This is one of the most informative videos! I am so happy to hear of the recent advances in spinal surgery. I have psoriatic arthritis, nearly no disc left L4/L5, and moderate to severe foramenal stenosis. Getting injections every 9 months at L4/L5/S1 for several years. Just had bilateral facet nerve ablation at L3/L4/L5. For the 1st time, my pain management doctor mentioned consulting with a surgeon. My mom has multilevel fusion cervical and lumbar. The lumbar was not very successful, and she's now in a wheelchair most of the time. I have a copy of her x-rays and tell every doctor, "I do not want to be like this."

Fascinating to see what is new and current in stenosis treatments and surgery. My former husband has had several procedures done over the years on his lower back. Stenosis is getting bad again in the past year, so he is working with his surgeon again to find a solution for him. I referred him to watching this video. I believe it will give him hope. He sees his surgeon again for next steps in a couple days. The timing of this video coming up is perfect. I feel so badly for people, many of whom have commented here, who suffer from severe chronic back pain. We live in faith and hope!! Thank you for being part of that faith and hope.

This is awesome. I am fused with instrumentation from T2 to L2 for idiopathic roto scoliosis and now experiencing issues with L5 and 4. Currently receive epidural that wears off in 2 months and I can only get them every 2 months. This video is very exciting to me and gives me information to talk with my doctor. Spent several hours watching this video. Learned a lot. Got excited. Looked up my Tricare insurance which Humana insurance used to cover this group. They no longer consider Holy Cross as an acceptable in program and dropped them for Tricare patients. That is a bummer for me. So the search goes on for a doctor that I could trust.
This has been incredibly informative. Your approach and manner in which you describe the positives and potential negatives of each procedure is amazing. As someone dealing with L4/5 S1 issues in the last 5 years and never a day before, the dilemma as you posed it is spectacular. What is the cost of doing nothing compared to the potential benefits and risks of doing something. At 57 and 6'9" the stressors on my spine are I am assuming somewhat unique compared to someone of average height simply from the standpoint of stress placed on the spine. The consults I have had seemed too willing to move forward with surgery and the phrase "minimally-invasive" was not a term that was uttered. Having your eyes on an MRI would be invaluable or a consult. Thank you!
I was diagnosed with spinal stenosis almost 10 years ago, along with severe facet joint arthritis. I had open back surgery - a laminectomy, no spinal fusion. I am 75 years old, play pickleball and golf. Before I had the surgery I got to the point where I could barely walk. I still have some back pain but it’s not debilitating. My surgeon, Charles Kanos, lives in Greenville SC. As far as I’m concerned, he’s one of the best in the country. A life saver.

I have recently received the L5-S1 LSS diagnosis, and have two months to learn all I can about surgeries, procedures, osteoporosis and LSS, and aggressive physical therapy. That's the new-patients-get-in-line wait, and I am grateful for the time to learn, and to see what relief PT can give me in the meantime. I will have to watch this video again, though, as this is all new to me. Again my thanks for your frank approach with hope attached. I’ve had 2 failed surgeries on cervical spine and then started have major issues with lumbar area, spinal stenosis etc. I don’t want anymore fusions. My last neurosurgeon told me I needed fusion surgery and when I told him i wasn’t ready for anymore spinal surgeries after 2 failed surgeries….his answer was “when you can’t walk anymore go to the hospital and call me, I’ll do surgery. So many doctors just like to practice their skills and we are the guinea pigs. Thank you Dr for looking for other ways to help relieve the pain we are in without major surgery.
I had a partial discectomy and a laminectomy and it changed my life. No fusion. Going on about 20 years and I have been free of pain. I know I'm lucky, though.

I had some rather severe sciatica due to spinal stenosis. It lasted for about 5 years. Then I found a chiropractor by chance who actually was involved as a young man in the development of decompression therapy. He assured me that if I committed to 24 treatments in a timeframe set up by him that he could solve my problem. Nearing the end of treatment I felt better but not completely cured. After 24 treatments though, he told me to continue the exercises he prescribed and wait 1 to 2 weeks. After that, no pain. That was a year ago. I see him about every 6 weeks for an adjustment and treatment but I am completely cured. The medical doctors thought surgery would not do much for me, and wanted to start me on injections into my spine. I am extreme lucky and happy I did not do that.
I have spondylolysis. I guess that means 2 small par fractures. They are going to operate on my l5 laminectomy/Gill body resection with l5 s1 fusion.I will have a nuro surgeon spine surgeon in the operating room.Im in severe pain with the nerve in my buttocks n half way down my leg. I am very scared .I went through 6 shots in my back along with PT for six weeks..I hope I am making right decision. Your video is awesome and so well explained. Very Interesting I’ve just found out that I have Lumbar Stenosis and my L5 is causing some Nero. Problems like sciatica. So when I see a doctor I’ll have some clue what he or she is talking about. Thanks for the podcast! Steve
@@swimfit57 the pool helps me so much, I use a waist belt and 2 noodles (1 under my arms and neck, the other under my knees) and relax with meditation and breathing exercises, it is like zero gravity and let’s every thing relax. Best if you could get to a pool that’s not too busy. Hope this helps.💦
I have been watching this video for the last two hours. I have been afraid of spinal surgery. This gives me hope. I am 87 and been diagnosed with lumbar spinal stenosis. Have reached the point where my right leg drags, I have trouble sleeping because of the pain. I have limited confidence in the spinal surgeons in my city. I would take the risk with this Doctor.
Thank you for a really interesting video, painting the picture as it really is. I just had a second MRI scan (the last one was three years ago) and my consultant is going to call me when he has the results. Depending on the MRI results, I will either be offered pain management or surgery. It's a tough decision, but I am finding it painful to walk long distances and it's sink or swim. Whatever option is suggested, it's a gamble. I can't get to sunny Florida, I am in the UK and I'm reliant on my local hospital. Also, this doctor produced an educational video. He can't help you, your mother, husband or pet dog. He gave generic info.
Hello doctor, please help me. My husband is sick. He was involved in an accident and became paralyzed from the waist down for a year and a half. The doctor said he needs physical therapy abroad, but I don’t have the money for treatment. He has been bedridden for a year and a half now. The doctor said his spinal cord is healthy, but there is a bruise, but vertebrae 11 and 12 are separated and he did a spinal stabilization. However, he has pain in the thumbs of both hands and he has no strength in his hands. Please help me. He also has small children. Please, in the name of what you like, help me. How can I reach you to treat my husband? Find someone to help me examine him, please.
My wife has had spine problems between her shoulder blades. very limited on things she can do. Some times a lot of pain. This area doctors won't touch. 30 plus years of pain. Maybe some day if she can live long enough. Have enjoyed the video.

Can this be done if you have Steffe plates and fusion done on the low back, Had 5 back surgeries and Still dealing with Chronic pain, It's a terrible way to have to live and then you have some doctors so scared to prescribe any type of opioids that'll help they just let you suffer and they want you to do therapy, Well my last surgery was in 23 and during therapy I tore my Back Muscle, The hell with therapy I don't recommend it, The torn muscle has taken more than a year to heal and it feels about as bad as my surgery! Something has to get better to help people that have bad backs, due to accidents or from birth!
It's hard to find a good back surgeon in Ohio, Good Luck! I had a good one and he retired Dr.Miely out of Riverside Hospital, He got me 13 more years of being able to work before it went bad, I ended up with Arachnoiditis! Good Luck!!!!
Oh, my Lord, you have described me down to. I don’t know how to put it. It’s making me cry. Nobody has ever shown me all this information before I went to a neurosurgeon who wanted to put a rod, a plate pens, all kinds of crap in my lower back. She said it would be very painful. It would be a year recovery at least I don’t want that I want and I want something that works. Oh gosh, I don’t know how I would ever get down to you that cost money of course but I would love to come down to youand see if there was a way you could help me.
So, in other words, we still can’t fix my back, I live in Florida, moved from California. I’m 64 in good , owned a health food store for 20 years. I have had two back surgeries in two years. One to help a nerve root, second one to remove scar tissue. I am in so much pain. I’m ready to check out.
@@juanitacoleman-l6v I to am in great pain and one of the best forms of pain relief is going to a quiet pool. I bought a flotation belt for around my wast and 2 good noodles, 1 for my arms the other behind my knees, it’s like being in space with zero gravity (almost to me that is). I just relax, do some gentle breathing methods or even meditation. I did not realize how good this would make me feel until the pool I go to had closed down for 3 months. I’m 65 and was told that what I had done was the best they could do. So working with the pain clinic and SCS is on the table, I had the trial unit for 15 days and I’ll rethink that decision after I get back to where I was going to the pool on a regular schedule. Easy for me to say but stay positive, keep learning and don’t give up Juanita, new methods and technology is always in the works as this is such a common health issue. A.

I want to play Squash again having been restricted by SS over the past 2 months. Great information! I have a cervical and spinal stenosis plus a straight spine. I was told I needed spine surgery for pain relief. Also physical therapy & a chiropractor were suggested which I refused.
If you can, I would definitely go to physical therapy and chiropractor -- I do both and am getting relief in my lumbar spine through decompression and laser along with chiropractic manipulation which reduces the mis-alignment especially in my L4/L5. (My chiropractors are not the old school crack your back and push you out the door.) PT has helped with increasing muscle strength (prior to possible hip replacement to make recovery easier and faster) and improving my upper body posture. All the best to you.
I 100% agree with the issues down the road. Laminectomy is great for a few years, but it takes away your overall stability of your spine. Why not a tlif or the disc repair like Dr. Yeung and Dr. perform?
What has your experience as a physician proven to you about Medical Insurance coverage on this semi invasive procedure? Is it less expensive? What about healing ? How long is recovery for this type of surgery? Thank you!
Interesting… may issue is aggravated by sitting and find relieve when standing but no long term. I had a lamindectomy L4/L5 11 years ago. Some relief but seems to be worsening.
Hello. I have been diagnosed with moderate stenosis. The MRI of your patient with the pinched area looks like my MRI. I don't have the pain down the legs- I will get occasional pinching sensation and cramps. I have trouble with urination and constipation. Sitting is more comfortable than standing or walking. I have seen two nuerosurgeons who want to fuse 3 disc. I would like to know more about the approach you use with these 3 devices. Also, what can you tell me about a TOPS device? I live in Wisconsin, so coming to you may not be possible. If the insurance company would let me, I would. One last thing, I had a disectamy years ago. I am told that the disc now moves side to side. Would any of the devices you mentioned work for me?
Hi, Thank you for for this fantastic video. I had lumbar fusion surgery 9 years ago in OKC, OK. 1 month after surgery I came down with sepsis. 3 more surgeries to remove the titanium and clean/out. Another 2 weeks sepsis was back , 1 last clean out surgery for a total of 5 ! Was on anti-biotic for 1year. I asked my doctor for people who could tell me about their experiences, got a lot of ''yeah, you betchas''', but nothing, no calls, no phone #'s. A video like this one would have gone very far to let me know what was going to happen. As usual in medicine the patient is never on the ''need to know'' list.

What about ADR? Sounds like most states are performing the procedure (single & multi level) with extremely good results. I personally know a few people with lumbar ADR and they are back to sports and activities they had to quit prior to ADR.

I’m 80 yrs old, have had a lumbar laminectomy on L2-3 9 mos ago. Currently Severe pain revealed L5-S1 has opening of only 7mm. A “rod and pins” was the only solution I was presented and because of my age and severe arthritis, my neurosurgeon will not do it. Would the “zip 51” work in this scenario?

Is post surgery calcium/magnesium supplementation recommended? I’ve had 7 back surgeries resulting in a fusion from T-10 through my pelvis (my S-I joints were bolted to my iliac).

I had L5-S1 microdisectomy but my lumbar spine stenosis and degenerative disc disease condition, still causing Piriformis, glute, butt, pain on outside thigh area and sometimes calf pain (all are on left side); possibly from compression of the narrowing cannal. Will the "minuteman" help? I prefer not to go through radiofrequency ablation which are advised to me. Thanks Doc
For facetogenic pain, minuteman is not the right tool. One should first proceed with diagnostic blocks of the facet joints, to establish a diagnosis. Ablation is by any measure, far less invasive than any spine surgery, lower risk, and around 70% of the time quite effective. Establish the diagnosis. If the ablations fail, then consider further more aggressive options.
@wportermcroberts Thank you Dr.Robert. I am hesitate to do RFA since it isn't permanent and I have bad pain experience with Epidural injection. My latest MRI doesn't show herniated disc but I still have back and leg pain when I walk, numbness when sit too long. Sometimes I feel my spine open up (crack) when I bring my abdominal and hips forward and the pain almost immediately gone. My friend had artificial lumbar disc replacement due to discogenic and he is now pain free over 2 years.

My Neurologist Says I May be a candidate for the MILD procedure- There is only a shaving, no apparatus correct? 61 year old Normal weight female. Spinal stenosis / they said it’s bad. I do not want a Metal apparatus In my back. I do not have osteoporosis / I am also a SLE pt. Lupus / thoughts? I do walk three times a day. Each walk is around a mile. I do at times have a leg go numb after sitting In the restroom.

Amazing information presented clearly! It would be nice to see the post surgery MRI of that patient.
I have some. But typically they do pretty well so an MRI is not indicated (and not paid for by insurance).
I am crushed that I did not see this before my LS 2 years ago Jan.

Very Interesting I’ve just found out that I have Lumbar Stenosis and my L5 is causing some Nero. Problems like sciatica. So when I see a doctor I’ll have some clue what he or she is talking about. Thanks for the podcast! Steve
@@swimfit57 the pool helps me so much, I use a waist belt and 2 noodles (1 under my arms and neck, the other under my knees) and relax with meditation and breathing exercises, it is like zero gravity and let’s every thing relax.
Best if you could get to a pool that’s not too busy.

I have been watching this video for the last two hours. I have been afraid of spinal surgery. This gives me hope. I am 87 and been diagnosed with lumbar spinal stenosis. Have reached the point where my right leg drags, I have trouble sleeping because of the pain. I have limited confidence in the spinal surgeons in my city. I would take the risk with this Doctor.

I just had a second MRI scan (the last one was three years ago) and my consultant is going to call me when he has the results. Depending on the MRI results, I will either be offered pain management or surgery. It's a tough decision, but I am finding it painful to walk long distances and it's sink or swim. Whatever option is suggested, it's a gamble.
I can't get to sunny Florida, I am in the UK and I'm reliant on my local hospital.
Also, this doctor produced an educational video. He can't help you, your mother, husband or pet dog. He gave generic info.

Hello doctor, please help me. My husband is sick. He was involved in an accident and became paralyzed from the waist down for a year and a half. The doctor said he needs physical therapy abroad, but I don’t have the money for treatment. He has been bedridden for a year and a half now. The doctor said his spinal cord is healthy, but there is a bruise, but vertebrae 11 and 12 are separated and he did a spinal stabilization. However, he has pain in the thumbs of both hands and he has no strength in his hands. Please help me. He also has small children. Please, in the name of what you like, help me. How can I reach you to treat my husband? Find someone to help me examine him, please.
My wife has had spine problems between her shoulder blades. very limited on things she can do. Some times a lot of pain. This area doctors won't touch. 30 plus years of pain.

Can this be done if you have Steffe plates and fusion done on the low back, Had 5 back surgeries and Still dealing with Chronic pain, It's a terrible way to have to live and then you have some doctors so scared to prescribe any type of opioids that'll help they just let you suffer and they want you to do therapy, Well my last surgery was in 23 and during therapy I tore my Back Muscle, The hell with therapy I don't recommend it, The torn muscle has taken more than a year to heal and it feels about as bad as my surgery! Something has to get better to help people that have bad backs, due to accidents or from birth!
I need to find a good surgeon in NE Ohio I have had a neck fusion, and two lower back surgeries last was a lower disc laminectomy.

I had a good one and he retired Dr.Miely out of Riverside Hospital, He got me 13 more years of being able to work before it went bad, I ended up with Arachnoiditis! Good Luck!!!!
Oh, my Lord, you have described me down to. I don’t know how to put it. It’s making me cry. Nobody has ever shown me all this information before I went to a neurosurgeon who wanted to put a rod, a plate pens, all kinds of crap in my lower back. She said it would be very painful. It would be a year recovery at least I don’t want that I want and I want something that works. Oh gosh, I don’t know how I would ever get down to you that cost money of course but I would love to come down to youand see if there was a way you could help me.
So, in other words, we still can’t fix my back, I live in Florida, moved from California. I’m 64 in good , owned a health food store for 20 years. I have had two back surgeries in two years. One to help a nerve root, second one to remove scar tissue. I am in so much pain. I’m ready to check out.

I bought a flotation belt for around my wast and 2 good noodles, 1 for my arms the other behind my knees, it’s like being in space with zero gravity (almost to me that is). I just relax, do some gentle breathing methods or even meditation.
I did not realize how good this would make me feel until the pool I go to had closed down for 3 months.
I’m 65 and was told that what I had done was the best they could do.
So working with the pain clinic and SCS is on the table, I had the trial unit for 15 days and I’ll rethink that decision after I get back to where I was going to the pool on a regular schedule.
Easy for me to say but stay positive, keep learning and don’t give up Juanita, new methods and technology is always in the works as this is such a common health issue.

Hard cenent with hard adhesive should replace screws..metal scres are dangerous..

Great information! I have a cervical and spinal stenosis plus a straight spine. I was told I needed spine surgery for pain relief. Also physical therapy & a chiropractor were suggested which I refused.
If you can, I would definitely go to physical therapy and chiropractor -- I do both and am getting relief in my lumbar spine through decompression and laser along with chiropractic manipulation which reduces the mis-alignment especially in my L4/L5. (My chiropractors are not the old school crack your back and push you out the door.) PT has helped with increasing muscle strength (prior to possible hip replacement to make recovery easier and faster) and improving my upper body posture. All the best to you.

Does the installation of those inter-spinal devices make a patient ineligible for future MRI scans of hips, etc.?
I don't know of any that would cause contraindication of future MRI of the extremities.
I 100% agree with the issues down the road. Laminectomy is great for a few years, but it takes away your overall stability of your spine. Why not a tlif or the disc repair like Dr. Yeung and Dr. perform?

Interesting… may issue is aggravated by sitting and find relieve when standing but no long term. I had a lamindectomy L4/L5 11 years ago. Some relief but seems to be worsening.
Hello. I have been diagnosed with moderate stenosis. The MRI of your patient with the pinched area looks like my MRI. I don't have the pain down the legs- I will get occasional pinching sensation and cramps. I have trouble with urination and constipation. Sitting is more comfortable than standing or walking. I have seen two nuerosurgeons who want to fuse 3 disc. I would like to know more about the approach you use with these 3 devices. Also, what can you tell me about a TOPS device? I live in Wisconsin, so coming to you may not be possible. If the insurance company would let me, I would. One last thing, I had a disectamy years ago. I am told that the disc now moves side to side. Would any of the devices you mentioned work for me?

Hi, Thank you for for this fantastic video. I had lumbar fusion surgery 9 years ago in OKC, OK. 1 month after surgery I came down with sepsis. 3 more surgeries to remove the titanium and clean/out. Another 2 weeks sepsis was back , 1 last clean out surgery for a total of 5 ! Was on anti-biotic for 1year. I asked my doctor for people who could tell me about their experiences, got a lot of ''yeah, you betchas''', but nothing, no calls, no phone #'s. A video like this one would have gone very far to let me know what was going to happen. As usual in medicine the patient is never on the ''need to know'' list.
How does Coflex compare to flospine? Is Flowspine also essentially a fusion? Coflex seems to still allow movement? Any experience with this?

Are these procedures doable if you have already had laminates ?
What about ADR? Sounds like most states are performing the procedure (single & multi level) with extremely good results. I personally know a few people with lumbar ADR and they are back to sports and activities they had to quit prior to ADR.

I’m 80 yrs old, have had a lumbar laminectomy on L2-3 9 mos ago. Currently Severe pain revealed L5-S1 has opening of only 7mm. A “rod and pins” was the only solution I was presented and because of my age and severe arthritis, my neurosurgeon will not do it. Would the “zip 51” work in this scenario?
Detailed imaging such as MRI or CT would add a lot to the conversation.

I have spinal stenosis and a curve in my lumbar area. What options do I have? I am currently receive steroid injections into the side of my spine.

osteoporosis is not a significant contraindication to MILD.

Is post surgery calcium/magnesium supplementation recommended? I’ve had 7 back surgeries resulting in a fusion from T-10 through my pelvis (my S-I joints were bolted to my iliac).
What about neck stenosis? The doctors in our area in Alberta Canada won’t try anything. So my husband suffers 24/7.

I had L5-S1 microdisectomy but my lumbar spine stenosis and degenerative disc disease condition, still causing Piriformis, glute, butt, pain on outside thigh area and sometimes calf pain (all are on left side); possibly from compression of the narrowing cannal. Will the "minuteman" help? I prefer not to go through radiofrequency ablation which are advised to me. Thanks Doc
For facetogenic pain, minuteman is not the right tool. One should first proceed with diagnostic blocks of the facet joints, to establish a diagnosis. Ablation is by any measure, far less invasive than any spine surgery, lower risk, and around 70% of the time quite effective. Establish the diagnosis. If the ablations fail, then consider further more aggressive options.
@wportermcroberts Thank you Dr.Robert. I am hesitate to do RFA since it isn't permanent and I have bad pain experience with Epidural injection. My latest MRI doesn't show herniated disc but I still have back and leg pain when I walk, numbness when sit too long. Sometimes I feel my spine open up (crack) when I bring my abdominal and hips forward and the pain almost immediately gone. My friend had artificial lumbar disc replacement due to discogenic and he is now pain free over 2 years.

My Neurologist Says I May be a candidate for the MILD procedure- There is only a shaving, no apparatus correct? 61 year old Normal weight female. Spinal stenosis / they said it’s bad. I do not want a Metal apparatus In my back. I do not have osteoporosis / I am also a SLE pt. Lupus / thoughts? I do walk three times a day. Each walk is around a mile. I do at times have a leg go numb after sitting In the restroom.
