I simply can't believe this video and all the examples of poor seamanship there are finally showing the pull out with the wake showing that they steered to port even with the guys on the starboard side of the vessel. I guess they figured if they lost the men they might as well make every effort to chew them up in the prop too instead of steering to port or shutting down immediately. I also saw absolutely no one throw a throwable or PFD to anyone! Amazing and thanks for sharing.
guys are drowning and up the bridge we have a man watching with his cigarette.. and the ship at sail while rescue boat lowering..well done captain you are one of kind!
Any chance y’all can sound an alarm and possibly tell the captain to back off the throttle 😂 maybe we can go get the other dude when we are finished saving these guys? I love how they stay balls deep in the throttle even after it’s a disaster.
They may have switched over the hooks and used the off-load mechanism which should only be used for the life rafts. The rescue boat has been lowered to the water (under the weight of the boat) and due to the mother ship making way, when the rescue boat hit the water, she was then being 'dragged' by the crane keeping it under tension therefore making the mechanism believe that the boat was still being lowered therefore preventing her from being let go. Might be wrong though - just a guess!
Sickening to see the way this happened, especially the way the vessel turned to port for persons who went over on the starboard side AND while another was in the FRC who could potentially be the 3rd MOB.
Thanks for posting this video. Drug tests would, of course, be immediately performed on the crane operator, everyone in that boat, the captain, chief officer on deck and pilot, for starters. Why they would even attempt to do that without having a clue as to what they were doing is an important question to answer in the formal inquest to follow, I would think.
I may be off on this but are they not launching a fast RESCUE craft and two guys are in the ocean way back? They now need to successfully launch a fat rescue craft to rescue those who ended up in the water due to the failure to launch the fast rescue craft. Does that make sense?
Classic mistake! The forward painter line is not there just for decoration, as most of the crew I have worked with thinks... Person in charge of holding forward painter line, literally holds a life od those in FRB in his hands!
First Mistake of all Bow-Line to short, to keep the FRC straight, Line should be fastend so far Forward as possible. Lenght shoulb be that the Line is free of Slack the Moment the FRC touches the Water. All other is Result of the First Mistake
I agree about bow line, but thre was also stern line not released before vessel touch water.FRC coxwain did not have control of FRC from begining.First step when FRC touch water is to keep him a/s mother vessel with engines, second is to release hook as soon is possible and bowline last. From video i am not even sure did they start FRC engine before lowering. All together disaster.
Any person with some sense could have foreseen this because the forward line (rope) was not held tight. The same insane event happened wirh a rib on Paul Watsons Sea Shepherd. Crazy.
Well considering this appears to be a drill, sounding the alarm would probably not do much good as everyone is pretty much already at their MOB stations, except the rescue crew who is in the water.
The crane has Korean letters on it. It might just be Korean-manufactured ship, but they usually have shipboard printing to reflect the language of owners and workers. So it's likely a Korean vessel.
His place is on the bridge (and he should have reduced the speed). During stand by duties the manning of a FRC could be 3 ABs. In a MOB situation it could be a mate, an engineer and an AB.
@jeryl1968 this might be a little bit difficult to comprehend but I'll try to use simple words: the ship in the picture isn't your uncle's fishing boat, mate. You can't just "stop" with a ship like this or whirl her around on a dime. As you can see in 1:46 the boat is already pulling a full emergency maneuver, trying to turn back around as tightly as it can.
Number Three: he turns to Portside, don't know if they even stopped the Machine Number Four: If there is a Man in the water, you better shout "Man over Board" and fucking point at him!
if you have a MOB then depending on the ship stopping distance and times are measured in miles an minutes. If you wait until the ship has stopped to launch the FRC then the MOB could already be dead or lost
Feel silly replying to a 10yr old comment, but you're wrong. You should never launch from a ship doing more than 2 knots. That was most of the problem here. Doesn't matter how many MOB's are in the water, the first goal is not to add to that amount by launching in such stupid conditions as these. Your own safety is more important first and foremost.
The winch hook release mecanis was locked. This accident is more common than we beleive. People DON´T know how to use their equipments. It was designed to release at the first contact with the water and loss of weight at the winch.
Gotta love the clown continuing his smoke during what should have been a rescue mission. Hold on there buddy we have to get the cameras for a snap shot.
Two mistakes, secondly: not releasing the falls when they began to broach, but first before that, there was not enough tension on the sea painter to keep the bow of the rescue boat into the sea instead of falling off and broaching.
Putting the engine in reverse its a big mistake in case if you saw the video to the end you saw that the back off the boat is where the mens were and that is because he went reverse so we don't now what happen to does 2 mens. In situations like this for the captain first thing is put the boat in neutral and turn the rudder to the same side that the mens were so the back of the boat will go to the other side and the 2 mens are safe for the other first thing is trowing the lifebuoy.
He didn't do a turning manoeuvre at all at 1.46, if you look at the wake the ship is still on a straight course. The reason that the stern went away is just the effect of the props when pulling astern. It is qiute dangerous to do a turn when that boat is still very close to the ship and connected. Anyway a lot went wrong, and the reaction of the crew is also not an example of good seamanship.
maybe they are, it takes close to a mile for a ship to stop, they dont have that friction needed to stop like cars trains and planes, they have water...not a good friction conductor.
Hi John, I'm guessing this is a video taken from a Vietnamese shipyard sea trial. Would easily explain the lack of seamanship.. They simply have no experience and are not professionals!
First of there is no painter line on that boat just some bloke holding a peice of string ?? so that's the result you will always get from a moving ship ,second why did the winch man not lift as soon as he seen the boat in trouble.
Consequently, the crew ran to the port side of the boat and launched their other FRC and lost 3 more guys because obviously the last thing that boat should be doing right now is slowing down. I'm still laughing :)
The "fail" is caused by the crane. On this fast deployment cranes, a Constant Tension System on winch is needed. This function allows you to place the boat on water line and paying in/out the rope following the wave movements. This function is needed for few seconds until the boatman starts the engine and can control direction of the boat, another person unhook the boad and they can sail away from the sinking vessel for example.
I do not want to come off as an armchair quarterback but I did not hear a "Man Overboard" Signal. There should have been three prolong blasts on the ship’s electrical bell and ship’s whistle. That is the basic international standard procedure. How do they have insurance? I feel bad for the crew working in such a dangerous environment (lack of training) caused by the management.
These people have no idea of geometry. The man holding the line attached to the bows should have walked further forward to the bows of the ship such that as the boat dropped the line attached to the bow is tightened and the crane downhaul will not drop vertically but according to the radius of the line attached to the bow which should always be tight as the boat reaches the sea surface, With the line to the bows always tight the bows of the boat will be held in . The hook on the crane is then released keeping theangled line on the bow tight and then this too is released as the nose of the boat is steered out. There was not teamwork and no idea of geometry and dynamics!
I believe it was a Sea Trials, that's why so many people on deck and bridge. The strongest guy on the bridge wing even didn't drop a cigarette! :) And main reason of this fail - hook was not released.
hmmm shouldnt we be hearing a general alarm in situation with man overboard? Who is watching where they missing guys are floating? Many people there so not sure. Honestly the crew is keeping its calm, its a bit like this happens every other week...
First thing to do ABSOLUTELY, after the men went OVERBOARD it would must be WILLIAMSON MANOUVERING, that consist of a turning on the same side where man felt in water, in order to avoid that the stern of the ship will hit the victim and then, only after 60º wheeling, you get full rudder angle OVER SIDE: in this way the ship will come back again on the same tracking of before.
sorry I'm a noob at these things but why launch a rescue craft when underway? If there's already man overboard isn't it better not to put distance between the ship and the MOB? thanks!
The crew of the boat is Thai. This video is typical of the way a lot of things go down in Thailand. It's amazing anything gets done at all in that country, but it is a great place to party and eat.
Number one mistake, no sea painter. A bow line should ALWAYS be lead well forward and secured to the ship released only by the boat crew when ready. The guy in the red coverall "tending" the bow line had no clue, he let that boat get away from him. Number two. When lowering a boat underway in rough seas the hook must be released prior to the boat reaching the water, the boat should drop the last few feet. In this case the main hook was never released dragging the boat under.
These FRCs should be launched from ship cruising at service speed, kept at hook until they get to ship's service speed then unhook, save the men overboard and then come, hook again and be lifted up, right? Well, I guess the problem here was that the fore part of the FRC had sailed away from the ship due to the poor fellow over there not being able to stop the fore line all by himself.
Many Big Mistakes: 1. Incorrect management of the painter (if used at all) 2. The FRB should be launched at 5knts max, it looks like they're are doing more than that 3. MOB alarm???? 4. Lifebuoys???? 5.Comms???? Whole thing completely mismanaged
the Captain and the Crew are lack of knowledge in launching fast rescue boat, and it is not advisable to launch while the Ships is Underway..Man overboard tuloy..
@kamandag0827 sorry about that !… I was not aiming at Filipinos in particular. I just meant underpaid, badly trained (if ever), overexploited poor men from third world countries
Are you blind Patrick. At 1:46 the vessel is pulling hard to Port. And YES I am looking at the wake. It's coming off the stern port quarter. If that vessel was on a straight course the wake would be directly behind it. Just run a line along that white deck railing and off into the sea. If the vessel was going straight as you suggest the wake would follow that rail line. I can't see what else the crew could have done. Many hands ran to the stern to keep a visual on the MOB.
What a.. ! You wait for any kind of reaction... but nothing happens. Phu,... but okay, it's not useless,... we can take it to our students to show them how not to.....
I'm never saw this kind of disaster in my life! totally fail procedure for launch FRB, this is lack training and lack knowledge about offshore vessel's. In seismic ships is normal this operations is daily operations.
There is one one person on the straboard wing who is still smoking while watching his crew member suffer!how can he act quickly his busy doing his shit!
I simply can't believe this video and all the examples of poor seamanship there are finally showing the pull out with the wake showing that they steered to port even with the guys on the starboard side of the vessel. now they need a fast rescue craft
guys are drowning and up the bridge we have a man watching with his cigarette.. and the ship at sail while rescue boat lowering..well done captain you are one of kind!
Its common practice to launch FRBs while underway. Nothing wrong with that. BUT everything else about this is wrong.
And this is why maintaining proper control of the painter line is mandatory.
Any chance y’all can sound an alarm and possibly tell the captain to back off the throttle 😂 maybe we can go get the other dude when we are finished saving these guys? I love how they stay balls deep in the throttle even after it’s a disaster.

MEN OVERBOARD! - Launch the FRC.
Classic mistake! The forward painter line is not there just for decoration, as most of the crew I have worked with thinks... Person in charge of holding forward painter line, literally holds a life od those in FRB in his hands!
Agreed, keep the bow forward and there is no issue.
yes. also frb is too far from hull when lowered. (i think)
@@kjdyatta yeah, ideally is should be almost touching the ships side.
what crew is this? phillipinos?
indiano.
My only question is where did the other two go
First Mistake of all Bow-Line to short, to keep the FRC straight, Line should be fastend so far Forward as possible. Lenght shoulb be that the Line is free of Slack the Moment the FRC touches the Water. All other is Result of the First Mistake

@vincent7520
FYI those officers and crew were NOT Filipino! Those were either Indonesians or Malaysians...
Any person with some sense could have foreseen this because the forward line (rope) was not held tight. The same insane event happened wirh a rib on Paul Watsons Sea Shepherd. Crazy.
If they used a " Sea Painter" this would not happen. I launched many MLB's while in the USCG.
Well considering this appears to be a drill, sounding the alarm would probably not do much good as everyone is pretty much already at their MOB stations, except the rescue crew who is in the water.
Are the two that fell off, fish food ?
No painter and RFD hook jam?
and every major aspect of the world. Do you think that means our ship is also sinking?
The crane has Korean letters on it. It might just be Korean-manufactured ship, but they usually have shipboard printing to reflect the language of owners and workers. So it's likely a Korean vessel.
It's a Thai vessel. I'm not sure if the markings on the crane are actually Korean, or a logo of some sort.
where is korean letter? you may think that words containing circle is korean letter. That looks convenient but actually it's not.
This is why we install tow booms with painter lines
There´s a guy holding painter line? Does they know what is "on load" launch?
They didin´t have no idea how to launch that mob ...
At least the rescue boat is close. Wait...
His place is on the bridge (and he should have reduced the speed). During stand by duties the manning of a FRC could be 3 ABs. In a MOB situation it could be a mate, an engineer and an AB.

Vessel must be stop prior allowing the rescue boat to approach for recovery, to avoid this incident
if you have a MOB then depending on the ship stopping distance and times are measured in miles an minutes. If you wait until the ship has stopped to launch the FRC then the MOB could already be dead or lost

Well THAT did'nt go well now did it ?
Love how the guy is calmly holding a cigarette (and commenting on?) at the right side on 0:21s ;)
Gotta love the clown continuing his smoke during what should have been a rescue mission. Hold on there buddy we have to get the cameras for a snap shot.
Why didnt they use a painter?
Two mistakes, secondly: not releasing the falls when they began to broach, but first before that, there was not enough tension on the sea painter to keep the bow of the rescue boat into the sea instead of falling off and broaching.
The hell are they still underway when they're launching it?!

How do you know he didn't react quickly to the man over board situation? The video shows he put the ship in reverse to stop.
Thanks for taking all the fun out of it...
The hook on the after line must have jammed shut because it's supposed to unlatch automatically.
If they made the hook ready for off load release.
He didn't do a turning manoeuvre at all at 1.46, if you look at the wake the ship is still on a straight course. The reason that the stern went away is just the effect of the props when pulling astern. It is qiute dangerous to do a turn when that boat is still very close to the ship and connected. I don't now where that captain took his license, maybe by phone!
Sorry for my bad english.
Seems to me they should have lifted the main drop cable, after failiar to release
Could it have been a Video on what NOT to do? If so, they Scored an A++ :)
First of there is no painter line on that boat just some bloke holding a peice of string ?? so that's the result you will always get from a moving ship ,second why did the winch man not lift as soon as he seen the boat in trouble.
They did stop the ship, but it is not a car you can do an emergncy stop with, it probably takes half a mile to stop something of that tonnage!
Consequently, the crew ran to the port side of the boat and launched their other FRC and lost 3 more guys because obviously the last thing that boat should be doing right now is slowing down. I'm still laughing :)

Wrong. They didn't have a proper painter. This will keep the FRCs head to sea, unhook the single lift, then let go the painter - last.
I do not want to come off as an armchair quarterback but I did not hear a "Man Overboard" Signal. There should have been three prolong blasts on the ship’s electrical bell and ship’s whistle. The crew of the boat is Thai. This video is typical of the way a lot of things go down in Thailand. It's amazing anything gets done at all in that country, but it is a great place to party and eat.
chunkEcheez It would have happened with every undrilled crew. This is not a thai problem, it's a knowledge problem.
Number one mistake, no sea painter. A bow line should ALWAYS be lead well forward and secured to the ship released only by the boat crew when ready. The guy in the red coverall "tending" the bow line had no clue, he let that boat get away from him.

now they need that boat to rescue the over-board men but the engine is flooded
They don't like those guys that much.
How many men died ?
yes ru ryt man y they dont eject a capsule life raft
the Captain and the Crew are lack of knowledge in launching fast rescue boat, and it is not advisable to launch while the Ships is Underway..Man overboard tuloy..
Release Hook Fail
NONONO!!!!! The water
why arent they stoping
Hei kamandag0827! those also not the indonesians or malaysians!..u can hear the officer speaking language..its thailand! its very big difference!
They seem to have life jackets at very least
@kamandag0827 sorry about that !…
I was not aiming at Filipinos in particular.
I just meant underpaid, badly trained (if ever), overexploited poor men from third world countries
the could pulled the onload the release and the boat would be free.
during the Williamson turn or what I assume was one, they were turning the wrong way. Yea lets kick the stern over to the side where people could be!
The last guy on the boat was like I aint going in that wter
No,Risk assessment,& Planning.
how to launch a F.S.C. Fast Sinking Craft!!!!!! LMFAO
Good job brother
First thing after an incident like this stop machines immediately.
Они живы ?

always release the giggly pin rather than the whirly bolt...this was a happening waiting to accident...

I'm never saw this kind of disaster in my life! totally fail procedure for launch FRB, this is lack training and lack knowledge about offshore vessel's. In seismic ships is normal this operations is daily operations.
Use a proper painter, unhook from the fall, let go the painter LAST, then this can't happen.
and the man holding the bow line should have pulled in slack to keep bow in to ship.
Ones just smoking a casual cigarette not even arsed
There is one one person on the straboard wing who is still smoking while watching his crew member suffer!how can he act quickly his busy doing his shit!
This is why you release the bowline LAST!
Who rescues the rescue boat? -Pagliacci
Man Overboard...Launch the Rescure Craft. Oh wait...
Launch another FRC to rescue the other FRC boys.
" Hello Engine Room ALL STOP MAN OVER BOARD "
HE SHOULD HAVE DROPPED THE MOB LIFEBUOY ALSO
THE CONNECTION BTWN THE BAOT AND THE SHIP SHOULD HAVE BEEN DETACHED
Never heard of a painter line? Total disaster.
captain to crew; launch the 2nd fast rescue craft..
