    Good video Nick. My Doc put me on Auto 12 to 18 which was nuts since I couldn’t sleep when it hit 16 and my mask blew off my face. I took LankyLefty advice and started narrowing the range down and I ended up with 13.5 to 15 and have slept well with AHI below 4 for a year and a half. Thanks great work since CPAP is really personal and needs personal adjustment.

    Greetings Nicko, great explanation of the patient's problem during the night and how she was affected by the algorithm used by the AirSense 10 Just for Her. It was very educational on how higher pressures can sometimes cause central apneas or more disruptions during the night and that changing the mode from "AutoSet for Her" to CPAP at a lower pressure produced better results for this one patient.

    Didn’t know too much pressure could make apnea worse. I messed with my settings on my ResMed 10 to INCREASE the pressure because I didn’t feel it was enough.

    • @CPAPReviews
      @CPAPReviews  2 ปีที่แล้ว +4

      Hi there . Very much so! It can also cause a lot of other cpap side effects like dry mouth, mask leaks, aerophagia just to name a few but Treatment-emergent central sleep apnea (TESCA) can also start to appear for some patients if their breathing can't cope with the higher pressures. This is why it's important to monitor you CPAP data from time to time to make sure it's providing good therapy.

    I wish I had known about this (or anything really) when I was first diagnosed with OSA and presented with the For Her machine last October. While I haven’t experienced the issue presented here, in fact my AHI has been consistently below 2, usually even under 1, I wonder if I should switch to APAP mode and see if I don’t feel more refreshed when I wake up. I had set my pressure range between 7 and 15, with the max on most nights ending up between 12 and 14. According to ResMed’s website, the For Her machine is recommended for females with mild to moderate OSA, and I fell just into the severe range at 32.3 AHI per hour, probably close enough to the moderate level that it might be appropriate for me. I’m pretty sure it’s the priciest machine they had to offer, and I hope that wasn’t the reason it was my only option. I was just so glad to have a treatment plan after falling asleep at the wheel and surviving a horrific rollover accident that I would have done anything they told me to do! It was a miracle that I walked away with nothing worse than a broken hand (and that was from the airbag). I figured I’d learn about the other modes when we start camping this summer and I take my nearly new AS10 backup/travel unit in the trailer, but maybe I should experiment right away with the other modes. I can hardly wait til the app is available for download, I’ve looked at OSCAR a bit but find it confusing.

    It is cool to know why KB's machine flew up to 10. So it's just another reason why ResMed making a "For Her" is just a marketing thing and has no real useful purpose ... and even worse, they KNOW about this problem, but don't apparently tell you!

    • @CPAPReviews
      @CPAPReviews  2 ปีที่แล้ว +2

      Hello friend . It certainly concerning that after only two small apneas the 'for her' algorithm cranked the pressure so high and then set the new min at that new high rate. It's such a small little sample ( literally seconds ) and to make such a large change based on that small sample is not a good way to treat sleep apnea. I do think it's more marketing than science personally. Thank for the great comment and your membership support

    I have an airsense 11 and after my first month of trying to use it I'm super annoyed with the whole thing...it either gives me dry mouth, so I adjust the humidity, and then it leaks water all over my face during the middle of the night. And just even wearing the damn thing over my face feels like breathing through a plastic bag..like I'm fighting for air. I've got the DME company calling me once a week to harass me about wearing it "at least 4 hours a night for 21 days out of 30 to meet insurance requirements", yet they sent me too small of a mask and then with these issues...it'll be a miracle of my insurance covers the "rental period"...I've got a phone appointment with my sleep doctor who charges me $150 (my cost after insurance) to chat on the phone for 15 minutes and I'm not sure it's even worth it at this point. The whole thing is just frustrating, and I spent all this time WAITING for my cpap to arrive, thinking it was going to magically fix the issues in my life.

    • @CPAPReviews
      @CPAPReviews  2 ปีที่แล้ว +2

      Hi Brittany you're certainly not alone mate. Most people struggle in the first month (sometimes longer). Sounds to me your main issue is with humidity. Here's a vid i did on that topic which might help - th-cam.com/video/MMKRLIUti-I/w-d-xo.html . If you're struggling to breathe through the 'plastic bag' try increasing the pressure relief (EPR) levels as this will help you exhale. You might also try increasing the minimum auto pressure up a little as sometimes a bit more air flow can help reduce that suffocating feeling. Keep at it mate and make sure you try a few different masks along the way as this can make a big difference also. Best wishes. Nicko

    Instead of changing her machine to fixed pressure machine, why not just change the machine to regular autoset in the settings? The whole purpose of the autoset is defeated if you change to fixed pressure. Why not use the autoset for a few days to see if it helps first? I had to buy the autoset for her because there were no others in stock anymore. Had problems getting used to it till I found out you could just change it to regular algorithm. Works great now.

    I was advised to go to the For Her in Dec 2020, when I was due for a 5 year change out of my cpap. I have had a number of things that I hear you mention, but because my Dr just writes RX and has no clue how to read the data, I have suspected something is off, but no way to prove it. Guess I need your software and need to see for myself what’s happening. I definite am not waking with the better results I had hoped for in going to AfH! I had original diagnosis of Upper Airway Restrictive Disorder, rather than full apnea, my breathing gets tighter and tighter. With my older AS 10, I was ok, though towards end,it seemed not as effective, which I thought was wear of over 5 years nightly use.

      Do you have a black autoset or the white one or is your black one set to "Soft" mode? I myself have found that CPAP works better for me. Like Kathy, I would find myself getting lots of CAs (Central Apnea means you stop breathing for no reason --at least thats what they say). I believe that the higher pressure and the puffs that the regular algorythm gives you when you have Obstructive Apneas actually tends to wake you. I believe that I subconsciously react to the puffs of pressure by refusing to breathe. So I set my machine to CPAP or sometimes APAP with a narrow range - for me 11.4 to 12.6 and I have been much happier. If you have the Oscar software you can see the graph that Niko is using. Look also at the 90% number. YOu might well set your range close to that number.

    Just subscribed to this excellent channel as I have sleep apnea. I was diagnosed with it about 4 yrs ago by a consultant in Northern Ireland (where I live). He did a sleep test and said that I stopped breathing over 40 times during the night and they immediately gave me a Philips Fixed CPAP machine. Last year I got a letter from my Health Board to say that some of the Philips machines had to be recalled but in the meantime, I should continue using the machine. I think they were hoping that Philips would recall all the machines and Philips would pay for the new machines....I have heard nothing since and it must be coming up to nearly a year. So I have decided to buy myself a Resmed Airsence 10 (which as far as I know is an automatic machine). Do you think I have done the right thing?Aafter seeing this video would it be better if I had have gone for a fixed CPAP machine? To tell you the truth I am a complete novice to these devices but I thought before seeing this video the automatic machine would be better but from this example, I may be wrong. What are your thoughts? I think my Philips machine is set to 7. Thanks again for this channel

      Hi Harry, it really depends on the results your getting with the Autoset mode (please know that you can also run a AirSense 10 Autoset in CPAP Mode). If you're sleeping well and your apnea is well controlled then there's no need to switch however if you do want to try it in CPAP mode it's very simple to change the mode. th-cam.com/video/qIfHud8ZhDE/w-d-xo.html

Is this something that my DME can program for me? Are the algorithms set so that a guy couldn't use it? I think I realize mine is BiPAP and this AirSense is a CPAP.. In a crunch.. or camping trip.. Is it likely it can be programmed for me? Your thoughts? I will hopefully find out this week once the DR provides new settings for my Aircurve IF I really need a BiPAP.. I may not.. I'd appreciate any thoughts here.. My BiPAP is certainly showing a LOT of events.. Probably mostly from exhale snoring.. but, I don't know yet. I'm an admitted dummy in all this.. but, I think the Exhale Pressure is way to low on my current machine.. I'betting if this can be fixed, I'll have better results.

      I don't care if it is for her or not, I just want the machines in different colours. Black is hard to pull off with some colour schemes, and it takes up a lot of bedside real estate so it's not unreasonable to want a pleasant look. She is on auto for her 5-12. And I am thinking at some point of the night it could be doing this, and upping her pressure to high and pushing her mouth open.
