No, you add all details on one printed page, unless the additional info exceeds one page

Then here comes the beneficiary who has used other names and space not enough, will I use the same page or print a different one to add the other names the beneficiary has used?

I have a friend who won green card and he was told to get form I864 form filled out. It’s so tricky and no one is willing to do it for him. I guess they gave him the wrong form for diversity visa. Now he will not process the visa just because of such unnecessary form.

Disclaimer: the form is supposed to be filled by the sponsor. You choose to fill part 1 just to lessen work for the sponsor coz you know yourself better. SO still you choose " for another individual"

You will use form I-864 form. That's what you will send them in your case.
How can a mailing address and a current address be the same? I assumed that your mailing address would be your permanent address in the United States, while your current address is where you are now.
Remember the form I-134 is supposed to be mailed to you by your sponsor while still in your country.

Hi! My fiancé (us citizen and petitioner) lives with his mom and his little sister (she is 20). I will move in with them once i will receive the visa. My question is what is the household size? Is it 2 or 4? His mom is disabled and his sister works. He does not claim them on his tax return. But i’m still confused if the household size is 2 or 4 bc they do live in the same house. Thank you!

It's the sponsor to fill it. However, download the form, fill your information then send it to the sponsor to fill his/her part. The sponsor then prints it, signs , scan and send it to you
This form is only go for one sponsor unless the joint sponsor is a spouse or kid.
