For someone in their mid 30's who isn't seeing the fat loss or excess skin but still has dark circles, how can I tell the issue and the solution? I stare at a computer screen all day for work and I have been told by my peers (aka non-experts) it may be thin skin where you can see the veins underneath, but again, they are not experts so I don't know...

I often notice that celebrities start looking tired in their 40s but they still look beautiful so I couldn't quite understand what this tired look was due to, I only knew it had something to do with the eye area. The more I dive into aesthetic medicine, the more interested I am in becoming an aesthetic nurse! Would love it if you did a summary of how to be an ethical nurse injector and use filler responsibly (how filler can help and hurt, what nurses should do in their scope of practice, and when they should refer out to a physician vs. trying to overcorrect with filler).

    • @dramirkaram
      @dramirkaram  ปีที่แล้ว +29

Thanks for the comment. Here is a summarized answer to your question, hopefully it can guide you in the right direction.
To be an ethical nurse injector and use fillers responsibly, it's important to adhere to the following principles:
Education and Training: Obtain proper education and training in facial anatomy, injection techniques, and the safe use of fillers. Stay updated with continuing education courses and certifications specific to aesthetic nursing.
Scope of Practice: Understand and adhere to your professional scope of practice as a nurse injector. Each country or region has specific guidelines and regulations regarding the procedures nurses are allowed to perform. Familiarize yourself with these regulations and operate within their boundaries.
Informed Consent: Prioritize informed consent by thoroughly discussing the treatment, potential risks and benefits, expected outcomes, and alternative options with the patient. Ensure they have a clear understanding and provide their consent willingly.
Assessment and Consultation: Conduct thorough assessments of the patient's medical history, current health status, and aesthetic concerns. Assess their expectations and suitability for the procedure. Consider factors such as allergies, medical conditions, and medication use that may impact the treatment or indicate the need for referral.
Individualized Treatment: Tailor the treatment plan to the individual patient, taking into account their unique facial anatomy, desired outcomes, and any contraindications. Avoid a one-size-fits-all approach and customize each treatment to the patient's specific needs.
Responsible Use of Fillers: Use fillers judiciously and responsibly. Ensure that the amount and placement of filler are appropriate for the desired outcome while maintaining natural-looking results. Avoid overfilling or overcorrection, as it can lead to an unnatural appearance or potential complications.
Referral to a Physician: Recognize the limits of your expertise and refer patients to a physician or appropriate specialist when necessary. This may include cases that require complex treatments, correction of previous complications, or when the patient's aesthetic concerns are beyond your scope of practice.
Continuing Education and Collaboration: Stay up-to-date with advancements in the field of aesthetics by attending conferences, workshops, and collaborating with other healthcare professionals. Engage in interdisciplinary discussions to enhance patient outcomes and safety.

What I like so much about Dr. Karam is that he’s able to put out these informative videos on a regular basis, while running a successful practice - AND still not look exhausted. It worries me when plastic surgeons with popular TH-cam channels look like they don’t get the rest they need. My reasons are selfish. Basically, it would be impossible for me to have any confidence in a doctor who has both a surgical practice and a booming TH-cam channel, if it means it’s taking a destructive toll on his sleep. I would not want anyone like that going near my face with a knife. Another reason I respect Dr. Karam’s channel as much as I do is because he’s not doing silly skits that (while being entertaining) cause me to wonder whether he would rather be a performer in the entertainment industry than a plastic surgeon. It’s reassuring to see how seriously and confidently Dr. Karam takes his craft. Finally, the before and after photos are “amazing” (such an overused word, and yet it applies perfectly here). Now, if only he wasn’t on the opposite coast from me. It doesn’t seem like the high altitudes on a plane would bode well in the healing process. 😂

Not sure how this video got in my feed but great video!. He's so right about that overfilled filler effect. Years ago, i had it done by a dr that was supposed to be one of the best. Right eye was good, but left eye kept looking puffy. The dr’s office told me it would eventually go down, it never did. One day my guy friend ask me why my left under eye always looked swollen. That comment bothered me so bad that after a year of it looking like that, I found a doctor to dissolve it asap. Went to another dr. To get it done again since my right eye was still looking good, but my left eye was looking gaunt due to the dissolution of the filler. Well he also didn’t know what the hell he was doing and it happened again. Finally found a dr who knows exactly what she’s doing and almost 2 years later, my under eye still looks good. Only issue is in the morning sometimes my under eyes are puffy which I suppose is the filler but after a couple of hours of being upright on my feet, it looks normal again. I’ll take that any day over the hollow Jack O Latern look.

Dr. Amir Karam its not fair that a knowlegeable great surgeon like you only exists in California state .. i'm sure many women that watch you from different states wish you can travel and have surgeries in other places in America..

I took your advice and went to famous Beverly Hills Occuplastic surgeon to get a fat transfer because you were too busy and had a year waiting list. Now I have a dent in my leg where he took the fat and my eyes look horrible. He put 2.5 cc under each eye. I have tons of pressure now under each eye and bulging eyes totally puffy 18 months later. I am so depressed. I saw 26 surgeons now and can't get help. Nobody wants to take on someone else's mistake. I finally found one surgeon willing to take it out, but he wants to charge me over $50,000 just for lower eyes and he will not take insurance. I am totally screwed suffering emotional damage as well as lid margin roll and exposure caused by the excess fat stretching things out for almost 2 years. I already spent over $10,000 on consultation fees and can't find anyone to help me in this area. This is the worst nightmare. Do you have any eye surgeons you recommend?

I also got fat transfer for my face and have a horrible dent at the side of my left thigh. Also it puffed the corners of my top eyelids and I can feel a bobble under my eyelid. Sometimes it seems better to age naturally as most surgeons, and beauty salons do not think properly, just do what they are taught and that one size fits all. the majority are not up to any standard. I wish you best of luck.

I agree that Dr. Karam is one of the best out there but I have to say that he promotes himself (which he should) and his abilities such that you really want him to be the one who performs this and other facial procedures. The problem I have is most people who are watching this video and making comments cannot afford him. I have no idea what this procedure cost but I am pretty sure it is out of range for most people. He promotes a vertical lift procedure that he does and clearly he is good at it but it will set you back 100K or more!

Thank you doctor Karam for great educational videos! 🙏🏻
Regarding CO2-laser under the eyes, it seems to me that many surgeons prefer this to a skin pinch, and that some even do both for optimal results. Their claim is that the laser will not thin the skin but rather make it thicker and better. When you say skin will become thinner and more fragile after laser resurfacing, do you then see this as a permanent result, or is it a temporary issue? And could laser tightening also result in changing the eye shape?
I struggle with vertical lines right under my eyes that go down my cheeks and therefore can not be successfully treated with Botox.. At the age og 46 I dont have much other ageing issues yet. Have seen different surgeons, but most are sceptical towards the skin pinch saying it will make the eye rounder in shape, and that its very easy to take too much off so the lower rim shows.. Makes me very confused as to which procedure to choose, as everyone has got great before and after pictures to show…

Dear Dr. Karam. You have great skin. However, you should shave the gray beard off. it's making you look much older.

Do you have a video about skin care and diet as prevention for eye aging? Including eye creams that are gentle, safe, and don't irritate the skin around the eyes? Thanks so much!

I had a plastic surgeon ‘grant’ my request for filler under my eyes when they appeared hollow only to be fully disappointed when there was the appearance of bubbles under the skin , following a follow up COVID vaccination . Apparently they was a known reaction . It looked terrible and I ended up having another dermatologist have to correct it by dissolving some of the filler with some injection and making it look smoother . What’s crazy is I was never really consulted on what procedure was appropriate for me , she just followed my request .. but I’m no plastic surgeon 😂

I had orbital surgery for Graves Disease 15 years ago and the end result was a "fat pad" was left below one lower lid that I didn't have before. I went back to the surgeon who performed the surgery and was told that to repair it would be "cosmetic" and not covered by insurance, even though the initial surgery was medically necessary. I'm now 63 and have a lump beneath my eye that I always say "I didn't earn." I have never had the confidence to have someone work on my eye(s) further because I do not want to look even "more different." I wish your practice was in Michigan, as you really seem to know how to keep a face "genuine." Most consults I've had have never even brought that up. Thanks for the info.

Great video, thanks! Great explanation of the aging undereye. I've got the hollowing but also puffiness - glad to know more about fat transfer. Seems like a lot of injectors are just pushing fillers

I was wondering why PRF was not discussed (EZ GEL) as a natural body substance as an option to under eye filler. It seems it is currently 'hyped' as the new preferred natural filler. Great info, thanks.

My practioner wants to do PRP injections under my eyes for the hollows because of aging. Is this not a good tool in the toolbox to consider?

Same question here. I think PRF is the newest and seems to be skill dependent. Seems like it would be best for cases with relatively less volume loss.

Another very informative video. Never knew about the muscles and fat pads, etc involved in the eye socket. The anatomical images explained what you were explaining are very helpful. Fantastic teaching video. Thank you very much for your time Dr. Karam for making this video.

I think the upper eyelids are the most important, droopieness etc. what to do about that short of surgery? As a long time Esthetician I really believe we all age and ageing naturally without going under the knife or injections are best. Plus it makes someone look awful. Great skincare, sun protection and being happy are natural skin anti agers. Not surgery. My mom lived to be 96 and she was so beautiful, no plastic surgery or fillers needed: her soul and heart was filled with beauty, lovliness, creativity and joy. Id rather spend 50k on a nice vacation, new experiences, than plastic surgery sorry doc

Such detailed information is important, the technique he uses makes sense. The fact that he does these procedures everyday speaks to his knowledge. What do we do with this information? How do we find a surgeon that uses his techniques? I don’t think any surgeon would appreciate a patient telling them there’s a different or better way to do surgery. Can any of us afford to pay what Dr Karam charges?

In the time it takes to earn the money for muh procedures,
Ageing will ahve already given you even more problems.
So the example of the ageing woman is like a hamster running on a wheel until it drops dead from exhaustion.
