I love the last risk, I absolutely feel this sometimes when others try to use their situation and use of EBM almost condescendingly as it makes me feel I have more shortcomings than I thought as a clinician. It is so variable though.

I am using this to teach pre-hospital responders that medicine changes, always. In my programs it was important to me that residents understood that the ground they stood upon, on the shoulders of those who went before demanded that they take the next step. As as framework EBM helps direct those thoughts and channel those responses .. Thanks.

Right now, however, there is not quality standard for FOAMed resources, so it's a bit of trial and error. If you're in Emergency Medicine, I'd recommend theSGEM.com - I've done some work with Ken and he's quite thorough.
if it's any consolation, ioannidis--a statistic professor at standford--says around 80% of medical studies are bullshit. (and by bullshit, i mean any of these things: 1) study has conflict of interest with funding, 2) shitty methodology, 3) unwarranted conclusions, 4) fasle positivies, etc.
Excellent questions. Regarding the 'best evidence' component of EBM, I would suggest that all research is relevant, but needs to be considered separately (re: generalizability, quality, etc.) and then a judgment made. In terms of balancing the three components of EBM, I would say "it depends". When the evidence is weak and my experience is limited, I put more weight to what the patient wants. This changes if there is strong evidence, or I've had relevant experiences... I hope that helps!
