  • It’s Compilation Thursday! Every week I’ll post a few of my favorite videos along with some behind the scenes Glaucomtrivia
    00:00 - Anesthesia On/Off Switch
    Most of my operating room themed videos come from real life experiences I’ve had in my career. As a 3rd year med student, I was assigned to do 6 weeks of vascular surgery (or hell, as I like to call it). At the end of a particularly long case involving suturing every blood vessel in the entire body, the surgical fellow was growing impatient with the speed in which the anesthesiologist was waking the patient up from a 7 hour case. The surgeon made a grave error he hopefully never made again. He asked, “what’s taking so long?” This prompted a particularly menacing drape glare from the anesthesiologist, who then replied “you know I don’t have an on/off switch back here.” We all laughed. 10 years later I turned this interaction it into a skit which is now my most popular video to date.
    00:46 - Surgery Add On
    I try my best to avoid tired medical joke cliches that everybody has heard a million times. However, some universal truths are unavoidable: Radiologists being dark adapted, slightly anti-social doctors, neurosurgeons never seeing their families, and anesthesiologists doing puzzles to pass the time in the OR. The only thing I regret from this video is the surgeon not asking for more add ons in exchange for the Dell Sudoku Spectacular, a puzzle book I found at my local gas station. He could have gotten much more, it was not a fair trade.
    02:00 - Surgery and Anesthesiology go to Therapy
    I love making therapy videos. The psychiatrist looks and sounds like a real therapist, but he is pretty terrible at his job. It’s what you’d expect from an ophthalmologist acting like a psychiatrist with zero psychiatric training. The charge nurse hat steals the show here. I found that hat at my local surgery center. There were about 30 different flower hats. I picked the most flowery. It contrasts well with the prickly demeanor of your average charge nurse. I try to work it into as many skits as possible. I feel most powerful when I pretend to be the charge nurse bossing myself around in a pretend operating room.
    03:56 - Anesthesia Helps Out Emergency Medicine
    This is part 2 of the emergency department turning doctors into different kinds of doctors. I am a big horror fan, so a fair number of my skits have horror undertones. My empty clinic at night sets the perfect stage for these. This skit idea came from an anesthesiology follower on twitter who posted about getting pulled into a trauma, then looking around and realizing the EM doctor who asked for help was nowhere to be found. The patient was now his. People in medicine have the best stories.

    What amazes me the most is how universal all of this is. I am an anesthesiologist from Brazil and these skits are 100% true here as well

      @@DGlaucomflecken On topic... sort of... if a patient gets general anesthesia for eye surgery, where does the gas passer sit? I know it's normally at the head, but that would be in the way.
      Asking for a friend who will be having long-overdue surgery for a superior oblique palsy...
      Also, would I--oops, he--be out of line to give a Sudoku book to the Milk of Amnesia dripper?

    I finally watched ER for the first time and the new ER chief came into the diner wearing bike gear and a helmet. He hadn’t even introduced himself yet but I already knew he was emergency medicine and I screamed hahahah. Best day ever.

    Used to be an OR Assistant in the ASC at Stanford about 25 years ago. After this one case finished, surgeon had left the room and Pt was waking up, she started moaning and saying "Oh, Keith, Keith. I love you...blah, blah, blah." Everyone still in the room snickered and thought it was real funny. Everyone except anesthesia, who acted more surprised and shocked. As she was leaving the room with the PT, anesthesia turned back toward the the Tech and I and whisper, "Her husbands name is Fred! I met him in preop!"
    Busted!

    I had a cesarean with spinal anesthesia 2 wks ago. Surgery was less than an hour long and I was awake throughout. The surgeon even spoke to me. And you know what? Halfway through, the anesthesiologist tapped out and a colleague took over AND I IMMEDIATELY THOUGHT OF YOU.

    I think you should do a video about how PAs and surgeons are trained in school to go far out of their way to hand the anesthesia provider the drape adhesive trash like they would be offended if they weren’t involved in the surgical draping process. 😊

    I was hoping there would be a skit next time when the anesthesiologist is with the ophthalmologist while doing phacoemulsification with IOL because the patient requested to get IV sedation but Jonathan eventually did it

    Oh man, relatable. I delivered general pharmacy and anesthesia gases (desflurane and isoflurane etc) to OR for pharmacy services.
    One time, I was theater side for a very long heart transplant, coolest thing I ever saw. 16 hours in, one of the CTs and the anesthesiologist were like, “hey, the dinning room opens back up at 2am for call staff, can you take my card and just like get me some snacks, I gotta trade out in a few hours but they’ll be closed then. They REFUSE to let us have snacks.”

    Serious question. Why do anesthesiologists have something blocking their view?

      I believe it’s to block the *patient’s* view, not anesthesia. You know… in case they wake up. (Anesthesia is with the patient’s head while the surgeon’s got the body ;) Wouldn’t want the patient getting a glimpse of the internal remodeling being done by surgery. Or by the OB/GYN making that cut across the belly to fish that baby (babies?!?) out during a C-section. It’s all a bit too traumatizing! Especially when heavily medicated.

      It's for hygiene. And also for a lot of operations we don't need to see every single step of the procedure. I can go around the drape or glance above it.

    Skit 1: Right now in my country anaesthesiologists spend most of their time on their phone. I have no patience to update my collegue who is playing candy crush on the ongoing procedure because he or she's too unprofessional to put their phone away and actually check on their patient and the ongoing operation. I know that a Traverso-Longmire that's going well is long and boring for the anaesthesiologist, but this still pisses me off because I image the family of my patient expecting him or her to be well taken care of during the procedure and they surely aren't picturing that.

    I get these are skits, but regarding the first one, that is really on anesthesia; not surgery.
    A good anesthesiologist will follow along with the surgery and know when the end is approaching.
    If you just sit behind the curtain twiddling your thumbs; completely oblivious to the procedure; you’re gonna have a bad time.
    Thats on you, anesthesia! That’s on you!

      Absolutely not. Anesthesia has much more important things to focus on. Like keeping the patient near death without letting him die so the surgeon can play god. It’s up to Surgery to keep Anesthesia abreast of the progress so he can begin bringing the patient back to life at the appropriate time.
      It’s all Surgery’s fault.

      I'm with Gas on this one, sorry. When I was in med school I saw an anesthesiologist reading Money magazine; that's when I realized what they do with their time. Gotta stay on top of your finances! (BTW, I'm family med, glasses are appropriately askew, time not my own, forget about money altogether)

      No, the beauty of anesthesiologists is that they can sit behind the curtain solving Sudoku AND follow along surgery and know when to start waking the patient up. Rumor has it they do this based off of the grunts and curses of the surgeons.

    Is anesthesia one of the only specialties who doesn't get curb sided regularly? How can you "lay eyes" on a patient that needs to be incubated and sedated 😂
