    A video on cathing would be great, but I think it would help to destigmatize it, show the benefits, go over different types, dealing with insurance, what to expect, etc. I thought I had mixed incontinence from a back injury, but it ended up being overflow incontinence from a neurogenic bladder. My first Uro set up a cathing appointment and it was terrifying. It is a big life change, and I think urologists often forget that. I found a new Urologist (who is awesome), and she took the time to help me get comfortable with starting a bladder management routine. It has improved my quality of life considerably, and I've been able to go from depends to guards for the occasional accident. Cathing doesn't take anymore time than regularly going to the bathroom once you get the hang of it

      Thanks for raising the cath question Jeff. I have very similar concerns about painful and difficult pre-op Foley catheter placement. I recently learned that the male urethra is about 8 to 9 inches long, nearly 3x that of females, and then we have that pesky enlarged prostate to navigate through at the far end. Much greater chance for pain and even damage to occur. My surgeon told me not to worry, the Foley will be placed after my sedation. That did not comfort me. Now I could be a helpless victim of multiple failed insertion attempts! 😳 Doc, can you please put my concerns in perspective? What are the risks of pre-op Foley cath problems? Is there a standard process every hospital follows to prevent post-op pain from catheters? Thank you for your informative content. You're a gem.

      @@robertlarson8370 If the catheter is going to be left in post operation, ask your surgeon to use a duette catheter. They have two balloons and irritate the bladder less than a traditional catheter. Also invest in guards or disposable underwear. Bypassing happens, and adding to the embarrassment of adjusting to a drainage bag is the sudden realization that you may have an accident anytime you sit up or move from bypassing. If the Foley is just for surgery, don't worry to much about it. They have to monitor fluid output and meds sometimes make the bladder signals not work. You don't want it to get distended.

      Found out today that i have a new lesion on my brain, possibly caused my storage of old urine in the kidneys. Ive gotta learn how to do catheters, and possibly have botox in my bladder. Eeeek! Oh i have multiple sclerosis btw. Ahh not looking forward to this!

    Finally got mine done here in Toronto what a relieve.i only get 200 cc due to my other botox injections for stroke in my arm and leg.wish they told us about this while I was in the hospital 8 years ago.its a process but worth it
    I would suggest a flexible scope.the rigid scope is not for the weak and I am strong😢.up here its done in the hospital..literally a 10 minute procedure but 1 hour to get ready

    I have this procedure at least three times. I have it at a surgery center or the hospital outpatient procedure. It works. I go from 13 times to 4.

    Please make a video about your first four years of college and then premed and then residency at Etc. Would like to learn more about your journey and why you chose urology. Thank you. My father survived prostate cancer but lost all quality of life due to what his surgeon did to him. I’m averaging a PSA a round 24 every six months and I have chosen quality of life. Thanks for your videos I enjoy them very much. You have a beautiful smile by the way.

    Unfortunately the Botox did not work on my bladder what's been effective is avoiding a lot of sugar. That's been the most effective

    I’m about to have this done, and thankfully they are gonna knock me out because they tried to do a cystoscopy yesterday and I couldn’t do it awake it hurt too much.

    Dr. Rena are you suggesting that if a patient that has unsuccessful botox procedure needs to have a catheter for the rest of his life? Or would this be temporary?

    Been suggested to try this next! Diagnosed with mix incontinence. I think it is caused by a combination of injuries and/or fibromyalgia 🤔

      Mostly. You usually have to demonstrate failed drug trials with anticholinergics

    Are there any risk factors that let you know in advance if you are at high risk for not being able to urinate after Botox and needing to use a catheter? Also, does The need to use a catheter also last six months? Until the Botox wears off?

    Had two TURPs. Passage seems open but Still problems. My spinal nerves are all suffering. So this Botox could be a solution.

    Dr. I am interested in non invasive help ngb ms. I dont eant botox or indwelling cath. I was just in cosultation with urological surgeon who strongly promoted suprapubic cath..with bag or valve.(uk)
    No thank you. Will stay with pads depends. Serious frequency incontinence. Mybetriq didnt help. Any suggestions. Dont believe botox sounds good but will find out more..ty

    I have Neurogenic Bladder and I have Botox every 4-6 months. I have a Suprapubic Catheter and Botox is amazing. The bladder spasms get reduced drastically.
    For me they put me under and they also change my catheter at the same time. This video is so very informative and I thank you so very much for making it.

      So every 6month you would take Botox?? And with anesthesia? Anesthesia will affect your health na? Can Botox fix OAB?

      @@shikhasoni5518 I actually had to move it to every 3-4 months because of the nerve damage I have. Anesthesia doesn't really affect my health. I don't know if it will fix OAB but it does help.

    It caused retention, now I have chronic UTI. But it does work, my Doc did it in the OR because after the PC I often grow scar tissue that can close my bladder completely.

    My Urologist suggested interstim over Botox. Her reason was in 20% of patients, the opposite problem of frequent urination occurs and you can't pee, then self catheterization is required.

    My mom isn't able to RETAIN any liquid in her bladder due to it being damaged and the pelvic muscles not working. Her doctor recommended botox, but this seems like the wrong direction for her since her issues isn't OA but something else?

    I dunno about that. I love Botox for cosmetic reasons but AZO messed me up temporarily and I had to quit taking that. Made it where I felt like I had a uti for a day.

    Anyhow, ISC was no real problem for me, using Vapro-Pocket 16ch.
    Diagnoses are NDO and DSD.

    Botox has been great for my neurogenic bladder/IC but the effect is wearing off quickly at the 2 month point. Is that unusual? I received 200 units

    Thank you, your presentation was helpful. I am scheduled to have botox in the OR under sedation soon, because the Interstim implanted a month ago has not solved the issue of urge incontinence, even though several programs and settings have been tried. Although, the implant has lessened the amount of incontinence supplies to some extent, it has not made any significant change in quality of life. Certainly not to the extent Metronics likes to brag about it doing. If botox fails to result in improvement, the next option will be to wear a bag, and by that, I don't mean my favourite Coach or Fossil...lol The reason I did not do botox first was because of the issue of possibly needing to use a catheter, but my Urologist says that happens far less than the medical papers say it does, in her experience it happens in maybe 1% of patients. Wish I had known that earlier. A pessary has also been provided, which seems to to help when laughing, coughing or sneezing. Urge incontinence is still causes major anxiety and distress.

      Hello how you doing with the Botox treatment
      Mi e fail I got urine retention for 6 months and before the treatment I didn’t have the urgency incontinencia they did Botox treatment for intersticial cistitis and pain but I got worse now I wear diapers every day no catheter any more
      I’m trying now the interstim to see how it goes
      I do have chronic UTI even before the treatments
      What can a bag they will add to you?if this don’t work ?

      @@IsabelRodriguez-dd6tt Like everything else I have tried, Botox made little if any difference. I have also had Bulkamid for stress incontinence, not much difference after that either. Obviously having my bladder radiated has not helped. I see the Urologist in a year for the Interstim battery to be checked. Other than that, losing more weight would mean less pressure on my bladder, so aways working on that. I will never be convinced to have an intrapubic catheter. I continue to manage if and when I drink/eliminate liquids, but even Interestim does not help prevent some sudden accidents. I had some improvement after the new Urologist reprogrammed my Interstim using custom programmes, previous Urologist never attempted to do that. I don't think it is common knowledge that there are custom programmes in addition to the ones one is sent home with. And, if they are aware, instead of actually trying each one, the patient is just told to try this, or that, which is less than helpful. There is nothing else the Urologist can offer me, other than booster Bulkamid shots after weight loss. It is what it is! I hope your results are far better than mine.

    A video on catheters would be appreciated. 67 yr old with MS. Diagnosed with neurogenic bladder. Reluctantly agreed to Botox a year ago. I feared it would result in needing to catheterize. It did and use a catheter for every urination. 6-10 a day. I no longer have accidents so good in that sense. But I carry a few catheters and lube packets with me when ever I leave the house. Post Covid I’m hoping to do more traveling and thinking about how I will manage urination on the road.

    i have been offered this but im not sure its the right procedure for me ? i thought my TVT tape was failing after 15 years in place... so saw my GP... she sent me back to hospital... i had a urodiagnostics? test ... they say i had a very small leakagr but tvt tape was doing its job...but felt i might have an inflamed bladder ?... so was expecting to get meds to stop the inflamation? but got a call from the urology dept offering me bladder botox or 12 weeks of ankle acupuncture... so not sure what to do as i often have to self catheterise anyway as sometimes i dont feel my bladder has fully emptied ? and i get bladder pain when i need to go

      It has side effects I got urine retention and over active bladder with the treatment and I cath for 6 months

    Hi I have to catherterise 6/7 times a day. As I don't empty my bladder properly due to radiotherapy. I have nerve damage to and dont get the sensation to go for a wee.I also wear a Diveen and pads and I still have accidents and wet myself. Should I and can I have botox in the bladder 😥

    Interesting. About to undergo TURP and the doc said it would probably not stop the urgency and accidents, but at least I'd be able to empty my bladder. If that's the case, then this would be my go to. I've catheterized myself before after a TUMT made me unable to urinate, and I've used used sounds which are a lot of fun.
    I don't want to keep wearing these effing pads! I hate them.

      That’s what I’m afraid of… it paralyzes the muscle to where you can’t urinate 😫

    I got my first procedure done in office but it was so painful! So I got sedated when I got it this previous time . It was horrific being conscious 😩

      Sounds like having a uterine biopsy in the office with no sedation. The worst possible pain ever. So, when it comes to shoving cameras and other tools up my undercarriage, I'd recommend the brown juice everytime. Total bliss when everything goes quiet and peaceful, and you don't wake up until it is over!

    when I was pregnant I had a 3 rd degree laceration during delivery. No problems until after 65. Now I have fecal incontinence with diarrhea and end up with UTI. Would Botox help with tightening the anal spincter?

    Done bladder Botox 2 weeks ago now finally removed the catheter today so I’m not sure if this normal after removed the catheter I’m still pee my pants 👖 I’m wearing pad n nappy,nurse said do self catheler but how can I do it if I’m still pee every 5 min 😢what’s the point to do in and out catheler when I’m still pee?Is this normal?does it go away soon?please someone help ??

    But can we talk about how if after your Botox you have to rely on a catheter I get constant Utis so bad that my doctor just put me on a daily antibiotic to keep the uti symptoms away . My biggest fear is antibiotic immunity or constant uti and losing bladder control and having to go back to wearing diapers

    Hi Dr. Rena, can i ask you one thing about bladder? some time when i'm about to sleep or after i poop or after i have masturbate, some time have the sensation to pee, not always but from time to time, as not all time have this sensation i didn't go to consult my family doctor, but is this can be related as i'm overweight and my fat can press on the bladder a stimulate to go to bathroom? as the sensation it feel it come from the tip of my penis. do you think i need to consult an urologist or as not chronocaly didn't need?

    Does botox help with capacity? Had a rigid cystoscopy with hydrodistension couldn't take more than 400ml under gen.anae my urologist was a perturbed on that finding capacity normally 120/150ml has come up to bout 200/250ml for the minute worried bladder augmentation could be only way fwd

      @shikhasoni5518 cathing once you get used to it is fine at worst now I find it inconvenient I am on the waiting list for surgery, like any surgery there wil be pain to be managed after everyone is different. It's 10days min in hospital so in the right place for pain relief

    This procedure paralized my bladder for over 1 year and had to cath myself during that time. It left constant pain that I have to live with now....do not have this procedure done!!

      First thought that entered my mind was if it would cause permanent weakness leading to retention which could be further exacerbated by bladder outlet obstruction such as with BPH. It seems after paralyzing the muscles they would not return, so I don't understand why it "wears off" in 6 months. Peace.

      Hello, could you please give me your e-mail ? I did the Botox in July and until now the bladder is always paralyzed. I have to Cath myself every day. The doctor tries to deny that it is due to the Botox. I don't know how to do. Will my bladder return to normal one day ? I am pessimistic. I would be grateful if you could give me some advice.     Somewhat related question. ..
    50yr old male here. I have noticed a difference in stream pressure when urinating. What causes this?
    Typically in the morning with an extremely full bladder (and sometimes after waking up with an erection that has subsided somewhat) the stream pressure is very weak. At other times - normal mid day trip to the toilet, the stream pressure is very strong.
    Shouldn’t an extremely full bladder equate to a high pressure stream?

      See a urologist. I have had similar problems and had at 50 bph with different strengths of urination. A drug called Hytrin or rhe generic terazosin can help relax the offending parts. It's a crappy blood pressure med that works for bph. I've taken it for years. It helps a lot.
      Undergoing TURP next week hopefully. Long as no one gets Covid. Fingers crossed!

    bladder muscle contractility gets affected by detrusor muscle problems as well u should work on treating that instead of making it paralysed and to save the kidneys one should do intermittent catheter
