With the antenna being external, you can package the receiver in a small form factor so it can be embedded inside vehicles or other objects that you want to track, and the antenna can be mounted with a clear view of the sky. Unfortunately, the antenna is not something that you can omit or reduce in size.
GPS antennas in smartwatches are a great challenge and require very precise engineering and planning. Most mini modules with integrated antennas just slap the ceramic antenna on the "back" side of the module to try and reduce the footprint. There was a module announced called Nano Hornet that was supposed to be 10x10mm GPS module with integrated antenna but its company went out of business as far as I know.
