left eye and they are still working on the right eye .     A friend of mine recently had cataract surgery. Apparently in both eyes at the same appointment. She says that her vision is actually worse now. That she cannot see well either at night on in daylight. She said she has diabetes, but does not have significant macular degeneration. According to my friend, what a second ophthalmologist who examined her later, has told her is that there is a "bump" in the retina of each eyeball, and that he had never seen such a bump before, and does not know what it is. She said he has told her that the surgeon who did her cataract surgery should have noticed the bumps before proceeding with surgery, and should not have proceeded with surgery.

    My cataract is much worse in left eye than right. My old eye doctor a few years ago said not bad enough for surgery. My new doc is encourging me to get surgery now. She said the longer you wait the more they have to cut. I read a lot and normally just hold book close and read without glasses. Not sure what I am going to do but tend to think I will wait another year.

    I had cataract surgery and afterward discovered that the new lens floureces with ultraviolet light. Ha, I now have a superpower! I was using my ultraviolet flashlight to check for scorpions at night.

    I have been wearing tri-focals for about 23 years. Had cataracts for years but they weren't bad.... Then, all of a sudden they got a lot worse. It was at Christmas and all the lights just blurred together. It was very noticeable and troublesome. So, I had the surgery two weeks ago. Right now I am still wearing my same trifocals. If I take them off and close the one eye that didn't have surgery I do see a little better with the eye that had surgery. Everything though is very bright and I have to wear sunglasses on a sunny day. I am still in the hopeful stage that after I do the second eye next month......I will get my aha moment. Right now I don't have it. Keeping my fingers crossed. I also have astigmatism but due to income status and insurance coverage could only get the traditional lens. So,,,,, I know I am going back into glasses. I just hope it won't be the full trifocals. I hate those things. . {{{{{{{{{{{{sigh}}}}}}}}}}}}
