I'm not paying any attention to a 7 day observational study done without proper controls. They were looking for a predetermined conclusion out of bias.

Agreed. If what he said is true, not to take a baseline is an irresponsible as a researcher, but then again, self reporting can be very skewed (...and yes, I know things can get skewed by changing parameters, but not getting baseline info is just plain ignorant). It's similar to saying, "I lost 10 lbs" and not measuring your starting weight.

My father is a diabetic and when he was in hospital he was put on a 'low carb diet ' he was aloud 150 carbohydrates per meal! 🙀 I found this insane, many doctors do not understand what low carb means, now my diabetic 78 year old mother doesn't listen to the doctors, she eats 20 carbs a day only, lots of meat and fat, she doesn't need insulin and her A1C is perfect, 😄

    • @HansenFT

150 what? There is no such thing as 150 carbohydrats. Must be calories or grams, I assume. There is a HUGE difference between 150g of life carbs and 150 calories from carbs. 150 grams of carbs will be a lot more than 150 calories.

I have been doing Keto for 2 years and started carnivore on March 17th 2023. I am feeling absolutely amazing. Joint pains are gone, and I no longer need to go to the chiropractor. Indigestion is gone as well. I started doing press-ups, and in the first week, I did 35 per day, which was 5 more than I usually do. I have steadily increased the number every day and did 70 a few days ago! I can't believe it!
What I added was boron, which was suggested by one of the many doctors I follow on TH-cam. That took it to another level. I am 67 years young and will never eat vegetables again. No carbs, absolutely no carbs. I have not put sugar in my coffee or tea for at least 40 years.
Another effect of this diet is that the plaque on my teeth has completely gone. After about a week, I could feel it coming off, a very strange sensation. Additionally, I no longer have bleeding gums. I only take thyroxine for my underactive thyroid. I hope to get off of it over the next year on this diet.
I think this is far superior than any 7 day trial. But then I’m not a scientist or doctor so what do I know.

@richardly1543 No, even on a strict keto you still eat 20-30 g of carbs because all vegetables have carbs. You cannot avoid them.
Besides, once you have regained your slim figure you can restart to eat every day up to 150 g of carbs without getting thick again.
      That is my experience.

Old Grandpa Jenkins, a venerable relic of Lowcarbville, was as much a part of the town as the old church steeple or the babbling brook that ran through the heart of the valley. His sun-worn skin was an atlas of laugh lines and tales untold, and his snowy white hair caught the afternoon light, lending him an ethereal glow. Seated on his well-worn porch, he overlooked the world with a sparkling gaze as clear as the blue skies above. By his side, like a golden ray of the setting sun, lay Bessie, his loyal Labrador, her fur shimmering in the tender light, eyes half-closed in contentment.
As they reclined in the warm embrace of the afternoon sun, Grandpa Jenkins took a sip of his ice-cold lemonade, a quiet symphony of citrus notes and icy chills dancing on his tongue. His mind spun tales, as the bubbles in his drink ascended in a sparkling procession towards the heavens above, like tiny celestial bodies on an eternal quest.
A ruckus arose from the backyard of the house beyond his porch, rousing Grandpa Jenkins from his contemplation. It was the Thompson twins, their energy as unfettered as the cascading waterfall in the nearby hills, and the notorious redneck kid from the house behind, Billy-Joe. They were huffing and puffing, tossing timbers over the fence into Billy-Joe's yard, their faces flushed with the thrill of their illicit endeavor.
Billy-Joe's parents, upon discovering the wooden intruders strewn across their yard, roared in indignation. They thundered at the Thompson twins, their voices echoing through the serenity of Lowcarbville. When the twins pleaded innocence, blaming Billy-Joe, the young renegade retorted, "You lie, you lie, you lie!"
Grandpa Jenkins chuckled, his heart warmed by the drama playing out. It reminded him of the recent fuss in town over a new study proclaiming that low carb diets caused insulin resistance. Being from Lowcarbville, the town took its carbs very seriously.
He picked up a copy of the report that lay on his table, the words printed as black and white soldiers of truth. Yet, as he delved into the pages, he felt a twinge of familiarity. He saw the Thompson twins in the statistics and graphs, their truth obscured by a swirl of numbers and scientific jargon.
The study's subjects reported their food intake in a manner almost akin to the kids' truth. It was as if the researchers had asked the twins to toss timbers of data over a metaphorical fence without understanding their origins or consequences. And when the findings emerged, voicing the shocking correlation between low carbs and insulin resistance, the townsfolk, like Billy-Joe's parents, had a fit.
Grandpa Jenkins took another sip of his lemonade, its icy sweetness cutting through the afternoon haze. He realized that just as the Thompson twins were wrongly blamed, so too were the low carb diets. Both victims of hasty judgment and misinterpretation.
Bessie stirred at his side, her golden coat gleaming with wisdom and comfort. Her gentle eyes seemed to understand the complexity of the situation, embodying the tranquility that comes with accepting that all truths have their shade of gray. She nudged her head onto his lap, a wordless comfort in the confusion that seemed to take root in the hearts of Lowcarbville's inhabitants.
As he scratched behind her ears, Grandpa Jenkins contemplated the way 'facts' could be bent, twisted, and skewed, all depending on the perspective. Perhaps, he thought, the true essence lay in unraveling the many layers of perception, and accepting that truth, like poetry, is often a prism, refracting different shades of reality based on the angle of interpretation.
And with that, he leaned back in his rocking chair, Bessie by his side, ready to witness the endless play of truth and illusion under the vast, cloud-speckled canvas of Lowcarbville’s sky.

And.. I bet you are not HANGRY!!
carbs are a filler.. and it fools the body,., For a minute, then it realizes there's no nutrients.. so... You get hungry again.
Today. I probably will burn 10,000 calories doing some spring cleaning.
I'm eating tuna with Mrs dash and a tsp of mayo. On criminal bread. With potato chips.
But my dinner will be meat and pan seared vegetables, in olive oil.
I eat baked potatoes and sweet potatoes, plantains.. those type of carbs once a week. But only before the work I know will metabolize it. And I start with a jump on my air jogger to prepare my body for the carbs in this bread. Then I sit 15 minutes and then get to 4 hrs of LABOR. WEIGHT BEARING LABOR.
      Otherwise, I don't eat them.
Today, I'll also allow myself some ice cream and granola with pure maple syrup or honey.
      TREATS .. RARE TREATS..
JUST BECAUSE I DO NOT DEPRIVE MYSELF OF A CRAVING. I eat it, figure out what my body is really craving, and eat that or take a supplement.
I was diagnosed with 2diasbetes 5 years ago.
I did metformin, nutritionist, and more exercise but in a particular order and began eating BREAKFAST. PROTIEN, which I never did for years. I'd eat at like 3.
I broke down into English what the nutritionist said.. after she said no one listens to her anyway, but it's her job so she did her speel.
I showed her my interpretation with visuals.. she said that's right. So I have her my visuals and told her she should explain it to all patients like that.
6 months later, when I saw her again, she said her patients were doing fantastic.
Her job description, in medical terms was shutting off the patients and made it sound like too much work. And too confusing throwing terms they never heard so couldn't understand without a medical dictionary. And I suggested they WANT dieticians to do this so patients fail and stay in the system.. $$
She never thought of it like that.. but agreed, as she had been spoken to after her patients started doing well and getting off metformin, which she was honest with me about at go,, " it is a gateway to insulin"
But used short term, in the right routine, one can be off it in 6 months, like Prednisone, it should be a taper dosing.
I was her first ever patient to go on and off it in 8 months.
      Never had issues since.
Meat and veg.. greens.. and I love my foods. I have a lot to choose from and never rotate the same foods weekly, I start by what's on sale..

I believe it’s about balance. When I’m in the rainforest doing Ayahuasca and being that aligned to nature we eat rice and plantains etc. I truly believe it’s a mix. If we look at ancient tribes our diets were never consistent. Some days all we ate was the meat we caught or just fish then there were days it was fasting or then veggies and fruits. Intuitive eating plays into that as well too. Our world is so disconnected to their bodies and we have been conditioned into regimented ways of eating and controlling diet. When I am my healthiest is when I am aligned to nature and following nature cues. Whenever I return to the city the food first of all tastes completely different. The chemicals and falsity of how we cultivate food disconnects our natural energetic connection to food. There is a link. I have only learned this through the shaman path and after years of digestion issues due to an autoimmune disease that attacks my liver. I am now almost 50 and at my healthiest with a range of fasting different times a week and mixing my foods between carbs and fat. I would suggest going and study with some shamans outside of our society because they have wisdom beyond these research papers

I was completely Keto for three years and then fell pregnant, and on a glucose tolerance test I was found to be barely over the mark for my first thing in the morning fasting glucose test. (In Ireland, this was a 5.1), although the glucose one hour after each time part of the test I easily passed. I was classified as "gestational diabetic" and made to go to a talk with some other pregnant women with information from the 1970s on how to deal with it. Hugely frustrated, I found taking a small snack last thing at night ended the issue. I had perfect blood sugar readings for the rest of my pregnancy (I was 45), and after the birth, I heard about cycling off and on Keto every 3-4 months. I then cycled off keto and then back on every 3-4 months for a couple of months. Because of being diagnosed "gestational diabetic" during pregnancy, I have to have yearly blood tests now. Everything gets checked on them. EVERYTHING IS PERFECT. My blood glucose was 4.8 this year. I am 47.

I appreciate the point about dietary history before this one week study, which is insane anyway because of detoxing. I tried clean Keto twice and both times it made me severely ill (second time triggered severe IBD flare up and I was in fetal position in agony for 6 months), however that is not because keto is bad but because I have so many auto immune conditions that conflict with one another that I need to figure out a way to get to a place where I can handle something like Keto (the book dirty genes is a great explanation for why healthy foods can backfire and how to work towards a body that can handle it).
For example, keto made my skin and brain feel much better, but my IBS&IBD couldn't handle all the cruciferous veggies and clean oils I was having each day. My body CRAVES protein and I do much better on high protein diets, but I have severe ME/CFS so because my mitochondria is malfunctioning, all my organs are sluggish/oxygen-deprived and I struggle to digest food in general. Starches are easier for people with ME, but they also trigger fungal infections for me. So, my body's immediate reaction to something does not determine how good or bad something is, only that I'm sensitive in this moment. The best option for me is the Auto-Immune Protocol diet (AIP) which is paleo. That doesn't mean it is better than keto, just that it is the one that causes the least suffering lol. So my point is, with such a small sample size in that study, it's hard to know what other factors could misrepresent the efficacy.
On the flip side, despite all my illnesses, my bloodwork has always looked insanely good and baffled doctors - so I can present as being super healthy when clearly I'm not. So I agree, the FULL picture needs to be presented for each person in a study. What is the disease history of these patients? Were there other life changes during that week such as more stress at work? But to be honest, the minute I see a sample size of less than 1k (and even that is small), and that it was only for a week.....yeah, that's not a serious study.

My son has T1D and about 3 years ago he tried going keto/low carb. When he was eating low carb, his blood sugars were perfect but if he had to correct for a low blood sugar with even 15 grams of carbs his blood sugar would shoot up 200 points higher than what it would previously off low carb/keto. He also had a harder time getting it back down. I’ve seen reports from other T1D having the same issue. I’ve read that it could possibly be consuming high amounts of fat overfilling the cells and not allowing the cells to utilize insulin. Definitely would love to see more research especially consuming saturated fat vs unsaturated. Thank you for always keeping us updated on the latest research. ❤

@@RobCGilliam A1C measures average bonded glucose in the blood so if the results are off, either you're not monitoring correctly/continously or there could be an issue related to your blood or something physiological in your blood affecting the test.
When i looked up possibilities i got:
Unhealthy hemoglobin conditions affect A1C accuracy - sickle cell anemia, thalassemia, iron deficiency, blood transfusion, kidney failure, or a similar disease.
Some hemoglobin variant causes A1C error. Most people with a hemoglobin variant does not have any symptoms, and unknowing carry it.
Higher than actual a1c may be due to Uremia and acetylated hemoglobin (massive dose ingestion of salicylates).
Kidney disease, liver disease, hemoglobinopathy, and recovery from blood loss decreases A1C number.
Vitamins C and E supplementation reported lower A1C by inhibiting glycosylation.
Low A1C has an association with Iron-deficiency anemia. Baseline HbA1c level in anemic patients was lower than that in the control group. Additionally, a significant increase in the patients' HbA1c levels after two months of anemia treatment.
Low A1C found in diabetic and non-diabetic pregnant women due to short erythrocyte lifespan.
A1C accuracy can also report being influencing by vitamin C and E intake. To avoid errors, avoid taking these supplements for about 24 hours before the A1C test.
Abnormal hemoglobin lifespan causes A1C error. If you have abnormal hemoglobin, then your HbA1C may be low in hemolytic anemia. The life span of RBC is short in hemolytic anemia so HbA1C may be on a lower side. In polycythemia and post-splenectomy patients where the red cell life is longer than usual. Thus HbA1C may be on the higher side. So be cautious when interpreting A1C results in these conditions.

My A1C is not inaccurate. Not sure how that is the topic here. Mutiple times measured by lab tests and twice home tests. The last home test was right at 5.5, the same as the lab test 2 months ago.
My comment was that Keto did not reduce my A1C after 18 months of strict ketosis with intermittant fasting.
In the last 2 months I've tested a vegan diet with moderate whole food carbs, low fat and moderate protein. Low fat because keto blew up my LDL-C levels. Sitting here this morning looking at my CGM the last day and it was in the 80's for my 14 hour fasting period. It barely hit 125 yesterday after meals with 50 g of carbs and quickly came down. • @RobCGilliam
    @RobCGilliam ปีที่แล้ว +10

Good stuff. Sounds like a bad study to be ignored.
We do need studies like this. I just came out of 18 months of strict keto where I started with lots of saturated fats, but the last 6-8 months stuck to more unsaturated (e.g., lots of nuts & nut butter) fats. Fully aware of the view that LDL does not matter in many minds. But mine did go from a lifelong 80's to 180 in 12 months. And my A1C did not budge from the prior period of Mediterranean-based diet. That was a main goal being prediabetic in 2019 @ 5.9 A1C. A 3 month keto test then pushed it down to 5.5. But this 18 months of keto landed me in the exact same spot. So my A1C did not change and my LDL skyrocketed. My worry was how high LDL would impact my vascular system long-term. The risk was too high for me give the lack of research.
So I'm on vegan + fish + some eggs now. Low fat, high protein, moderate carb. And I got a CGM to track this new change in diet. I'm getting days with average blood glucose around 100 which equals an A1C of 5.2 or so... lower than I've seen in many years. Lots of days in the 104-106 range. I feel great. I did on strict keto too for honesty and liked that diet. I'm seeing nighttime glucose in the 80s for long periods of sleep. When I first transitioned 6 weeks ago, I had massive spikes in blood glucose up to 180-200 from bowl of oatmeal. As I've strategically incorporated carbs, these spikes have reduced a lot. They were largest with the first meal of the day on fasted belly. Now I'm eating two big pieces of Daves killer bread toast with eggs whites and my one avocado a day, followed by a vegan protein shake with lots of berries and oat milk and my blood glucose peaks at around 125-130 and then back just below 100.
My 700-800 cal dinners with beans, tons of veggies and fish peak at 125. If I have a lot of beans up to 135, but quickly back down to 100 and then in the 80-90s by 10 pm sleeping.
I do not think low carb causes insulin resistance, but it may mask it. I do not think it solves insulin resistance. You're running on fat and that blocks insulin from unlocking the cells for glucose. Blood glucose only goes down because there is not carb intake. That's cool if you never need to worry about high LDL. I was just not willing to risk cardiovascular disease down the road. Pretty happy now on this mostly plant-based low fat diet. Once my LDL is back to normal, I'll add in more fats and reduce carbs a little, which may further lower my blood glucose and any spikes.

For those who won't read a lot of studies you always want to think about bias and power when really examining whether a study is useful. Observational studies, while not useless, are much more helpful when the population examined is really large (not this study). However, they contain bias as subjects are not blinded etc and can be influenced by the fact that they are in a study. Randomized controlled trials are useful in reducing this bias as subjects and interpreters are usually blinded (i.e. double blinded), and you will often hear Thomas talk about "meta analysis" which basically put together several studies (often can be a mix of RCTs and observational) which as you would imagine increase the power due to larger numbers and are considered higher quality. Regardless, good stuff as usual Thomas. Thanks

Kind of matches my story John. Keto did push my A1C down from 5.9 to 5.5 in 4 months and I lost 25 lb down to an ideal weight for me of 170. A1C of 5.5 was too close to prediabetes, so in the last 18 months I did even stricter keto and my A1C did not budge, but my LDL doubled. Decision time. Trust that high LDL was no big deal and keep keto even though it was not helping lower avg blood glucose, or try something different. I did like the cleanish keto. Main drawback was constant electrolyte problems and cramping while surfing.
I moved to low fat pescatarian plant-based diet and cramps disappeared and after less than 2 month I got crazy lean. This is telling me my LDL is improving drastically as well. And this new CGM technology is showing glucose dropping in the 80s all night and now barely spiking after meals. If all days were like yesterday according to my CGM, my A1C is headed towards 5%.

I knew that from the get-go. if you hardly ever eat carbs, your insulin secretion is low and the effect of your insulin is also botched. best thing is eat what you like, in moderation. Hello, isnt that what your parents always told you (those of you who are age 45-75) also, you can reverse insulin resistance simply by hard exercise if you can do it, thats how i fixed mine back in the day, and you can also just lose 20 to 30 lbs, that works as well, and there's a diet that is almost 100% carbs, called the starch solution, that also works. carbs are not the enemy. while you can fix some things with low carb, you have to stay low carb, and that's not enjoying your life in my opinion. This is for IR and type 2. type 1 is altogether different and is caused by not enough insulin secretion. type 2, often is caused by longstanding IR, whereby your insulin just isnt working even though you release a lot or enough. you can also stop eating for a few days to reverse IR. see Dr. Jason fung, that's his M.O.

The best diet is intermittent fasting. No depravity of food, just limits on how often you can eat. It’s nearly impossible to eat all your calories in 1 setting. As well, after a few days your appetite will naturally decrease. Just eat all you want in 1.5 hours, just make sure it’s a balanced diet not cupcakes. Yes, you can include dessert. After you hit your goal weight, divide your meal into 2 meals if you wish.

Good quality studies are extremely rare. Here is a perfectly fine alternative explanation with exactly opposite conclusion: people with insulin resistance tend to naturally eat lower card diet, e.g because too much carb makes them sick or other ways their body has some sort of signal pointing to the problem. Also, do I get it right that like half of the group of accidentally picked 120 people were insulin resistant (7 days of not..)? Or we are talking about 5 people out of 120 who were the low carb part of the gorup?

Hi Thomas, I'm with Richard Horton, the editor of the Lancet. Here's what he says about studies:
“The case against science is straightforward: much of the scientific literature, perhaps half, may simply be untrue. Afflicted by studies with small sample sizes, tiny effects, invalid exploratory analyses, and flagrant conflicts of interest, together with an obsession for pursuing fashionable trends of dubious importance, science has taken a turn towards darkness”

The study for 'Low carbohydrate intake correlates with trends of insulin resistance and metabolic acidosis in healthy lean individuals' was funded by Kuwait Foundation for the Advancement of Sciences. The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest.
The authors mostly come from the Dasman Diabetes Institute and I did see some indications they promote a vegetarian (bordering on vegan) high fiber diet. The same diet that gave me chronic constipation! It would also appear that might be a conflict of interest with a true low carb / keto diet, which is often high animal flesh/fat.
If someone was consuming up to 45% of energy from carbs, that implies they are on a mixed macro diet (often carbs & fat) and that will cause activation of the Randle Cycle, which is probably not a good thing.
I've never done Atkins diet but I thought that started as proper low carb / keto then progressed towards mixing some carbs back in to the diet, so ultimately it was not low carb / keto? The study did not attempt to analyse the actual diets that were used and admits they did not check, so it might be a weakness in the results.
They do comment on 'low' carb causing metabolic acidosis. Having metabolic acidosis comes with a long list of possible symptoms and I suspect most people on long term true low carb do not have any of those symptoms because they often feel better when low carb, not worse. I have never had any of those symptoms, except not wanting to stuff my face with food throughout the day and lower appetite is a symptom.
I don't know who to believe these days. I found low carb / keto appears to have positive benefits for me with zero obvious negatives, whereas I know higher carb was causing issues, so I stay with low carb (
