As in your video.. u said carvedilol is non selected beta blocker thats why it also blocks the alpha 1 mediated receptor of blood vessels so due to which ejection volume is decrease in stable CHF .. but what about metoprolol which is selective beta 1 blocker and no action on blood vessel inspite of its negative ionotropic affect on heart which exacerbate tha condition.. Thank you very much! I do have a question. In my notes, I have that Beta blockers are contraindicated when there is signs of heart failure. Is this referring to the decompensated acute heart failure patients that your'e talking about?
Yes, beta blockers should not be given in acute decompensation of heart failure. But if the the patient is already taking beta blockers then they should not be stopped. Instead give diuretics. If there are contraindications of beta blockers like asthma or AV node block for example, then then they should of course not be given. Look at the video one more time and you will see that I talked about this :)
