Hidden Costs of a TWINS surrogacy journey

  • Intended Parents looking for a large family consider reducing costs by having their surrogate carry twins. The hope is that a twins pregnancy is a sort of “Buy one; get one free” deal. But a twins surrogacy has serious medical and financial costs.
    Twins have a high probability of premature birth (about 65% of twins pregnancy ends prematurely.) Premature births run a higher risk of physical and cognitive developmental problems later in life. For this reason the ASRM recommends that embryo transfers use only one embryo. Also many surrogates will refuse to transfer multiple embryos to intentionally have twins (although would agree to carry twins if they arrive naturally) because of the difficulty and potential risks to their own health.

    Many people try to manage their surrogacy journey and arrange the timing to fit into their normal lives. My experience is that it almost never works out as you intend. Babies have a mind of their own, and they will arrive on their own schedule. The more plans you make, the more disrupted you're going to find yourself. Be prepared to be accommodating (from now, and until they leave for college).

    What can be done if you are international and only have insurance for a singleton when the embryo splits into two(= twins)?

      hello Ralph. I wish I had a good solution. There are insurance polices that will cover twins, but they are really expensive. They are backed by Loyds of London, and can cost $20K or more. There are creative options, but they are not 100% approved by insurance companies.

    Would these costs be added in a surrogacy program in Ukraine or Colombia or are they built into the original price typically? I know that each program differs.

      Hello JF... In most overseas programs (including Kiev and Bogota) there are no additional clinic fees for a twins pregnancy. Also medical complications are covered by national health care (which the US doesn't have), so there is no additional cost of the babies require time in NICU. The only additional cost comes from extra compensation to the surrogate for her added risk and challenges of carrying twins. In those countries surrogates get paid an additional $2500 for a twins pregnancy.

    Question is there a reason you all chose surrogacy in India vs Borgata ?

      Hi Jimmy. The reason is that 9 yeas ago India was the international hotspot for surrogacy. The clinics were great and the price was right. Colombia at the time was in the midst of a civil war, so that was not even on the radar. India has since closed it's borders for international surrogacy -- and other countries step in as the most viable destination.

      That's right. India closed for foreigners in 2013. Now it's only possible for Indian citizens living in India. The most popular (and legally secure) spot outside of the US is Ukraine. (www.sensiblesurrogacy.com/surrogacy-in-ukraine/)

      Hi Melissa. The best country is the United States, which has the most successful clinics and a very secure and supportive legal framework. Of course the US has it's disadvantages -- the cost and the absurd privatized medical insurance market begin the top 2 disadvantages. Outside the US the best options depends on your situation. Ukraine has the best legal framework, but obviously there are problems there at the moment. (Although most Ukrainian clinics and some agencies are reconstituting their programs in the Western part of that country, away from military zones.) Mexico is also an up an coming option with a legal framework created by a 2021 Supreme Court decision there.
